Comparative diagnostic value of contrast-enhanced ultrasound and SMI in renal tumors

Abstract:
Objective:To compare the value of contrast-enhanced ultrasound (CEUS) and microangiography (SMI) in differential diagnosis of benign and malignant renal tumors.Methods:CEUS and SMI images of 241 patients with renal tumors (221 malignant tumors and 20 benign tumors) confirmed by surgery and pathology were retrospectively analyzed. The consistency of the two methods for the differential diagnosis of benign and malignant renal tumors with pathological diagnosis was analyzed. Receiver operating characteristic (ROC) curve was used to compare the diagnostic efficacy of the two methods.Results:In CEUS, the main manifestations of renal malignant tumors were fast forward (155 cases, 70.1%), fast regression (194 cases, 87.8%), and high enhancement (181 cases, 81.9%), while the main manifestations of renal benign tumors were slow progression (11 cases, 55.0%), slow regression (9 cases, 45.0%), and low enhancement (14 cases, 70.0%). There were statistically significant differences in perfusion mode, enhancement degree and regression mode between the two groups ( P<0.001). According to diagnostic criteria of CEUS, 224 cases were diagnosed as malignant tumor and 17 cases as benign tumor, the consistency with the pathological diagnosis was moderate ( Kappa=0.62, P=0.581). In SMI, the Adler grade of renal malignant tumor was mainly 2-3 (196 cases, 88.7%), while the Adler grade of renal benign tumor was mainly 0-1 (18 cases, 90.0%), the difference was statistically significant ( P<0.001). According to the diagnostic criteria of SMI, 215 cases were diagnosed as malignant tumors and 26 cases as benign tumors, which were moderately consistent with pathological diagnosis ( Kappa=0.57, P=0.238). The areas under ROC curve of CEUS and SMI for benign and malignant renal tumors were 0.789 and 0.823, respectively, without statistical significance ( P=0.711). Conclusion:CEUS and SMI have different advantages in the diagnosis of renal tumors. SMI is a simple, non-invasive and inexpensive and effective method for the diagnosis of renal tumors.
