Effect of critical thinking to optimize nursing process for patients with acute myocardial infarction

Abstract:
Objective To explore the effect of critical thinking in the care of patients with acute myocardial infarction.Methods During the period from January 2010 to February 2010,nurses in CCU received short-term comprehensive training to strengthen critical thinking,and their improvement was compared before and after.Decision tree was used to apply critical thinking to develop optimized nursing process for patients with acute myocardial infarction.Patients with acute myocardial infarction in CCU from September 2009 to December 2009 and from March 2010 to July 2010 were divided into the intervention group and the control group.The control group received conventional nursing procedures,while the intervention group received optimized process and follow-up for 3 months after discharge.Patients' clinical results,satisfaction about nursing,nursing document quality,patients' life quality,compliance and KAP status were compared after nurses' critical thinking was improved and the nursing process was optimized.Results The total score of nurses' critical thinking ability was (310.63 ± 0.347) after short-term training,higher than (289.75 ± 6.532) before training,and the difference was statistically significant (t =3.487,P <0.05).Except systematic ability and cognitive maturity,other dimensions of critical thinking ability had all been improved significantly (P <0.05).There was no significant difference of clinical results between the intervention group and the control group (P > 0.05).Patients satisfaction about nursing and changes in life quality,compliance,knowledge and behavior were all better in the intervention group than in the control group (P < 0.05).Conclusions Comprehensive short-term training to strengthen critical thinking can improve nurses' critical thinking ability to a certain extent.Application of critical thinking in nursing process for patients with acute myocardial infarction can improve patients' nursing satisfaction,life quality,compliance,knowledge and behaviors,but still needs further study on the clinical outcome.
