4.No-incision pubovaginal suspension for stress urinary incontinence: 15 cases.

Fifteen stress urinary incontinence patients were treated using the modified endoscopic suspension of the bladder neck that eliminates all incisions during the period from January 1990 to December 1990. The average operation time was 26 minutes and the mean days of urethral Foley catheter removal were 3.9 days operation. Two point three days were required to restore the normal voiding pattern. Of 15 patients treated. 1 failure occurred, for an overall success rate of 93%. There had been no significant complications. Therefore, no-incision pubovaginal suspension was simple and effective means treating stress urinary incontinence.

5.A case of leiomyoma on urethrovaginal septum.

Leiomyoma is rare. Most of patients present with urinary tract infection, hematuria, dyspareunia, or complaint of mass. A case of leiomyoma on urethrovaginal septum in a 36 years old female was presented. The presenting symptom was a protruding mass at the anterior vaginal wall The mass was removed surgically and confirmed as a leiomyoma pathologically.

6.Clinical Results Comparing Transurethral Needle Ablation with Transurethral Resection of Prostate for the Treatment of Benign Prostatic Hyperplasia.

PURPOSE: We compared the outcome of transurethral resection of the prostate (TURP) with that of transurethral needle ablation(TUNA) of the prostate in patient with BPH. MATERIALS AND METHODS: From May 1996 to April 1997, 21 patients were treated with TURP and 25 with TUNA. In TURP group, the mean age was 67.2 years and mean prostatic volume was 52.7ml. In TUNA group, the mean age was 67.2 years and mean prostatic volume was 32.1ml. Assessment parameters were International Prostate Symptom Score(1-PSS), quality of life question, uroflowmetry and complications. Post-treatment morbidity was noted if the patients experienced erectile dysfunction, retrograde ejaculation or urinary incontinence. Patients were examined at 3 months from the day of treatment. RESULTS: At 3 months, the maximum flow rate improved from the mean of 10.2+/- 3.2m1/sec to 20.5+/-10.7m1/sec for the TURP group and 9.3+/-1.9m1/sec to 12.5+/-4.3 for the TUNA group. 1-PSS improved from the mean 22.0+/-7.6 to 6.4+/-4.6 for the TURP group and 21.2+/-6.6 to 13.1+/-6.8 for the TUNA group. Quality of life improved from the mean 3.8)1.6 to 1.3+/-1.0 for the TURP group and 4.3+/-0.9 to 2.9+/-1.1 for the TUNA group. Erectile dysfunction was reported in 43% (9/21) of TURP and 5%(1/20) of TUNA. Retrograde ejaculation was reported in 57%(12/21) and urinary incontinence was in 5%(1/21) of TURP. Retrograde ejaculation and urinary incontinence was not reported with TUNA. The mean operation time, hospital stay and a catheterization time in the TUNA group were 45.4min, 1.2days, 4.4days in comparison with 58.7min, 7.0days, 3.6days in the TURP group. CONCLUSIONS: After TURP and TUNA, there was significant improvement in all clinical parameters. But TURP was more effective in improving maximum flow rate, 1-PSS and quality of life. TUNA, however, produced minimal morbidity with no serious complication. With acceptable low morbidity, TUNA appears to be a safe and effective procedure in the treatment of BPH.

7.Digital Pulse Oximetry for the Screeing of Lower Extremity Arterial Disease.

STUDY DESIGN: Retrospective study. OBJECTIVES: The aim of the study was to investigate the usefulness of a handheld digital pulse oximetry in the detection of lower extremity arterial disease. SUMMARY OF LITERATURE REVIEW: Pulse oximetry is a well-established method for noninvasive evaluation of arterial oxygenation. MATERIALS AND METHODS: A Retrospective study was performed in 45 patients with lower extremity arterial disease. We compared the accuracy of a handheld digital pulse oximetry and the ankle brachial index (ABI). Digital pulse oximetry was conducted for 42 patients with 84 limbs to measure the SaO2 of their index fingers and big toes in supine position. The ABI was defined as abnormal if it was less than 0.9. Pulse oximetry of big toes was defined as abnormal if the SPO2 was less than 96% or it was more than 2% lower than that of the index finger. RESULTS: Digital pulse oximetry had a sensitivity of 47%(95% CI, 34-60%) and specificity of 86%(95% CI, 64-96%). ABI had a sensitivity of 49%(95% CI, 34-64%) and specificity of 95%(95% CI, 72-99%). Positive predictive values were 91%(95% CI, 74-98%) for digital pulse oximetry and 96%(95% CI, 77-99%) for ABI. Negative predictive values were 37%(95% CI, 24-51%) for digitial pulse oximetry and 43%(95% CI, 25-72%) for ABI. In 22 cases with acute ischemicwere the sensitivity 73%, the specifity 100%, the positive predictive value 100% and the negative predictive value 79%. CONCLUSIONS: Handheld digital pulse oximetry of the big toes seems as accurate as ABI to detect lower extremity arterial diseases. The combination of both will help to distinguish low extremity arterial disease and spinal radiculopathy.

8.Clinical Trial on the Hypotensive Effect of Buterazine.

The hypotensive effect and side reactions of Buterazine were evaluated in 21 patients with essential hypertension. The results were as follows; 1) Before medication and after 2 and 4 weeks of medication, the over all average systolic and diastolic pressure were 188+/-23/112+/-7, 168+/-13/101+/-17, and 158+/-12/95+/-8mmHg, respectively. After 2 and 4 weeks of medication, the over all average systolic pressure decreased by 20 and 30mmHg(P<0.005, P<0.001), and the over all average diastolic pressure decreased by 11 and 17mmHg(P<0.025, P<0.001), respectively. In 69% of all cases, marked or moderate degree of hypotensive effect was observed. 2) There was no significant changes in heart rates before and after treatment. 3) In 84% of all cases, improvement of symptoms were observed. 4) There was no side effects which required discontinuing the treatment, except 2 cases which discontinued the medication.
