Obiective To compare the sensitivity and specificity of strain imaging(SI)and restredistribution Tl-201(RR-201 TI)SPECT imaging in evaluating myocardial viability among patients with STelevation acute myocardial infarction(AMI).Methods Twenty-six AMI patients underwent echocardiography and RR-201 Tl-SPECT imaging a week(baseline)after percutaneous coronary intervention(PCI)therapy. At baseline, wall motion score(WMS)and systolic strain were assessed,SPECT scoring were performed on 16 segments of left ventricle(LV).WMS was repeated more than 3 months later to assess myocardial viability, which is deemed as reference. Results ①Two hundred and sixty segments in the"at risk"regions were followed up for(5±2)months,among which 201 were identified as viable and 59 were not, according to the WMS.②In comparison to the reference standard, the sensitivity and specificity of RR-201 Tl-SPECT to predict myocardial viability were 87%(175/201)and 58%(34/59)respectively.③The sensitivity and specificity of systolic strain(Set)to predict myocardial viability after AMl were 81% and 66% respectively at the optimal cut-off value of-60%.④Set was moderately concordant with SPECT scoring(Kappa=0.40)and the agreement between the two methods was 77%(199/260).Conclusions In ST-elevation AMI,SI and RR-201 Tl-SPECT appears equivalent in predicting segmental recovery after reperfusion, and SI could be a convenient and low-cost alternative for the non-invasive evaluation of myocardial viability.

Chinese Journal of Ultrasonography 1995;4(5):209-211
The measurement of renaI volume by ultrasound was carried out in 488 women with sigleton pregnahey.Sixty-eight women had infants with the intrauterine Browth retardation(IUGR).The average of renal volume in nofmal pregnant females and females with IUGR increased 57.2% and 8.4% respectively during trimester.Hence, the neonatal weight outcomes were cortelated with the renal volume in their mothers. The specficity of measurement was 96%,and the sensitivity was 65%.The sensitivety was influenced by type of IUGR and individual difference of renal vohme. The positive predictive vaIue was 74%.
4.Correlation between Ultrasonography and Pathology in Expansive Type of Hepatocellular Carcinoma
Chinese Journal of Ultrasonography 1995;4(6):247-250
Expansive type of hepatocellular carcinoma (HCC) is a subtype of HCC. Correlation between the preoperative sonography and postoperative pathology was studied in 35 cases of HCC. According to the classification of nakashima, 21 cases belonged to the expansive type whereas the other 14 cases the infiltrative type. Different from the infiltrative type. the expansive type of HCC had four specific sonographic appearances: hypoechoic ring, hypoechcic halo, pedunculated and small nodular echo which had different pathological bases. Expansive tyje of HCC was not so commonly seen as infiltative type but its rate of surgical resection was mu~h higher than that of infiltratine type (JP< 0. 01). Thus, it is the kind of HCC with significant clinical importance. Preoperative ultrasonographic diagnosis help to decide the way of treatment.

Objective To explore the enhancement patterns of pulmonary carcinomas by contrast-enhanced ultrasound(CEUS).Methods Thirty-eight patients with pulmonary carcinomas proven by pathology[28 with peripheral pulmonary carcinomas and 10 central pulmonary carcinoma with obstructive atelectasis(OA)]were examined by baseline ultrasound and contrast-enhanced ultrasound, then the arrival time(AT),time to peak(TTP)were analyzed with time-intensity curve analysis software and the dynamic enhancement pattern of each lesion was assessed. Results Twenty-four peripheral pulmonary carcinomas demonstrated delayed AT about 6-16 s after application of contrast medium, three lesions demonstrated early AT about 4-5 s and one lesion demonstrated absence of contrast enhancement. The lesions exhibited hyper-,hypo- and non-enhancement were 14,13 and 1,respectively.Seventeen lesions were heterogenous enhanced with non-enhanced necrosis areas and enhanced septa, while ten lesions homogeneous enhanced and one lesion no enhanced.Ten central pulmonary carcinoma with OA demonstrated a characteristic pattern:OA appeared a short AT(mean AT 4.8 s)until enhancement and strong contrast enhancement, while the central tumors appeared a delayed AT(mean AT 10.5 s)and faint enhancement. Conclusions CEUS can be useful in differentiation between solid and cystic pulmonary lesions, and detection of the latent lesions underlying the atelectasis.

Objective To evaluate the left ventricular systolic synchrony in hemodialysis depended(HD)patients by tissue synchronization imaging(TSI).Methods GE Vivid 7 ultrasound system with TSI and Q-analyze software was performed in 70 HD patients and 35 control individuals.According to the left ventricular enddiastolic diameter(LVDd),HD patients were divided into left ventricular enlarged group(HD1,LVDd>55 mm)and non-left ventricular enlarged group(HD2,LVDd≤50 mm).Systolic asynchrony was determined by measuring the time to peak systolic velocity of all 12 segments corrected by the heart rate(Tc).Results ①The mean Tc and the Tc of most segments were greater in both the HD1 and HD2 groups than in controls(both P<0.05).②The Tc was prolonged in 463(463/840)segments in HD group and left ventricular systolic asynchrony was detected in all patients(70/70).③The mean peak systolic velocity(PSV)and the PSV of most segments in HD group were lower than those in the control group(P<0.05).④Compared with HD2 group,Tc was increased and PSV was decreased in partial segments of HD1 group(P<0.05),but no significant differences in mean Tc and mean PSV were found between HDI and HD2 groups(P>0.05).Conclusions Left ventricular systolic synchronicity was impaired in hemodialysis depended patients.TSI was shown to be useful for the detection of myocardial abnormalities and asynchrony in such patients and provided reliable parameters for clinical therapy.

Objective To investigate the effects of hypertension on left atrial myocardial properties by strain and strain rate(SR)imaging. Methods A total of 56 consecutive patients with hypertension were divided into 2 groups:33 with normal LVMI and 23 with left ventricular hypertrophy. These patients and 20 controls underwent transthoracic echocardiography, strain and SR imaging examination.Atrial strain and SR values of hypertensive patients were compared with those of controls. Results When compared with the controls, early diastolic SR(ESR)was significantly decreased in hypertensive patients(P<0.01-0.001),with time to peak late diastolic SR corrected for heart rate(TASRe)significantly increased in normal LVMI subgroup(P<0.05).Moreover,late diastolic SR(ASR)were significantly decreased(P<0.05)in hypertensive patients with left ventricular hypertrophy as compared with normal LVMI.Conclusions Enlargement of left atrium leads to increased reservoir function in hypertensive patients, which compensates for the deterioration of conduit function. The duration of atrial systole may be prolonged to meet the need of ventricular filling in consequence of lower left atrial booster pump function.Noninvasive quantification of left atrial function by SR imaging enables evaluation of left atrial dysfunction due to hypertension.

Objective To evaluate the left ventricular twist and radial displacement in patients with hypertrophic cardiomyopathy(HCM)by 2-dimensional(2D)ultrasound speckle-tracking imaging(STI).Methods 2D images of left ventricle at basal and apical short-axis views were acquired in 35 patients with HCM and 35 healthy subjects, and then were analysed off line to assess left ventricular(LV)rotation and radial displacement.Peak rotation(Prot)and the time to peak rotation in basal and apical short axis views were measured separately.Peak twist(Ptw),twist at aortic valve closure(AVCtw),twist at mitral valve opening(MVOtw),untwisting rate(Untw R),half time of untwisting(HTU),radial displacement were also calculated and compared and compared between HCM group and the control group.Based on the value of radial displacement, the LV twist-displacement loop was established and the features of the loop were analyzed. Results ①Compared with the control group, the value of Prot-MV,Prot-AP,Ptw,AVCtw,MVOtw and HTU increased significantly(all P<0.05)and the Untw R decreased(P<0.05)in the HCM group.②In HCM group, time to Prot in apical view was higher than that in basal.③ Twist-displacement loops could be constructed noninvasively. The liner slope changed significantly in HCM group compared with that of the control group. Conclusions The characteristics of LV rotation in patients with HCM were significantly different from the normals, and the STI measurement could be used noninvasively to evaluate the characteristics of LV twist and radial displacement.
