Abstract:
Objective To investigate the differences in the concentration of oxcarbazepine (OXC) and the clinical efficacy and safety of OXC between Xinjiang Uygur and Han nationality.Methods Ultra performance liquid chromatography was used to measure the steady valley concentration (Css) of 10-hydroxy carbamazepine (MHD) in 400 cases of epileptic children orally taking oxcarbaze-pine,the influence of race on Css was analyzed,and the curative effect and adverse drug reactions were observd,and finally the relationship between therapeutic effect and blood drug concentration was investigated using Logistic regression and receiver operating characteristic curve (ROC) analysis.Results The correlation of OXC dose (D) and blood drug concentration in the treatment group and control group were (r =0.361,P < 0.01) and (r =0.627,P <0.01),all were positively correlated,and the control group was better than the treatment group.The effective rates in treatment and control groups were 88.29% (171 cases/205 cases) and 89.23% (174 cases/195 cases) without significant difference (P >0.05).The Css of valid and invalid groups in treatment group were (19.87 ± 5.39) and (12.96 ± 8.25)mg · mL-1,and in the control group were (18.76 ±5.61) and (8.46±5.91) mg · mL-1,and the differences were statistically significant (all P < 0.05).The area under the curve AUC that D as predictors of diagnosis in the treatment and control groups were 0.765 and 0.927,and the area under the curve AUC with Css as predictors of diagnosis were 0.494 and 0.517,two groups of Css for curative effect in children with forecasting accuracy is better than D.The clinical efficacy can be better guaranteed when the children' s Css is greater than or equal to 9.90 mg · L-1 in treatment group and greater than or equal to 9.56 mg · L-1in control group.Conclusion Monitoring the blood concentration of OXC can improve clinical efficacy and avoid adverse drug reactions,providing a basis for the development of individualized drug regimen for children with epilepsy in Xinjiang.
