7.Hairy Cell Leukemia: A case report.

Hairy cell leukemia is an uncommon lymphoreticular disorder which primarily involves bone marrow, spleen, and peripheral blood. Patients, mostly men, present with splenomegaly and pancytopenia usually. A 62-year-old man was admitted with an abdominnal mass which had grown slowly for 20 years. On physical examination, an enlarged spleen was palpated without tenderness. An abdominal CT scan showed a diffusely enlarged spleen, which measured 20 cm in greatest dimension. In the peripheral blood, many atypical lymphocytes with abundant, delicate, surface projections were noted. They had tartrate-resistant acid phosphatase(TRAP) activity. Thrombocytopenia (60,000/mm3) was observed in the complete blood counts, Other laboratory data were within normal limits. He underwent splenectomy. The submitted spleen measured 26x15x5 cm and weighed 2150 gm. It was well encapsulated and the outer surface was smooth. Cut surfaces were diffusely dark-red. White and red pulps were indistinct. There was no mass-like lesion. Microscopically, the spleen consisted of monotonous mononulcear cells which involved red pulp. The white pulp was diminished, and could be barely recognized. Cells had small round nuclei and abundant cytoplasm. Ultrastructurally, cells with numerous slender surface projections were noted. In Korea, hairy cell leukemia is exceedingly rare. We report a case of hairy cell leukemia with characteristic pathologic features of spleen as well as those of peripheral blood.

8.Correlation between the Penile Size and Prostatic Volume.

PURPOSE: To evaluate the clinical correlation between penile size and prostatic volume. MATERIALS AND METHODS: A total of 196 men were included in this study, 96 patients diagnosed as benign prostataic hyperpalsia(BPH, mean age: 67.2+/-18.8) by transurethral ultrasonography(TRUS), and 100 healthy volunteers(mean age: 48+/-16.2). The stretched penile length and penile circumference were measured by one examiner. We compared penile size with prostatic volume. BPH patients were classified as group I(30~39 ml), group II(40~49 ml) and group III(above 50 ml) by prostatic volume. RESULTS: For penile length and penile circumference, the mean values were 12.3+/-2.0 cm and 9.1+/-1.1 cm in group I, 12.6+/-2.2 cm and 9.2+/-1.0 cm in group II, 13.9+/-2.7 cm and 9.8+/-1.2 cm in group III, and 12.3+/-1.3 cm and 9.0+/-1.2 cm in control group, respectively. Compared to the control group, penile size was significantly higher in group III(p <0.05). CONCLUSIONS: Larger prostatic volume, especially above 50 ml, is associated with a significant increase in penile size.

9.Ectopic Hamartomatous Thymoma: A case report.

A 49-year old man was admitted to the Korea university hospital, department of surgery, for evaluation of anterior neck mass which was slowly growing for five years. His past history was unremarkable except for known hypertension for several years. Physical examination revealed high blood pressure, measuring 180 mmHg in systolic phase and 120 mmHg in diastolic phase. A soft nontender mass was palpated at anterior neck just above the sternal notch with smooth surface and its size was about 4 x 5 cm in cross. On laboratory examination, diabetic evidence such as high blood sugar (FBS 170 mg/dl, PP2hr. 234 mg/dl) and glucosuria. The CBC finding suggested polycythemia with high hemoglobin (18.0 g/dl) and hematocrit (54%) levels. The differential count and platelet count were within normal limits.

10.Lymphadenopathy of Kimura's disease Associated with Nephrotic Syndrome: A case report.

Kimura's disease is an important category of reactive lymphadenopathy especially among Asians. It is a chronic inflammatory disorder of unknown etiology and is characterized by angiolymphoid proliferation and eosinophilia. The patient was a 17-year-old man with a 3-year-history of submandibular lymphadenopathy Who was referred to Kyung Hee University Hospital for evaluation of enlarging mass. He had a history of nephrotic syndrome for 7-8 years since 1984. Laboratory findings were not specific except for peripheral blood eosinophilia. The submandibular lymph node showed florid germinal enters, increased postcapillary venules in the paracortex, eosinophilic infiltration, and sclerosis. In this paper, a case of lymphadenopathy of Kimura's disease associated with nephrotic syndrome is reported.
