The application of the early goal-directed therapy to severe sepsis and septic shock

Abstract:
Objective To investigate the effect of implementing the early goal-directed therapy on patients with severe sepsis and septic shock.. Methods Thirty inpatients of study group admitted from June 2005 to July 2009 were compared with 31 patients of history control group admitted from June 2001 to December 2004 in the indicators containing therapy effects, organ function and mortality. Patients in the study group performed the early goal-directed therapy. Results There were no significant differences between two groups in base-line data. However, the rate of repeat resuscitation decreased from 64.5% in the control group to 16. 1% in the study group. Meanwhile, at 7 days after resuscitation, the indicators including PaO2 and PaO2/FiO2 in the study group were significantly increased while the indicators containing transaminase and serum creatinine significantly decreased. In addition, mortality dropped from 48. 4% to 33. 3%. Conclusions The early goal-directed therapy improves outcomes of patients with severe sepsis and septic shock.
