3.Evidence based evaluation of the infertile male

The goals of the evaluation is to identify; 1) potentially correctable conditions, 2) irreversible conditions but are amenable to ART using male partner sperm, 3) irreversible conditions not amenable to ART, fro which donor IUI or adoption are possible options, 4) life- or health threatening conditions that may underlie infertility and require medical attention and 5) genetic abnormalities that may affect offspring if ART is employed.

6.Meckel-Gruber Syndrome: An autopsy case.

We report an autopsy of a male fetus that showed multiple congenital anomalies that could best be designated as Meckel-Gruber syndrome. The fetus was born dead at the gestational age of 38 weeks. His parents denied any history of congenital malformation. And the parity of the mother was 0-0-0-0, but she had the past history of receiving herb medication for common cold. The congenital anomalies found in this case consited of occipital meningoencephalocele, midline cleft palate, bifid epiglottis, hepatic fibrosis, choledochal cyst, bilateral polycystic kidneys, postaxial polydactyly of both hands and feet, aplasia of the left testis, secundum type atrial septal defect and patent ductus arterious. This malformation syndrome is rare and lethal. The prenatal diagnosis should be made by ultrasound study or analysis of the amniotic fluid for alpha-feto protein during intrauterine period. The kidneys showed Potter type III cystic change and there was a characteristic hepatic fibrosis.

7.Buerger's Disease of Paratesticular Tissue: A case report.

We report a case of Buerger's disease manifested by a scrotal mass, in view of its rarity. A tender bean sized mass was palpated in the left scrotum of a 34 year-old male heavy smoker, who had a history of right pain two years ago which subsided spontaneously. The left testis with the paratesticular tissue was excised. Vessels of the spermatic cord and epididymis showed microscopic changes of Buerger's disease. The involved arteries and veins revealed a predominantly subacute pattern with granulomatous inflammation and Langhans' type giant cells within the thrombi.

8.A Case of Esophageal Granular Cell Tumor.

Esophageal granular cell tumor of is rare neoplesin, and 3 cases were reported in Korea so far. We report a case of esophageal granular cell tumor in a 28 year old man. The patient was a 28 year old man who visited this hospital because of upper abdominal pain, acid belching and weight loss. A small nodule, 5 mm in diameter, was found in mid esophagus on esophagoscopy. This nodule was proved to be a granular cell tumor.

9.Changes of Heart Rate, Bolld Pressure, and Respiratory Rate after Ethanol Administration in the Flushing and Non-Flushing Men .

Cardiovascular respons follwing ethanol ingeation was conapared in two groups of male college studenta; 20 who ahowed visble facial flushing and 25 whe showed on flushing. Ethanol was administered as 25% Korean liquor (Kumbokju) a dose of 2ml per kg body weight, the heart rate, blood pressure and respiratory rate were measured. The reaults areas follows. The heart rate after ethanol ingestion increased significantly in both flushing and non-flushing groups. However, the degree of the heart rate increased in the flushing group was aignificantly higher than that in the non-flushing group. The blood pressure was decreased significantly from 40minutes after ethanol ingestion in both groups. Howyer, the flushing group showed higher systolic pressure, and lower diastalic pressure and mean arterial pressure than the non-flushing group. The reapiratory rate following ethanol administration was increased in the flushing group but decreased in the non-flishing group when compared with the resting respiratory rate. The results clearly indicate that a signifcant difference in cardiovascular resoponrses to ethanol exists the flushing and non-flushing groups. The flushing group shows a greater tachycardia and greater fall in. mean arterial pressure followig ethanol ingestion than the non-flushing group.

10.A Study of Changes ia Norepinephrine and Epiaephrine during Endotracheal Intubation .

This work was done to measure the plasma concentration of catecholamines esp. norepinephrine and epineparine after eadotracheal intubation and so study the interrelation between the blood pressure and pulse rate and catecholamines. All seven patients studied were normotensive men with general anesthesia including eadotracheal intubation, systolic blood pressure, diastolic blood pressure and pulse rate were increased to 36+/-4.72 torr, 31+/-5.71 torr, and 34+/-6.23min. respectively. Norepinephrine and epinsphrine were also increased to 165+/-28.55pg/ml and 88+/-27.97pg/ml respectively. Those results show that the rise of blood pressure and pulse rate after endotracheal intubation may result from increased catecholamines.
