1.Corticotropin-releasing factor and its receptor in gastrointestinal disease

Corticotropin-releasing factor (CRF)is a neuroendoerine peptide that stimulates the synthesis and release of adrenecortieotropic hormone from the pituitary. CRF widely distributed in the body has been implicated in the regulation of endocrine, neural, behavioral responses and has relevance in the the physio- logical effects and pathophysiology of gut. The delayed gastric emptying, inhibited small intestinal transit and stimulated colonic transit are the most common responses evoked by CRF. CRF delay gastric emptying by ac- tivating CRF2 receptor while the stimulation of colonic motility is mediated by the activation of CRF1 recep- tor. Development of antagonists of CRF receptor may treat a new therapeutic strategy for treatment of stress- related gastrointestinal disease.
2.Clinical application of endoscopy on the cholangiopancreatic neoplasms

Fast track surgery(Frs) is an idea and theory which has been highlighted by more and more clinic doctors and scholars, and widely used in the surgical operations recently. ~TS has greatly changed the mode of clinic therapy, ameliorated the velocity of the rehabilitation and prognosis after operation, and improved the quali- ty of life and the therapeutic effect. We reviewed the application of FTS in the surgery in this article.
4.Clinical observation of fluorouracil implanting to abdominal cavity during operation of advanced colonic cancer patients

Objective To explore the safety of fluoreuracil implanting to abdominal cavity during operation of advanced colonic cancer patients, search for new approach for preventing from local relapse of colonic cancer. Methods Divide 60 patients accepting radical colonic cancer surgery into 2 groups, treating group and control group. Treating group are implanted of fluorouracil along the wounded surface of tumor and draining path of lymph nodes. To observe the curing of wounded surface, stomal leak ,wound infection, intestinal obstruction and hospital day. Results There is no significant differences between treating group and control group in stomal leak,wound infection, intestinal obstruction and hospital day (P>0.05). Conclusion It is safe to implant fluoruuracil during radical colonic surgery, which is an effective path of regional chemotherapy after operation.
5.Influence of anisodamine chronic administration on the vascular function and morphology spontaneously hypertensive rats

Objective To investigate the influence of Anisodamine chronic administration on the vascular function and morphology of SHR. Methods The SHR were injected by chronic abdominal Anisodamine; Then the SHR were sacrificed after ten weeks. The abdominal aorta were obtained after perfusion and the changes of aorta morphology were observed under a microscope. The influence of Anisedamine on the vasculovr funtion was investigated by the rat's aorta rings perfusion. Results In the physiological state, given SHR long-term Anisodamine, the vascular endothelial and smooth muscle relaxation will significantly improve; Anisodamine chronic adminstration can inhibit the formation of aortic aneurysm and change a benign vascular structure. Conclusion Anisodamine chronic adminstration will improve the vascular endothelial and smooth muscle relaxation, and change a benign vascular structure, Anisodamine has a protective effect on endothelial and smooth muscle.
6.DNA methylation and gallbladder carcinoma

DNA methylation is a normal modification mode of eukaryon, there is an intimate relationship between aberrant promoter methylation of tumor-related genes and the generation or development of neo- plasms,emerging significantly biological effectiveness. Aberrant promoter methylation of tumor-related genes as the epigenetic markers, maybe play a very important role in the incidence, diagnosis, therapeutic effects, prognosis judgements and other aspects in gallbladder carcinoma.
7.Neoadjuvant chemotherapy for advanced gastric cancer

At present, most of patients diagnosed as gastric cancer with clinical therapy belong to ad- vanced stage in China; The patients in early stage are relatively few. While patients in advanced stage have poor prognosis in general and low rate of curative resection. Neoadjuvant chemotherapy for advanced gastric cancer can obtain downstaging, increase rate of curative resections, and has the possibility of improving sur- vival. Neoadjuvant chemotherapy might be an effective and feasible therapeutic method for advanced gastric cancer.
8.Effect of MMDB on the expression of TLR4 in cholestatic jaundice in rats

Objective To research the mechanism of impairment that cholestatic jaundice is treated as- sistly by MMDB. Methods Obstructive jaundice models with SD rats, which were divided into three time points randomly, namely 7,14,21 days,were established. In each time point, there were three groups(each six rats) : sham operation(SO + NS) ; BDL + MMDB; (BDL + NS). The rats were sacrificed at different time point, in order that liver tissue and inferior vena cava blood were taken out. The concentration of serum glutamate transaminase(AST), albumin(ALB), and total bilirubin (TB) were measured with biochemistry; the expression of TLR4mRNA was measured by reverse transcription PCR. Results On the 7th day after BDL, serum TB,AST level in BDL + MMDB and BDL + NS group were upgrade compared with SO group, in which BDL + NS group were more obviously upgrade than BDL + MMDB group(P<0.01). There was differ- ent to compare between BDL + MMDB group and SO group(P <0.01). On the 14th, 21st day after BDL, serum TB, AST coNTents were to last upgrade in BDL + MMDB and BDL + NS group. It was opposite to the SO group(P <0. 01). Conclusion The level of TLR4 could be weaken by MMDB. It may relieve the im- pairment of liver in cholestatic jaundice by LPS.
9.Laryngeal mask airway general anesthesia combined epidural block in laparoscopic cholecystectomy

Objective To observe the feasibility of laryngeal mask ventilation general anesthesia com- bined epidural block in laparoseopie eholecystectomy. Methods One hundred and forty eases of selective laparoscopie eholecystectomy were performed to T8~T9 gap catheterization, with 1.5 percent lidoeaine epi- dural block, block levels in the following T4. After conventional anesthesia into 4# or 5# LMA, balloon gas was injected in 20 mL~30 mL, manual ventilation, respiratory resistance and the situation thorax ups and downs were observed. Results The patients epidural catheterization smoothly, in the anesthesia plane fol- lowing T4, insert the LMA blood pressure, heart rate without significant change. Pneumoperitoneum after the rebound in blood pressure[(20.6 5.0) mm Hg], heart rate did not change significantly, and then airway pressure increased[(5.7 1.6)cm H2O] , surgery performed smoothly, and quickly regained consciousness after the surgery, when all patients admitted gallbladder, they have resumed breathing independently. Con- dusion Laryngeal Mask Airway general anesthesia combined epidural block cause mechanical damage vocal cords and airway, make the stress response light and the sense of rapid recovery, which is a safe and feasible method of anesthesia.
10.Gene mutation and targeted therapy development of gastrointestinal stromal tumors

Gastrointestinal stromal tumors are the common mesenchymal tumors of the gastrointestinal tract. The constitutive activation of KIT is one of the earlies ttransforming events in GISTs and occurs mainly through activating mutations in the kit gene. PDGFRa is associated with the pathogenesis of GISTs too. The main mutative site of KIT gene are exon11, exon9, exon13 and exon17 and those of PDGFRA are exon18, exon12 and exon14.The clinical parameter and the response to imatinib of GISTs are associated with the site and type of gene mutation. Imatinib mesylate is a small-molecule inhibitor that selectively inhibit the tyrosine kinase and the clinical use of it is one milestone of the targeted therapy of GISTs. We review the gene mutation and targeted therapy development of gastrointestinal stromal tumors in this article.
