1.A exploration of organization and implementation for configuration management of medical equipment during preparing to establish general hospital

Objective:During establishing a big general hospital, to pay attention to the organization methods and implementation steps of medical equipment management can shorten the erection time and increase establishing efficiency.Methods: According to government investment for medical equipment and infrastructure project plan, to optimize medical equipment planning scheme; to carry on expert verification, declaration and purchasing by invitation to bid; and coordinate the equipment factories, infrastructure engineers and technicians to finish the installation site condition in order to make sure the equipment installation timely and acceptance check. Results: Based on the amount of government investment, to finish the 0.5 billion equipment plan, hundreds of medical equipment purchasement, arrival, installation and acceptance so as to ensure the hospital basic clinical requirement. To put forward solutions to difficult problems about medical equipment management can ensure every work smoothly.Conclusion: The construction of a new hospital is a system engineering included from planning design, capital construction, equipment inviting bid and installation, and decoration to application. There is affinity between each point and equipment in this system, and this system need higher professional ability and coordinated ability of equipment manager. At present, Shenzhen hospital of Southern Medical University is establishing, managers should communicate and coordinate the process and progress of various aspects; provide planning ideas and implement methods; improve quality; shorten project period and then obtain the whole advancement effect based on possible problem in different stage of equipment management.
2.The effect of continuous quality improvement of nursing care on senile patient with gastroscopy

Objective:To explore the influence of continuous quality improvement of nursing care on the compliance of senile patient with gastroscopy.Methods: 100 senile patients with gastroscopy were selected, and they were divided into control and improvement groups ( each group included 50 cases ) depended on the different time of hospitalization. The control group was treated with routine nursing quality method, while the improvement group was treated with continuous quality improvement method. A series of indicators in the endoscopic examination, such as the success rate of one time intubation, the used time, yes or no cough and other adverse reactions and the patient's satisfaction for nursing services were compared between the two groups.Results:After 5 months of continuous quality improvement nursing, the success rate of one time intubation in the improvement group was significantly higher than that in the control group, and the checking time of improvement group was significantly less than that of control group, the differences were statistically significant(t=7.162,t=6.61; P<0.01). Besides, on check process, the incidence of adverse reactions in the improvement group was significantly lower than that in the control group and the satisfaction of nursing service was significantly higher than that of the control group, the differences were statistically significant(x2=23.077,x2=10.699,P<0.01).Conclusion:The application of continuous quality improvement nursing for senile patients during gastroscopy can improve the success rate of one time intubation, shorten the gastroscopy time, reduce the incidence of adverse reactions, enhance compliance of gastroscopy and satisfaction for nursing service, and its clinical effect is obvious.
3.Effect of pancreatic kininogenase for renal function and hemodynamics of patients with clinical diabetic nephropathy

Objective:To observe the effect on renal function and hemodynamics of pancreatic kininogenase in patients with clinical diabetic nephropathy(DN,Ⅳ Period).Methods: 145 patients with clinical diabetic nephropathy(DN,Ⅳ Period) were divided into the observation group (75 cases) and the control group (70 cases) as random method. The control group was given conventional treatment, and the observation group was given pancreatic kininogenase based on the conventional treatment. After 8 weeks, to observe and analyze the changes of renal function indexes and hemodynamics indexes of the two groups.Results:After treatment, the serum creatinine(SCr), blood urea nitrogen(BUN), Serum Cys C(CysC) and urinary albumin excretion rate(UAER) of observation group were obviously decreased compared with that before treatment; the Cys C, UAER of control group also were obviously decreased, but SCr, BUN of control group had no significant changes compared with that before treatment; and all indexes of the observation group were lower than those of the control group, with significant differences (t=4.2744, t=3.0832,t=6.8261,t=8.0936;P<0.05). After treatment, the systolic pressure(SBP), diastolic blood pressure(DBP) and resistance index(RI) of two groups were decreased; both of peak systolic blood flow(PSV) and end-diastolic blood flow velocity(EVD) were increased, and the change ranges of every index of the observation group were improved more than those in the control group, with significant difference (t=5.6270,t=6.2009,t=5.0926,t=4.764,t=3.1844;P<0.05). Conclusion: Pancreatic kininogenase has the clear effect on protecting renal function in the treatment of clinical diabetic nephropathy(Ⅳ Period), and it can reduce urinary albumin excretion and improve hemodynamics.
4.Observation on the curative effect of modified amniotic membrane transplantation for agricultural pterygium

Objective:To observe the curative effect of modified amniotic membrane transplantation for agricultural pterygium.Methods: A total of 276 cases (276 eyes) with early pterygium were randomized divided into traditional group (138 cases) and modified group (138 cases) as random table. The traditional group was treated with pterygium excision in combination with autologous conjunctival transplantation; the modified group was treated with modified amniotic membrane transplantation; to observe the curative effect of the two groups.Results: After all of patients were followed up for 1 year, 122 cases were cured and 16 cases were recurrence, and the recovery rate was 88.41% in traditional group; 131 cases were cured and only 7 cases were recurrence, and the recovery rate was 94.93% in modified group and was better than traditional group; the difference between the two groups was statistical significant (x2=5.02,P<0.05).Conclusion: The modified amniotic membrane transplantation method is simple in operation, mini invasive for eye tissue, and can retain conjunctival tissue in utmost; it also can improve success rate of surgery and be an effective mean for treatment of pterygium.
5.Using perineum four-dimensional pelvic ultrasound stereo imaging to detect abnormal situation of pelvic floor muscle and diaphragmatic hiatus of primipara

Objective:To use the perineum four-dimensional pelvic ultrasound stereo imaging for detecting injury situation of pelvic diaphragm hiatus and anal levator ani muscle. Understand abnormal classification of pelvic floor muscle. investigate the application of four-dimensional pelvic ultrasound stereo imaging in pelvic floor dysfunction (PFD) for primipara in order to provide more accurate data for clinical practice.Methods: 270 primiparas with pelvic floor dysfunction were divided into the vaginal delivery group (135 cases) and selective cesarean group (135 cases) as random number table; pelvic floor muscle strength, longitudinal diameter, transverse diameter, area of pelvic diaphragm hiatus and thickness of the anus levator were detected by the perineum four-dimensional pelvic ultrasound stereo imaging, respectively, and then to compare differences between the two groups. Results: In 270 cases, 162 cases (60%) were abnormalities in pelvic floor muscle strength, and 189 cases (70%) were abnormalities in vaginal pressure; the results of pelvic floor muscles fatigue grade situation revealed: 127 cases were abnormal muscle fiber strength ofⅠtype (47%), 30 patients were abnormal muscle fiber strength ofⅡ type (11%). There were no significant differences between the two groups in the abnormal rate of pelvic floor muscle strength, vaginal pressure and muscle fiber strength. Longitudinal diameter, transverse diameter and area of pelvic diaphragm hiatus in the vaginal delivery group were higher than that in the selective cesarean group, while thickness of anal levator were thinner than that in the selective cesarean group with significant differences (t=3.730,t=3.467, t=3.826,t=1.809;P<0.05).Conclusions: The perineum four-dimensional ultrasound diagnosis can intuitively and clearly show morphological characteristics of female pelvic diaphragm hiatus, therefore, it can quickly finish diagnosis, and provide the basis for clinical diagnosis and treatment evaluation of postpartum PFD.
6.Diagnostic value of magnetic resonance imaging for lymphocele after the pelvic lymph nodes dissection of gynecologic malignant tumor

Objective:To explore the imaging features of complicated lymphocele after the pelvic lymph node dissection. Methods: After pelvic lymph nodes dissection, lymphoceles in pelvic cavity of 47 patients with gynecological malignant tumor were found by magnetic resonance imaging (MRI) and all of these imaging data were retrospectively analyzed.Results:All of 47 cases had lymphoceles in pelvic cavity, and the number of lymph node lesion was 52 (30 were at left side of pelvis, 22 were at right side of pelvis); the distribution of most lymphocele were around of external vein and artery of iliac; the main MRI features were roundness, oval, reniform or dumb-bell shape with smooth and uniform lymphocele wall, and the intracystic manifestations were long T1signal and long T2signal; the manifestation of DWI was high signal of limited dispersion; after enhancement scan, the wall of lymphocele and intracyst appeared middle reinforcement signal, and there was no obvious reinforcement signal in cystic contents.Conclusion: There are characteristic MRI imaging manifestations in lymphocele, and the diagnosis of MRI is reliable and accurate.
7.Quality control method and detection data analysis of electrosurgical unit

Objective:To establish the quality control method of electrosurgical unit(ESU) through the safety performance test and data analysis for electrotome.Methods: According to the testing standard of ESU, Fluke ESA612 electrical safety analyzer and QA-ES II electrosurgery unit analyzer were applied to test the properties of ESU were using during 2015 and 2016, and the testing results were analyzed.Results:In 2015 and 2016, the qualified rate of quality control results were 79% and 81%, respectively; the problems of unqualified ESU were resolved and their hidden risks in clinical applications were eliminated.Conclusion: By testing the properties of ESU, we can master the performance of the machine; find and deal with the risk of machine in time, and avoid the hidden danger and enhance the risk management for ESU, so that the machine can be safely and effectively applied in clinical surgery.
8.A design of automatic classification system for leukocyte

Objective:To design a automatic classification system for leukocytes in order to increase detection speed of manual microscope inspection and reduce the inaccurate detected results in clinical laboratory; and to evaluate this system.Methods: In this system, the image processing and algorithm classifying were achieved by MATLAB software consisted of digital image processing module and automatic classification module. Classification decision for discrimination function and simulated detection for this system were achieved by using automatic classification module.Results: In the simulation experiments, the detection results for sample cell demonstrated the recognition accuracy can achieve to 93% and the speed can achieve to 97.8 cells per second for this system.Conclusion: The automatic recognition and classification system for leukocyte not only reduces human consumption, but also improves the detection accuracy and detection speed for leukocyte, and it has some significant in clinical application.
9.Discussion on the influence factor of induced radioactivity radiation field of high-energy medical linear accelerator and its safeguard measures

Objective:To discuss the influence factor of induced radioactivity radiation field of high-energy medical linear accelerator and its safeguard measures.Methods: The simulation test was applied to research the radiation level of the induced radioactivity for high-energy medical linear accelerator; and to research the influences of different factors, such as radiation dose, range of exposure and site of exposure and so on, on the changes of radiation field.Results: The research showed that the high-energy X-ray radiation field strength would increase with the increasing of radiation dose, while reduce with the increasing of radiation time; and the obviously reductive trend appeared after radiated 5minutes. Meanwhile, radiation intensity would reduce with the increasing of irradiated area, but the reductive trend was not totally proportional in accordance with the increasing of irradiated area. Additionally, the head part of linear accelerator could produce the most intensity of radiation field on exposure site, and the farther away the head part of linear accelerator, the intensity the lower; and the reductive trend would increase when the distance exceeded 150cm.Conclusion: The induced radioactive radiation field intensity of high-energy medical linear accelerator has some characteristics of change. The effective safeguard measures were to reduce the work time in equipment room, reduce the adjustment time of patient position and choose reasonable exposure site and dose, and all of these measures can reduce the radiation injury.
10.The application and progress of contrast-enhanced ultrasound in rheumatoid arthritis

The contrast-enhanced ultrasound(CEUS) been equipped in most commercial ultrasound system, is a newer technology which could evaluate the blood flow of soft tissue. It could not only make qualitative and semi-quantitative analysis, but also the quantitative analysis could be acquired. At present, it has been applied on cardiovascular imaging and tumor perfusion assessment in clinical. The clinical usage of CEUS in rheumatoid arthritis(RA) was summarized in the paper which included its common clinical type, dose of ultrasound contrast agent and various method of usage described. Besides, the paper not only introduced the evaluation of the synovitis in RA with the parameters resulted from CEUS, but also illuminated the progress of the researches. Through compared these parameters, indexes and methods, this summary revealed the limitation and application value of CEUS to RA, and more exploration need be done in future.
