1.A Case of Woringer-Kolopp Disease.

Woringer-Kolopp disease, so called pagetoid reticulosis, is most likely represent a chronic localized from of reticulosis. A 24-year old male patient visited with asymptomatic slowly enlarging, scaly single patch on his left thigh for 10 years duration. Histopathologic studies were taken with H-E stain, D-PAS, transmission electronmicroscopy and monoclonal antibody(OK6) tissue stain. Infiltration of atypical cells showed in the lower portion of epidermis like pagetoid patterned, none in dermis. It seems to be that the cell in this case probably originated from the stimuIated T-lymphocyte.

2.Treatment of Witiligo with Oral Methoxsalen and UVA.

Among ]20 vitiligo patients who visited our clinic during the period of January 1984 to February 1985, 39 patients were treated with oral methoxsalen (8-MOP) and UVA for more than 6 months, The following conclusions were obtained by analysis of the clinical histories and efficacy of PUVA treatment: 1. The mean age of 39 patients was 31 and that of onset was 21. An average of 1() years elapsed from the onset of the disease prior to visiting our clinic. The initial involvement was observed, in decreasing order of frequency, on the trunk, face and/or neck and extremities. 3, The repigmentation started most frequently from the trunk followed by the extremities, face and/or neck after treatment of 2 months and 14 irradiations on the average. 4, There was a direct relationship between the frequency and duration of treatment and its effectiveness. 5. The trunk showed the best response to the treatment, followed by the extremities; face and/or neck. The most resistant sites were the the hand and/or foot. 6. No specific relationship was found between the patient's age, duration of illness and responsiveness to the treatment. 7. The side effects were minimal and transient.

3.Ichthyosis Hystrix Treated by Vitamin A acid Ointment Application.

Ichthyosis hystrix is a localized variant of Bullous Congenital Ichthyosiform Erythroderma by Broq, and also, localized form of Epidermolytic hyperkeratosis according to Frost and Van Scott's classification of ichthyosi-form dermatoses. Clinically it is generally considered to be widespread systematized epidermal nevus, and demonstrating an autosomal dominant pattem of inheritance, and characterized by verrucous scale at or shortly after birth with particular involve- ment of flexural areas. A case of 21 year old girl with ichthyosis hystrix was reported, The skin lesion showed multiple rice to pea sized linear verrucous papules on left forearm, neck, axilla and upper chest. The skin biopsy was done, the finding shows remarked laminated hyperkeratosis, acanthosis, papillomatosis, and vacuoliization on malpighian layer. Diagnosis was confirmed by clinicaI apperance and histopathologicaI finding. The method of treatment is 0. 05% Vitamin A acid ointment application and show marked improvement.

4.A Case of Leukemia Cutis in Erythroleukemia.

A 13 years old girl came to our department with complaints of multiple ulcerating and non-ulcerating nodules on her back and buttocks since 1 month prior to visit. Skin biopsy specimen showed diffuse monomorphous infiltration of leukemic cells in the dermis and subcutis. Bone marrow biopsy specimen showed changes of erythroleukemia. Treatment was initiated with combined chemotherapy which was a combination of cytosine arabinoside, adriamyein and 6 thioguanine. When combined chemotherapy was finished, she was in complete remission state and the nodules cured after 1 month of combined chemotherapy.

5.Ehlers - Danlos Syndrome.

Ehlers-Danlos syndrome is an inherited disorder of connective tissue in which joint hypermobility, hyperelasticity of skin, bleeding tendency, and scar formation are the most prominent features. It is a generalized disease with essentially universal involvement. A 3-year-old female child had velvety skin, skin hyperextensibility, joint hypermobility, subcutaneous mobile masses on the left shin, many scars on both knees, subcutaneous hematoma, and thick and very folded skin on both palms and soles, but no evidence of internal disorder. Cutaneous histopathologic findings were nonspecific without increase of dermal elastic fibers on Verhoeff stain. Diagnosis was confirmed by clinical and histopathological findings as Ehlers-DanIos syndrome.

6.Multiple Granuloma Pyogenicum on Shoulder area.

Granuloma pyogenicum is a benign vascular tumor, usually about 0.5 to 2cm in diameter. It occurs as a single lesion with few exceptions and consists of pedunulated nodule with a dull red or slightly purplish color. We observed a 29-year old man who had multiple satellite recurrences in the scapular region which developed after excision of a single lesion of granuloma pyogenicum. Histapathologic findings showed many newly formed capillaries that had prominant endothelial cells and showed varying degrees of dilation without appearance of epiderrnal collarette sign. The lesions were much improved by X-ray irradiation.

7.Four Cases of Unusual Pigmentation after UVB Phototherapy for Psoriasis.

The ability of photochemotherapy(PUVA) or UVA phototherapy to stimulate melangenesis is well known. It is now recognized that the PUVA lentigines, irregular in outline and often more darkly and unevenly pigmented than solar lentigines, are related to the additional pigmentary effects of the PUVA therapy. We present herein four cases of unusual pigmentation after one month of UVB irradiation. Raticulated dark brown pigmentation appeared on the lower extremities of the patients. Histopathologicially, an increase in melanin in the basal layer was found, but no dysplasia or malignant change. We suggest UVB as the cause of this unusual hyperpigmentation.

8.Quantiative Study of Epidermal Langerhans Cell in Vitiligo.

Report. about numeric change of LC between vitiliginous skin(VS) and adjacent normal appering skin (ANAS) are hard to find, Epidermal Langerhans Cell (LC) dersities in VS and ANAS were studied in 18 patient with generalized vitiligo. Adenosine triphosphatase(ATPase) stain was used to characterize LC. Epidermal LC densities were calculated by means of an eye piece reticule and expressed per mm. The results were as follows; 1. The was significant body site variation of LC densities in ANAS and the range of densities of LC per mm were from 668+165 to 1,241 _3 and the mean density of LC per mm was 944+258. 3. The LC densities of VS was similar to that of ANAS(p: not significant). In conclusion quantiative change of epidermal LC in vitiligo doesn't seem to have significant relation with pathogenesis of vitiligo.

9.A Study of Combined Methotrexate-UVB Therapy for the Treatment of Psoriasis.

We evaluated the treatment schedule of a 4 week course of methotrexate followed by UVB radiation therapy on 18 patients of generalized psoriasis. The patients received the weekly triple doses of methotrexa,te(MTX), 5 mg given every 12 hours for three doses on weekend for 3 weeks. On 4 th weekend, the doses was halved to 2. 5 mg every 12 hours for three doses, and then stopped. After a 4 week course of methotrexate, total doses 52. 5 mg, UVB treatment was commenced. The results are as follows: 1. 13 patients showed clearing of psoriatic involvolvement over 50, after 4 week course of MTX therapy. 2. Complete clearing of skin lesions was achieved in all 18 patients with the average,"j, 8 times of UVB exposure in trunk and 5 6 times in extremities respect ively. 3. The average dose of UVB exposure at clearance was 187 mJ/cm in trunk lesion and 263. 6 mJ/cm in extremities, The mean cummulative dose of UVB exposure at clearance was 800. 7 mJ/cm in trunk lesion and 994. 3 mJ/cm in extremities. In conculuaion, the brief 4 week course of MTX allows for clearing of psoriasis at relatively low dose of UVR, and thus may reduce the long-term cumrnulative toxicity of both agents.

10.Effects of Topical Anti-inflammatory Agents and Emollients on UVB Erythema and Pigmentation in Normal Korean Males.

In healthy Korean adult males, 6 hour-MED was 51 2+22.6mJ/cm,24 hour-MED was 67. 5+26. 7mJ/cm, and MMD was 86. 5 t21. 3mJ/cm for UVB irradiation respectively. UVB-MED and UVB-MMD was increased by desoxymethasone, hydrocortisone, and bufexama.c creams. Hydrophilic ointment base increased only 24 hour-MED. For 1 MED of UVB, all test agents inhibited erytherna for 48 hours, For 2 MEDs of UVB, desoxymethasone, bufexamac; and hydrocortisone creams inhibited erythema at 6 hours, while desoxymethasone and hydrophilic ointment base could suppress erythema at: 24. hours after irradiation. However, desoxymethasone was the most effective. Hydrophilic ointment base was efficacious only at 24 hours after irradiation. For 1 MMD of UVB, desoxymethasone, hydrocortisone, and bufexamac creams could reduced the delayed tanning (DT) reaction.
