1.Determination of Diagnosis and Treatment for Head and Neck Cancer Patients Considering Frailty

As population ages, much more elderly patients have been diagnosed with head and neck cancer especially in Korea. Diverse factors such as comorbidities and disabilities make the establishment of evidence-based guidance more challenging. Chronological age, literally measured in days, months and years from the time the individual was born, is not an ideal predictor in determining personal health condition; individualization of treatment protocols with identifying the concept of frailty is the key to produce an advanced treatment plan of elderly patients suffering from head and neck malignancy. Frailty is a type of geriatric syndrome which refers to the status with age-associated declines in physiologic function and increased vulnerability to stress conditions independent of age. Therefore, screening the status of frailty may be beneficial for the elderly patients to determine decent treatment option and maximize the efficiency. In order to provide otolaryngologists prognostic information adapting the concept of frailty, research papers published from 2000 to 2018 are comprehensively reviewed in this article. For optimal care of elderly patients with head and neck cancer, frailty is definitely required to be assessed in order to establish an ideal treatment option and increase the efficacy of the treatment.

Aspiration, the entry of material into the airway below the true vocal folds, has been reported in various type of postsurgical head and neck patients. The important of correctly identifying the cause of aspiration in the head and neck surgical patient. Clearly each cause represents a different physiologic or anatomic disorder which requires different management strategy. We discuss about the cause of aspiration during head and neck treatment.

4.A Case of Leiomyosarcoma of Larynx.

Leiomyosarcoma is a malignant tumor originating from the smooth muscle tissue and is mostly found in the genitourinary and gastrointestinal tract and the peritoneum. A leiomyosarcoma involving the larynx is extremely rare because the smooth muscle is only present in the blood vessel in larynx. There are only few cases of pure laryngeal leiomyosarcoma that have been reported in modern medical literature. Recently, we experienced a 78-year-old male patient with laryngeal leiomyosarcoma. So we report this case with a review of literature.

5.Protocols and Results of Resident Neurosurgeon's Transfemoral Catheter Angiography Training Supervised by Neuroendovascular Specialists.

OBJECTIVE: Transfemoral catheter angiography (TFCA) is a basic procedure in neurovascular surgery with increasing importance in surgical and non-invasive treatments. Unfortunately, resident neurosurgeons have relatively few opportunities to perform TFCA in most institutions. We report a method developed in our hospital for training resident neurosurgeons to perform TFCA and evaluate the efficacy of this training. METHODS: From May 2011 to September 2011, a total of 112 consecutive patients underwent TFCA by one resident neurosurgeon supervised by two neuroendovascular specialists. Patients who underwent elective diagnostic procedures were included in this study. Patients who underwent endovascular treatment were excluded. Demographic data, indications for TFCA, side of approach, number of selected arteries, and complications were analyzed. RESULTS: This study included 64 males and 48 females with a mean age of 51.6 (12-81) years. All procedures were performed in the angiography suite. Common indications for procedures were as follows: stroke-induced symptoms in 61 patients (54.5%), Moyamoya disease and arteriovenous malformation in 13 patients (11.6%), and unruptured intracranial aneurysm in eight patients (7.1%). Right and left femoral puncture was performed in 98.2% and 1.8% of patients, respectively. A total of 465 selective angiographies were performed without complications. Angiographic examination was performed on 4.15 vessels per patient. CONCLUSION: TFCA can be performed safely by resident neurosurgeons based on anatomical study and a meticulous protocol under the careful supervision of neuroendovascular specialists.

6.A Case of Small Cell Carcinoma of the Maxillary Sinus Coexisting with Fungus Ball.

Small cell carcinoma commonly originates in the lung, with only about 4% of cases arising at extrapulmonary sites. Furthermore, small cell carcinoma of the sinonasal tract is extremely rare. In Korea, only 2 cases of primary sinonasal small cell carcinoma have been reported in the nasal cavity and the nasal septum, respectively. Recently, we have experienced a rare case of small cell carcinoma arising from the right maxillary sinus coexisting with a fungal ball lesion. Herein, we report this case with a review of the literature.

7.Korean Standardization of General Medical Health Rating on Dementia Patients.

Concurrent medical problem is common in dementia patients and critical to their care. Despite its importance, there was no bedside global rating scale for the seriousness of medical comorbidity. Lykestos et al. newly developed a reliable bedside scale, the General Medical Health Rating(GMHR). The objective of this study was to standardize the GMHR in form of Korean version(KGMHR). The study population consisted of 35 dementia patients in nursing home. Rating was performed by 1 physician and 2 nurses. Forty percent(14/35) of patients had one or more unstable medical illnesses. KGMHR ratings no more than 3 were 71.5%(25/35) of patients. The value of interrater reliability coefficient alpha was 0.9121. Correlations between KGMHR ratings and number of unstable medi-cal illnesses were high(r=-0.487, p<0.01). KGMHR ratings were also correlated with number of medications being taken for comorbid conditions(r=-0.542, p<0.01). In conclusion, KGMHR is a very reliable and simple rating scale for medical comorbidity in dementia patients. So the KGMHR could be a useful tool for evaluation of comorbidity in dementia patients. To verify the prognostic value of KGMHR, further large sized long-term study are needed.

8.A Case of Primary Nodal Type Marginal Zone B-Cell Lymphoma of the Intra-Parotid Lymph Node Mistaken to Primary Benign Parotid Mass.

Marginal zone B-cell lymphomas are indolent lymphomas that arise from the memory B cells in the marginal zone of lymphoid tissue. They include mucosa-associated lymphoid tissue lymphoma, splenic marginal zone lymphoma, and nodal marginal zone lymphoma. These three subtypes share morphologic and immunophenotypic features, but can be distinguished by primary site at presentation. Recently, we experienced a rare case of primary nodal marginal zone lymphoma that arose from an intra-parotid lymph node. So we report this case with a review of literature.

9.Extraskeletal Myxoid Chondrosarcoma of Larynx.

Primary cartilaginous tumors of the laryngeal lesion are rare and it accounts for only 1% of laryngeal tumor pathology. They are most commonly located on the endolaryngeal surface of cricoid cartilage, and it is extremely rare to find them in vocal muscle lesion. In most cases, treatment of choice is based on surgical excision, and postoperative radiotherapy is typically recommended. We experienced a case of a 22-year-old young woman without underlying disease, presenting extraskeletal myxoid chondrosarcoma of vocal cord, especially in the vocal muscle. The patient underwent mass excision through suspension laryngoscope and radiation therapy.

10.A Case of Ceruminous Adenoma Arising from the External Auditory Canal.

Ceruminous adenoma is a benign neoplasm of the ceruminous gland, and its development in human external auditory canal is very rare. Due to the limited number of cases, controversy still exists about nomenclature, classification, diagnosis and its treatment. Recently we experienced a 55 years old woman who was diagnosed with ceruminous adenoma of the external auditory canal, and who was treated with wide excision. So we report this uncommon case in combination with a review of the literature.
