Traumatic dislocation of the radial head without fracture of the olecranon is very rare, especially in adults. We experienced a case of irreducible radial head dislocation with fracture without involvement of ulna. Open reduction and internal fixation was performed. During surgery, brachialis was interposed between capitellum and radial head, and also interposed between the fragments at the fracture site of the radial head. At 12 months after operation, the radial head was well reduced with normal rotation.

PURPOSE: To evaluate the clinical results of Acutrak screw fixation for ulnar olecranon fractures. MATERIALS AND METHODS: We reviewed 15 cases of ulnar olecranon fractures which were treated with Acutrak screws from February 2003 to September 2007. Follow-up period is from 12 months to 42 months. We used Mayo classification. Radiologic results were analyzed according to step-off, gap, reduction loss, and functional results were analyzed according to pain and ROM. We analyzed union time, operation time, incision size and complications. RESULTS: In functional results, there were 3 good cases out of 3 Mayo type IA, 8 good cases and 2 fair cases out of 10 type IIA, 1 fair case and 1 poor case out of 2 type IIB. In radiologic results, there was 1 case of reduction loss. Average union time was 9.4 weeks, average operation time was 24 minutes and average incision size was 1.8 cm. CONCLUSION: We conclude that Acutrak screw fixation can be a treatment option for olecranon fracture of Mayo type IA and IIA.

Anatomically olecranon fracture is an intra-articular fracure, so reduction must be exact. Using the principle of Zuggurtungsosteosynthese by Weber, Pauwels' clinical study of olecranon fracture showed the superiority of tension band. 15 patients with olecranon fracture of the ulna who were treated with tension band and could be followed at the Department of Orthopedic Surgery of Han Il Hospital from January, 1974 to December, 1980 have been reviewed. The result obtained is as follows. We obtained the more rigid fixation with modifying the standard tension band locating the end of K-wire in the cortex of opposite side cortex of ulna. So, the result of treatment was satisfactory.

We have experienced multiple fractures of forearm both bones, which revealed the following fractures: comminuted fracture of olecranon, short oblique fracture of proximal ulnar shaft, transverse fracture of ulna mid-shaft, comminuted fracture of radial head, comminuted fracture of distal radius.

PURPOSE: The purpose of this study is to evaluate the surgical outcomes of olecranon fracture dislocation in accordance with the direction of the dislocation. MATERIALS AND METHODS: From December 2006 to February 2016, the surgical outcome in patients who had been followed-up for a minimum of 6 months accompanied with olecranon fracture and elbow instability were reviewed retrospectively. We classified olecranon fracture as either the anterior type or the posterior type. Moreover, we evaluated the clinical results by the Mayo elbow performance scores (MEPS) and checked for any associated injury, age, injury mechanism, and complication. RESULTS: Fourteen patients had anterior transolecranon fracture dislocation, with an average age of 46 years. The associated lesions were radial head fractures found in 2 patients (14.3%) and coronoid process fracture found in 5 patients (35.7%). Patients with anterior transolecranon fracture showed an average MEPS of 93.2 (70–100). Eight patients with posterior olecranon fracture dislocation had an average age of 66 years (22–87 years). The associated lesions were radial head fractures in 6 patients (75.0%) and coronoid process fracture in 8 patients (100%). Patients with posterior olecranon fracture dislocation showed an average MEPS of 94.4 (80–100). In comparison with the anterior type, posterior type occurred more frequently in elders and showed a greater association with injuries, such as radial head fracture and coronoid process fracture. However, there was no significant difference between the two groups with respect to the clinical outcome. CONCLUSION: There were differences in frequency of associated injuries and age in accordance with the direction of olecranon fracture dislocation. Moreover, good clinical outcomes were achieved by surgical treatment.

PURPOSE: The purpose of this study is to evaluate the clinical results of locking compression plate (LCP) fixation for olecranon fractures with proximal ulna comminution. MATERIALS AND METHODS: We review 10 cases of olecranon fractures with proximal ulna comminution treated with LCPs from August 2011 to August 2013. Follow-up period was from 12 months to 18 months. Mean age was 63.1 years (35-84 years). According to the Mayo classification, there were eight type IIB, and two type IIIB fractures. We used Mayo classification. Clinical evaluation was performed based on radiographic union of olecranon and measurements of range of motion at last follow-up. Disability of the arm, shoulder and hand (DASH) score and Mayo elbow performance score (MEPS) were used for evaluation of functional recovery. RESULTS: All patients had bone union. According to the MEPS, nine of ten patients had a good or excellent outcome. The mean DASH score was 18.6. All cases started postoperative range of motion (ROM) within 14 days. Elbow ROM was more than 110degrees in all cases except one. Mean radiological bony union time was 4.2 months (2.5-6.0 months) postoperatively. Complication was hardware irritation in three patients. CONCLUSION: Internal fixation using LCP for olecranon fractures with proximal ulna comminution can be a good treatment option which obtains good clinical results and enables early ROM.

Numerous advances have been applied for the management of hemophiliac patients and the life expectancy of them was markedly improved. But they have many problems yet such as transmission of infections, adverse reactions to plasma products, development of factor VIII inhibitor and inadequate coagulation in the medical and/or surgical treatment. Anesthetic care should be managed with gentle, atraumatic intubation and careful positioning of patient pointing to the prevention of bleeding from hemorrhagic diathesis. We present a case of hemophilia A who had in operation for olecranon fracture of left ulna. It is important that anesthesiologist should realize that certain pathophysiological changes, especially coagulation disorders, might be occurred during surgical procedures and that perioperative preparation and management for them should be kept well.
