Abstract:
Objective To evaluate the efficacy of nursing in percutaneous coronary intervention (PCI)for a patient with myocardial infarction and heart failure under the support of extracorporeal membrane oxygenation (ECMO). Methods An 85-year-old man suffered from myocardial infarction twice during 1-month hospitalization and complicated with a severe heart failure (eject fraction is 27%), pleural effusion and pulmonary infection. We decide to perform PCI under the support of ECMO for his hemodynamic instability. The key point of nursing in PCI under the support of ECMO: On the one hand, pre-PCI interview and preparation of all the drugs and items needed in operating ECMO 1 day in advance were key steps to guarantee the successful operation. On the other hand, high standard nursing technique, mastery of knowledge about ECMO and tacit cooperation with doctor were the bases of the successful operation. Results Under the support of ECMO, the patient' s hemodynamic condition came to stable. He underwent the coronary angiography and was implanted a stent in left coronary artery successfully under the meticulous care of doctor and nurse. Conclusions ECMO could provide life support for patients with severe heart failure. The adequate preparation for ECMO could guarantee the successful operation for nurse in catheter lab. Both intra-operative warming and prevention of postPCI fever caused by cold stimulation are important nursing measures.
