Effect of Anticoagulant Treatment on Pulmonary Hypertension in Patients with Intermediate-Risk Pulmonary Embolism

Abstract:
Objective:To explore the therapeutic effect of anticoagulant treatment on pulmonary hypertension(PE) in patients with intermediate‐risk PE .Methods:A total of 65 patients with intermediate‐risk PE admitted to Zhongshan Hospital ,Fudan University during Jan .2008 and Dec .2013 were chosen .According to the disease course on admission ,39 PE patients within 70 d were allocated to group A ,and 26 PE patients over 70 d were allocated to group B .All patients underwent Doppler ultrasound Heartbeat graph for detecting the mean pulmonary arterial pressure(mPAP) ,as well as D‐dimer test ,on admission .Patients in two groups received the same anticoagulant treatment ,and the mPAP value was detected again as comparative index after treatment .Results:In group A ,the mPAP after treatment decreased significantly while compared with that before treatment (P<0 .01) .There was no significant decline regarding mPAP in group B(P> 0 .05) .D‐dimer in group A was significantly higher than that in group B on admission(P<0 .05) .Conclusions:D‐dimer significantly increases in patients with intermediate‐risk PE within 70 d of disease course ,and the mPAP decreases significantly after receiving anticoagulant treatment .However , the increase of D‐dimer is not significant in patients with intermediate‐risk PE over 70 d of disease course ,and the decrease of mPAP after anticoagulant treatment is not significant .
