The traumatic etiology is accepted generally. Some author believes that a relative discrepancy in the distal extent of the radius and ulnar plays a major role in the genesis of avascular necrosis of the lunate bone, the other belives little bearing. Over the years, the author have collected the ten cases of Kienbocks disease in Korean and then was interested the anatomic variations in radial and ulnar lengths in the wrist. The author measured the ulnar variance in 296 random normal Korean adults and in ten cases of Kienbocks disease to find out whether Hultens theory was borne out or whether Chan and Huang's observations were futher supported. The results obtained are as follows: 1. Mean ulnar variance in 296 normal Korean adults was +0.692 mm and +0.593 mm, +0.884 mm in male and female respectively. 2. In 296 normal Korean adults, the positive ulnar variance was 50.3%, in 29.7% neutral variance and in 20.0% negative ulnar variance. In ten cases of the patients with Kienbocks disease, the positive ulnar variance revealed in 4 cases, the neutral variance in 4 cases and the negative ulnar variance in 2 cases. 3. The relationship associated between the negative ulnar variance and Kienbock's disease reveals no statistical significance in Koreans.

The twelve patients, whose ages ranged from 18 years to 87 years, 5 of males and 7 of females, were admitted to Department of Orthopaedic Surgery of Korea University Hospital from January, 1978 to November, 1979 for total hip replacement surgery: 5 cases of the post traumatic femur neck fracture, 2 case of idiopathic aseptic necrosis, 2 cases of post infectious bony ankylosis of the hip, 1 case of sequele of Legg-Perthes disease, 1 case of secondary osteoarthritis secondary due to congenital acetabular dysplasia and acetabular dysplasia secondary to old infection. For these cases, the authors carrid out the surgery of total hip replacement through the new method of anterolateral “S” approach,combined with innominate osteotomy and autogenous bone graft of resected femoral head. Regular type of eleven Muller prosthesis and one Charnley prosthesis were adequately inserted in the enlarged acetabular socket. The results obtained are as follows: 1. This approach had the following advantages; such as the sufficient exposure of the operative field without the greater trochanteric osteotomy, the shortened operative time, the decreased amount of blood loss and the easy widening of the acetabular socket during the surgery. 2. There was no difficulty to perform simultaneously innominate osteotomy with autogenous bone graft of resected femoral head to the ilium during the same operative period. 3. The release of the rectus femoris from the anteroinferior iliac spine does not cause any weakness of the knee extension. 4. In all 12 patients, the end results of the function after this new method revealed saticefactory.

The authors analyzed clinical and radiographic results of the 87 cases which had done THRA with porous coated femoral stem followed up more than 2 years to study the difference of clinical and radiologic findings between the absence (group 1 ; 53 cases) and presence(group 2 ; 34 cases) of the porous coating in superolateral portion of the femoral stem. The following results were obtained : 1. Harris hip scores were improved 34.4 points from 56.5 points preoperatively to 90.9 points postoperatively in group Ι, and 32.7 points from 61 points preoperatively to 93.7 points postoperatively in group 2. The clinical results of the group 1 were Excellent in 45 cases, Good in 5 cases and Fair in 3 cases, and clinical result of the group 2 were Excellent in 33 cases and Good in 1 case. 2. The changes of femoral stem angle were below 2 degrees in 7 cases, above 2 degrees in 2 cases in group 1, and below 2 degrees in 5 cases, but not found more than 2 degrees of change of the femoral stem angle in group 2. 3. The osteolysis around femoral stem were 15 cases(28%) in Group 1, and 5 cases(15%) in Group 2. Diffuse osteolysis were 3 cases in Group 1, but was not observed in Group 2. 4. The vertical subsidence more than 2 mm was 3 cases in group 1, but was not observed in group 2. 5. The stable femoral stem were 51 cases in group 1, and 34 cases in group 2. The unstable femoral stem were 3 cases in group 1, but was not observed in group 2. Above results suggest that the porous coating in the superolateral portion of the femoral stem showed better results in radiologic and clinical findings in short term follow up.

The author performed an experiment to measure the bonding strength of acrylic bone cement when the bonding surfaces of bones were under various conditions; namely different axial compression forces and different means of preservation of bones, in the air and in Hartman's solution. Mechanical tests for compression forces revealed that the bonding strength was greatest when the bonding surfaces were dry, being weaker when soaked with alcohol and whole blood, in that order. When different strengths of axial compression forces were applied, the bonding strength was greatest at 5 kg of compression force, being weaker at 3 kg and 1 kg, in that order. The bonding strength was greater when the bones were preserved in the air than in Hartman's solution.
