2.Solitary Benign Neurilemmoma of the Sciatic Nerve, 2 Case Report

Neurilemmoma is a benign neoplasm arising from the Schwann cell of the nerve sheath. They are usually painless and are most common in the upper extremity. Pathologically, neurilemmoma is encapsulated and made up of organized elements: the Antoni A cells, the Antoni B cells and the Verocay bodies. Two cases of solitary benign neurilemmoma of the sciatic nerve are presented with a review of the literature. Tumors were completely enucleated and confirmed by pathological examination. The post-operative course has been satisfactory showing no recurrence for the period of more than one year.

3.A Case of Colonoscopically Removed Granular Cell Tumor in the Ascending Colon.

This case report describes a patient with semipedunclated granuylar cell tumor(granular cell myoblastoma) of the proximal ascending colon removed by endoscopic polypectomy. A 39-year old femalie was visited to our hospital for intermittent abdominal pain and loose stool. Colonoscopic examination showed a semipedunclated lesion covered with yellowish white smooth mucosa in the proximal ascending colon. The tumor was removed by endoscopic polypectomy. The collected specimen was 0.9x0.8 cm in size. The cut surface of the specimen showed a white round firm tumor covered with mormal epithelium of the colon. Using immunohistochemical study, the tumor cells were to be positively stained by S-100 protein and negatively stained by desmin, which supports the concept of the Schwann cell origin of granular cell tumor. In Korea, 2 cases of granular cell tumor of the colon and rectum, which were treated by surgical removal, have been reported. This case in the first endoscopic polypectomy of granular cell tumor of the colon and retum in Korea. The endoscopic polypectomy is thought to be useful for the therapy of small pedunclated or semipedunclated granular cell tumor, intracolonic type.

4.A Comparing Study of Herniorrhaphies Laparoscopy, Lichtenstein and Conventional Repairs.

PURPOSE: The optimal surgical technique for inguinal hernia repair continues to be debated. This study was designed to investigate optimal surgical procedures in inguinal or femoral hernia. METHOD: We analyzed 153 cases of herniorrhaphy on inguinal or femoral hernias between August 1996 and November 2000. We divided patient into four groups according to the methods of hernia repair, i.e., 1) 78 cases of laparoscopic herniorrhaphy, 2) 42 cases of Lichtenstein herniorrhaphy, 3) 24 cases of Bassini herniorrhaphy and 4) 9 cases of McVay herniorrhaphy. RESULTS: The patient in the laparoscopic and Lichtenstein herniorrhaphy groups needed shorter hospital stays than those in the Bassini or McVay herniorrhaphy groups. The severity of pain was assessed by the total amount and duration of nonsteroidal anti-inflammatory drug injections, which was minimal in the laparoscopic group. There were no differences in complications between the groups. One patient in the laparoscopy group had a hernia recurrence and was reoperated with Lichtenstein herniorrhaphy. We compared two tension-free herniorrhaphies with each other. The numbers of patients not needing analgesic injections were more in the laparoscopic than the Lichtenstein herniorrhaphy group, reflecting less pain in the former group. Hospital stays were also shorter in the laparoscopic than the Lichtenstein herniorrhaphy group. CONCLUSION: We concluded that tension-free herniorrhaphy is superior to tension herniorrhaphy in terms of postoperative pain & recovery. Of the tension-free herniorrhaphies, laparoscopic herniorrhaphy is associated with less postoperative pain and shorter hospital stays than Lichtenstein herniorrhpahy.

5.Clinical Study on the Antiplatelet Aggregation Effect of Ticlopidine Hydrochloride(Ticlid(R)).

The antiplatelet aggregation effect of ticlopidine was studied in 22 cases of coronary artery disease(CAD) and 17 cases of control by obseving changes of plarma beta-thromboglobulin(beta-TG) and platelet factor 4(PF-4) before and after administration of ticlopidine 500mmg daily for 2 weeks. 1) Compared with the controls, CAD patients had significantly greater plasm levels of beta-TG(52.6+/-32.7ng/ml. mean +/-SD vs. 91.0+/-52.0, P<0.05) and PF-4(17.5+/-12.8 ng/ml vs. 32.9+/-24.5, P<0.05). 2) In controls, plasma levels of beta-TG and PF-4 didn't change significantly after taking ticlopidine. 3) In CAD patients, plasma levels of beta-TG (91.0+/-52.0ng/ml vs. 53.9+/-20.0, P<0.05) and PF-4(32.9+/-24.5ng/ml vs. 18.8 +/-11.9(P<0.05) decreased significantly after ticlopidine. 4) The side effects were observed in 2 cases such as mild indigestion and urticaria.

6.Preoperative and Postoperative Echocardiographic Findings in Atrial Septal Defect.

Preoperative and postoperative echocardiography were performed in 33 patients with isolated ostium secondum atial septal defect(ASD), confirmed by right heart catheterization and operation at Seoul National University Hospital from November 1980 to June 1982. The ratio of right ventricular enddiastolic dimension to leftventricular enddiastolic dimension(RVED/LVED) was compared to the pattern of interventricular septum, before and after operation. In patients with persistent paradoxical septal motion during 2wks after operation, repeated echocardiograms were performed upto 9 months, postoperatively. The results obtained are as follows: 1) The ratio of RVED/LVED in patients with ASD, was 0.95+/-0.35(mean+/-standard deviation), which was significantly larger than that of 14 normal subjects(p<0.005). 2) There was statistically correlation between the ratio of pulmonary blood flow to systemic blood flow(QP/QS) and the ratio of RVED/LVED(r=0.44, p<0.025). 3) Thirty three patients were divided into 3 groups according to the pattern of intervent ricular septal motion.(Group "M": Patients with normal septal motion, Group "B": Patients with paradoxical type B septal motion, Group "A": Patients with paradoxical type A septal motion) Preoperatively, the ratio of RVED/LVED was 0.68+/-0.28 in Group "N" (N=9), and 0.88+/-0.23 in Group B (N=9) and 1.14+/-0.34 io Group "A" (N=15). In Group "A, the ratio of RVED/LVED was significantly larger than of Group "N" (p<0.005). Postoperatively, the ratio of RVED/LVED was 0.51+/-0.13 in Group "N" (n=23), and 0.68+/-0.15 in Group "B" (n=7), and 0.79+/-0.14 in Grop "A" (n=3). In Group "A" and Group "B, the ratio of RVED/LVED was significantly larger than that of Group "N" (p<0.005, p<0.01). 4) Postoperative RVED index(18.9+/-4.9) was significantly decreased, compared with preoperative RVED index(27.6+/-8.9) (p<0.005). But there was no significant change in LVED index, before and after operation. 5) Among 10 patients with persistent paradoxical septal motion, repeated echocardiograms were performed in 5 patients. Interventricular septal motion was normalized in 4 out of 5 patients. Thus the ratio of RVED/LVED has significant correlation with QP/QS and the pattern of interventricular septal motion. These results suggest the evidence that interventricular septal motion is determined by septal position at enddiastiole.

Recently therapeutic efficacy of stomach cancer was greatly improved due to early diagnosis and irnproved diagnostic and therapeutic modalities. Especially in early gastric cancer, 5 year survival rate is near 100%. Recarrence of early gastric cancer after curative surgery is rarely reported. Recently authors experienced one case of recurrent stomach cancer arising from remmnant stomach 14 years after gastrectomy of EGC type I(m), so we reports it here with review of literatures.

9.Follow-up Results of Percutaneous Pulmonic Balloon Valvuloplasty in Adult with Pulmonic Stenosis.

Percutaneous pulmonic balloon valvuloplasty(PPV) was performed in 23 patients with moderate to severe degree of pulmonary stenosis(PS) from March, 1986 to May, 1991. The patients were 26.4 years old, range from 15-44, and 13 of them were males. 19 patients of them had valvular stenosis and the other 4 patients had combined combined stenosis with infundibular stenosis. Associated cardiac defects were atrial septal defect(ASD) in 3 patient, patient foramen ovale(PFO) in two patients, and patent ductus arteriosus(PDA) in one patient. Transpulmonic peak systolic pressure gradient(big up tri, openP) decreased from 88.6+/-34.2mmHg to 31.2+/-21.3mmHg after PPV(p<0.01). The right ventricular peak systolic pressure(RVSP) decreased from 105.8+/-35.3mmHg to 47.1+/-20.3mmHg(p<0.01). after PPV. In 7 patients, follow-up(F/U) cardiac catheterization and right ventricular angiography was performed 13 months later(6mon-29mo). big up tri, openP at pre PPV, at immediate post PPV and at long-term F/U were 90.3+/-44.3mmHg, 29.6+/-23.4mmHg and 21.9+/-11.9mmHg, respectively. There was significant decrease in big up tri, openP between pre PPV and immediate post PPV(p<0.01), but no significant change between the immediate post PPV and long-term F/U. The RVSP at pre PPV, at immediate post PPV and at long-term F/U were 109.6+/-45.4mmHg, 48.1+/-23.0mmHg and 43.3+/-13.1mmHg, respectively. There was significant ddecrease in RVSP between pre PPV and immediate post PPV(p<0.01), and no significant decrease in RVSP between immediate post PPV and long-term F/U(p>0.05). The sum of amplitudy of R wave in V1 and that of S wave in V6 in ECG(R1+S6) represents the degree of rigth ventricular hypertrophy. The ECG was followed-up for 15.3months(6~29months) in 11 patients and R1+S6 at pre PPV and at follow-up were 15.9+/-9.1mm and 9.3+/-4.1mm(p<0.01). In summary PPV can achieve very good short-term and long-term results in relieving moderate to severe pulmonic stenosis in adult.

10.A Case of Truncus Arteriosus.

Truncus arteriosus is a rare congenital heart disease which is diagnosed in from 1 to 2 percent of congenital cardiac birth. Whithout surgical intervention, survival beyond infancy is unusual. Unoperated patients who survive to adult life have associated pulmonary stenosis or have developed pulmonary arteriolar disease. We report a case of truncus arteriosus in a 17-year-old man with a review of literature.
