2.Scanning Electron Microscopic Study of the Effect of Tetracycline-HCl on the Change of Implant Surface Microstructure according to Application Time.

The present study was performed to evaluate the effect of tetracycline - HCl on the change of implant surface microstructure according to application time. Implants with pure titanium machined surface, SLA surface and TiO2blasted surface were used. Implant surface was rubbed with 50mg/ml tetracycline - HCl solution for 1/2 min., 1 min., 1 1/2 min., 2 min., and 3min. respectively in the test group and with no conditioning in the control group. Then, the specimens were processed for scanning electron microscopic observation. The following results were obtained. 1. In the pure titanium machined surfaces, the control specimen showed a more or less rough machined surface composed of alternating positive and negative lines corresponding to grooves and ridges. After treatment, machining line was more pronounced for the control specimens. but in general, test specimens were similar to control. 2. In the SLA surfaces, the control specimen showed that the macro roughness was achieved by large-grit sandblasting. subsequently, the acid-etching process created the micro roughness, which thus was superimposed on the macro roughness. 3. In the SLA surfaces, irrespective of the application time of 50mg/ml tetracycline - HCl solution , in general, test specimens were similar to control. 4. In the TiO2blasted surfaces the control specimen showed the rough surface with small pits. The irregularity of the TiO2blasted surfaces with 50mg/ml tetracycline - HCl solution was lessened and the flattened areas were wider relative to the application time of tetracycline - HCl solution. In conclusion, pure titanium machined surfaces and SLA surfaces weren't changed irrespective of the application time of tetracycline - HCl solution. And the TiO2blasted surfaces conditioned with tetracycline - HCl solution began to be changed from 1 1/2 min. This results are expected to be applied to the regenerative procedures for peri-implantitis treatment.

3.Comparative study on tissue responses of 3 resorbable membranes in rats.

The purpose of this study is to evaluate histologically the resorption and tissue response of various resorbable collagen membranes used for guided tissue regeneration and guided bone regeneration, using a subcutaneous model on the dorsal surface of the rat. In this study, 10 Sprague-Dawley male rats (mean BW 150gm) were used and the commercially available materials included acellular dermal matrix allograft, porcine collagen membrane, freeze-dried bovine dura mater. Animals were sacrificed at 2, 6 and 8 weeks after implantation of various resorbable collagen membranes. Specimens were prepared with Hematoxylin-Eosin stain for light microscopic evaluation. The results of this study were as follows: 1. Resorption : Inner portion of porcine collagen membrane was resorbed a lot at 6 weeks, but its function was being kept for infiltration of another tissues were not observed. Freeze-dried bovine dura mater and acellular dermal allograft were rarely resorbed and kept their structure of outer portion for 8 weeks. 2. Inflammatory reactions : Inflammatory reaction was so mild and foreign body reaction didn't happen in all of resorbable collagen membranes, which showed their biocompatibility. 3. In all of resorbable collagen membranes, multinucleated giant cells by foreign body reactions were not observed. Barrier membranes have to maintain their function for 4-6 weeks in guided tissue regeneration and at least 8 weeks in guided bone regeneration. According to present study, we can find all of the resorbable collagen membranes kept their function and structure for 8 weeks and were rarely resorbed. Foreign body reaction didn't happen and inflammatory reaction was so mild histologically. Therefore, all of collagen membranes used in this experiment were considered proper resorbable membranes for guided tissue regeneration and guided bone regeneration.

4.Effects of 2% minocycline gel as an adjunct to scaling and root planing on the treatment of adult periodontitis.

The objective of the present study was to evaluate the clinical and microbiological effects of scaling and root planing combined with local application of 2% minocycline gel to patients with moderate to advanced chronic adult periodontitis. 27 healthy patients with moderate to advanced chronic adult periodontitis were enrolled in the study. The quadrants that had 2 or more teeth with 5~8mm probing pocket depth and radiographic evidence of alveolar bone loss were selected and divided into test side and control side according to the splitmouth design. All patients received standardized oral hygiene instructions at the beginning of the study. Subsequently scaling and root plaining was performed on all remaining teeth until 0 week. The 2% minocycline gel was applied to periodontal pocket at 0, 1, 2, 3week in the test side. The normal saline was irrigated subgingivally for about 30 seconds in the control side. The clinical and microbiological analysis was carried out at 0, 4, 8, and 12weeks. The results of this study were as follows; 1. 2% minocycline gel delivered subgingivally as an adjunct to scaling and root planing provided benefit in reducing sulcular bleeding index and pocket depth than the use of normal saline. 2. The relative proportion of cocci and non-motile bacteria was increased in the test and control groups with time, and there was no statistically significantdifference between two groups. 3. The proportion of spirochetes was slowly reduced in the control group, but, inthe test group, they were remarkably reduced from the 4th week, and there was a statistically significant difference between two groups. 4. In both groups, the relative proportion of motile rods was notably decreasedat the beginning of the study, and remained until 12th week in the test group,but, in the control group, they were slowly increased from the 4th weekand finally similar to that of the initial examination. In conclusion, local application of 2% minocycline gel may be effective in the clinical and microbiological aspects as an adjunct to scaling and root planing in periodontal disease sites.

5.Effect of deproteinized bovine bone mineral on cell proliferation in the procedure of guided bone regeneration.

One of the bone substitutes now in routine use, deproteinized bovine bone mineral(DBBM), is regarded as resorbable and osteoconductive, but some studies refute this. The present study was performed to evaluate the effects of DBBM on guided bone regeneration using titanium membrane on the calvaria of rabbit. At 2 weeks, 4 weeks, 8 weeks, and 12 weeks after surgery, the animal was scrificed. Non-decalcified specimens were produced for histologic analysis. The results of this study were as follows: 1. Titanium membrane was biocompatible and capable of space-maintaining, but there was ingrowth of soft tissue through the pore of titanium membrane. 2. There was no resorption or reduction of DBBM with time. 3. Some of the DBBM particles were combined with newly formed bone. But, apart from host bone, a great part of the particles were surrounded by connective tissue. 4. The bone formation was slight vertically and restricted to superficial area of host bone. Whithin the above results, DBBM dose not appear to contribute to bone formation. DBBM may disturb the migration and proliferation of mesenchymal cell derived from host bone and increase the growth of connective tissue. Therefore, careful caution is needed on selection of bone graft material and surgical protocol at guided bone regeneration for implant placement.

Implant surface was rubbed with 50mg/ml tetracycline - HCl solution for 1 min., 1 1/2 min., 2 min., 2 1/2 min. and 3min. respectively in the test group and with saline for 1 min. in the control group. Then, the specimens were processed for scanning electron microscopic observation. The following results were obtained. 1. Pure titanium machined surfaces conditioned with saline for 1 min., showed a few shallow grooves and ridges and were less irregular. 2. Pure titanium machined surfaces conditioned with 50mg/ml tetracycline - HCl solution showed more irregular and corrosive surfaces compared to the control group irrespective of the application time of tetracycline - HCl solution. 3. Titanium plasma-sprayed surfaces conditioned with saline for 1 min., were deposited with round or amorphous particles and showed round or irregular pores that can be connected to each other. 4. The irregularity of titanium plasma-sprayed surfaces conditioned with 50mg/ml tetracycline - HCl solution was lessened and the flattened areas were wider relative to the application time of tetracycline - HCl solution. 5. Titanium plasma-sprayed surfaces conditioned with tetracycline - HCl solution for 1 min. and 1 1/2 min. were very similar to that of the control group, but the surfaces conditioned for 2 min., 2 1/2 min. and 3 min. were changed. In conclusion, pure titanium machined surfaces were changed irrespective of the application time of tetracycline - HCl solution. And titanium plasma-sprayed surfaces conditioned with tetracycline - HCl solution began to be changed from 11/2 min. This results will be applicable to the regenerative procedures for peri-implantitis treatment.

7.Regenerative Effects of Alloplastic Grafts in Rat Periodontal Fenestration Defects.

The purpose of this study was to investigate effects of the natural coral(NC) and the hydroxyapatite/calcium sulfate hemihydrate(HA/CS) on an early stages of wound healing in the rat periodontal fenestration defects. In this experiment, twelve male rats(Mean : 520g in BW) aged 8 to 9 months were used. Experimental periodontal fenestration defects were surgically created with tapered fissure bur at the buccal surface of the left mandibular 1st, 2nd molars. The buccal aspects of molar roots were carefully denuded of their periodontal ligament through a bony window created in the left mandibles of rats under general anesthesia. Each experimental periodontal fenestration defect was grafted with natural coral and HA/CS, randomly. An area without bone graft was assigned for negative control group. At 10, 35 days, rats were serially sacrificed via intracardiac perfusion with 2.5% glutaraldehyde and specimens were processed with Hematoxylin-Eosin stain for light microscopic evaluation. The results of this study were as follows: 1. The defect areas were filled with dense connective tissues at 10 days in control group. But in the test(NC, HA/CS) groups, the connective tissues around graft materials were formed more loosely and the response of inflammation by graft materials itself was not found. 2. The defect areas were filled with new osteoid tissues and new cementum was not formed on the cut root surface at 35 days in the control group. 3. New osteoid tissue formation was more prominent at 35 days in control than test groups. 4. The NC and HA/CS particles were encapsulated by loose connective tissues at 10 days and by dense connective tissues at 35 days, respectively. 5. In the test groups, resorption of graft particles was not found through the experimental time. From the above results, natural coral and hydroxyapatite/calcium sulfate hemihydrate may be biocompatible and osteoconductive and have a weak adverse reaction to the periodontal tissues.

8.EFFECT OF ELECTRICAL STIMULATION ON BONE FORMATION IN THE EXTRACTION SOCKET OF RAT.

On the basis of the evidence that electrical stimulation could promote healing and regeneration of bone, this study was performed to investigate the effects of electrical stimulation on rat extraction socket, and to evaluate the potential of clinical application of electrical stimulation. Forty rats were used and divided into control groups(10)and the experimental groups(30) in this study. The maxillary 1st molar were extracted in both groups. In experimental group, electrical stimulation was given at the current intensity of 1mA(Test-1), 10mA(Test-2), 25mA(Test-3) each day. At 1, 3, 5, 7 days after the tooth extraction, rats in both groups were serially sacrificed. And the specimens were prepared with Hematoxylin-Eosin stain for the light microscopic evaluation. The results of this study were as follows; 1. At 1 day after the extraction, the periodontal ligament was found in the extraction socket wall. The formation of blood clot with dense infiltration of inflammatory cells in control group and there were less inflammatory cells in test group. 2. At 3 day after the extraction, the cells and collagen of the periodontal ligament were so actively proliferated and synthesized that invaded into the connective tissue of the extraction sockets in the control group. There were the formation of new bone in the basal & lateral portion of socket wall in test -2 and -3. 3. At 5 days after the extraction, there were no formation of new bone in control group. But the more electrical stimulation was applied, the more formation of new bone in test group. 4. At 7 days after the extraction, the extraction sockets were almost filled with trabecular bone in each group. Bone maturarity was remarkable in test-3. 5. The electrical stimulation at 10mA and 25mA was more effective in the bone formation at 5 and 7 days after the extraction. From the above results, electrical stimulation could promote the extraction socket wound healing, and be utilized in the clinical application of the residual ridge expansion.

9.Effects of Ethanolic Extracts of Scutellaria Radix on the alveolar bone formation in the extract socket of rat.

The purpose of this study was to evaluate the effects of ethanolic extracts of Scutellaria Radix on the alveolar bone formation in the extract socket of rat. Thirty-six Sprague-dawley rats were used in this study. Mean body weight of rat was 130+/-5g. Experimental animal were administered 0.4% beta-aminopropionitril(Sigma, USA) with the solid commercial food for 5 days. All the maxillary 1st molar of the rats were extracted by using of the tissue forcep under the general anesthesia with Pentobarbital sodium(Tokyo Chemical Co., Japan) injection into intraperitoneal space. All the extracted rats were divided into two group, experimental group which were feeded the solid food mixed ethanolic extracts of Scutellaria Radix, and control group which were feeded same food without reagent. At 1, 3, 5, 7, 9 and 14th days after tooth extraction, rats in both groups were serially sacrificed respectively. All the specimen were treated as usual method and prepared Hematoxylin-eosin stain for the light microscopic observation. The results were as follows : 1. Bone formation of extracted socket starts from the area on remained periodontal ligament than other area. 2. In the case of administration of the extracted Scutellaria Radix showed rapid healing process of connective tissue than non-administrated group. 3. In the case of administration of the extracted Scutellaria Radixshowed rapid osteogenesis than non-administrated With above results, it was concluded that ethanolic extracts of Scutellaria Radix may play a favorable role on the healing process of exatraction socket after extraction in rats. It was suggested that further study to evaluate the different concentration and administration method of ethanolic extracts of the Scutellaria Radix into same experimental model.

10.The effects of autogenous composite grafts on bone regeneration after sinus elevation.

The purpose of this present study was to investigate the effect of autogenous bone with histological evaluation of regenerated bone after sinus elevation. The study involved genaral healthy 6 patients participated in this study and were treated with 2-stage sinus elevation procedures using a combination of demineralized freezeddried bone allograft (DFDBA) and coralline calcium carbonate with or without autogenous bone. At 6months after sinus elevation, bone specimens were obtained and stained with Hematoxylin-Eosin for light microscopic evaluation. The results of this study were as follows : 1. Autogenous bone grafts present trabecular patterns at 6 months in test groups, consist of woven bone and lamellar bone, but more compact than control groups. 2. Resorption of bone graft particles, osteoblast-like cells, newly formed osteoid tissue were observed at 6 months in test groups, but seems to be more frequently than control groups. 3. New osteoid tissue was formed from the surface of graft materials and gradually expanded around them. 4. The appearance of connective tissue around graft materials was densely formed, but more prominent in test groups than control groups. 5. Bone graft particles were resorbed incompletely and slight inflammatory infiltrate, newly formed capillaries, and adipocytes were observed. From the above results, autogenous bone is effective in bone regeneration after sinus elevation, could provide favorable conditions in implant placement.
