2.Efficacy of traditional Chinese medicine in the treatment of intrahepatic cholestasis of pregnancy

Objective To discuss the efficancy of traditional Chinese medicine in treatment of intrahepatic cholestasis of pregnancy .Methods A total of 60 patients who were admitted to our hospital for severe intrahepatic cholestasis of pregnancy(ICP) from January 2009 to December 2012 were randomly divided into two group using random number table .Patients in study group (n=30) took traditional Chinese medicine three times a day plusUDCA twice a day orally .Patients in control Group (n=30)were on‐ly given UDCA twice a day orally .Conventional treatment and care for ICP were given to both groups .Biochemical index (TBA , ALT ,and AST)level ,pregnancy outcome and Caspase‐3 of the two groups were recorded and compared and compared .Results The expressing of Caspase‐3 in placental cytotrophoblast ,syncytiotrophoblast .Decidual cells and interstitial cells of study group were (10 .58 ± 2 .62)% ,(15 .8 ± 4 .02)% ,(10 .17 ± 3 .58)% ,(14 .23 ± 4 .39)% ,respectively ,which were significantly lower than those in control group(P<0 .05) ,which were (20 .46 ± 8 .68)% ,(33 .89 ± 7 .05)% ,(20 .56 ± 4 .23)% ,(28 .29 ± 5 .58)% .TBA ,ALT、AST in two groups were declined ,and ctedy group was better than in control group(P<0 .05) .The index of liver function were all lower in study group than that of control group(P<0 .05) .Incidence rate of neonatal asphyxia was lower in study group than that of con‐trol group (P<0 .05) .Conclusion Traditonal Chinese medicine can reduce the expression of Caspase‐3 in study group .It is safe and eflfective in the treatment of ICP .
3.Reconsidering the use of neoadjuvant chemotherapy in breast cancer

With the intense development of neoadjuvant chemotherapy for breast cancer research in recent years, this approach has been used widely in clinical treatment. Neoadjuvant chemotherapy can increase the chance of operation for patients with local y advanced breast cancer. Moreover, it can improve breast-conserving rate in larger-tumor patients, thereby improving the quality of life of these patients. However, some controversies regarding the use of neoadjuvant chemotherapy for breast cancer patients remain to be resolved. This review focuses on clinical studies and re-recognition of neoadjuvant chemotherapy as a viable treatment for breast cancer.

8. Effects of dexmedetomidine on isoflurane-induced neuroapoptosis and expression of CRMP2 in neonatal rat hippocampus

OBJECTIVE: To investigate the effects of dexmedetomidine(Dex) preconditioning on neuroapoptosis in hippocampus induced by isoflurane and the expression of CRMP2 proteins in neonatal rats. METHODS: Sixty neonatal rats at postnatal day 7 were assigned randomly into control group, Dex preconditioning control group, isoflurane group and isoflurane with Dex preconditioning groups which at dose of 25, 50 and 75 μg·kg-1. Rats in groups of isoflurane or control were exposed 0.75% isoflurane or air for 6 h. Rats in groups of Dex preconditioning were injected intraperitoneally with different doses of Dex 20 min before exposure and rats in other groups were injected intraperitoneally with 150 μL saline. At the end of exposure, the hippocampus of some rats were separated and proteins expression of cleaved caspase-3, phospho-CRMP2 (p-CRMP2), CRMP2 were detected by Western blot (n=6); other rats were perfused and their brains were embedded by paraffin, the neuronal apoptosis in hippocampus CA1 was detected by TUNEL staining 2 h after exposure (n=4). RESULTS: The number of TUNEL positive cells in hippocampus CA1 of rats in isoflurane group was increased by 434.99% (P<0.001) when compared with control group, Dex at dose of 75 μg·kg-1 significantly inhibited the increase of isoflurane-induced TUNEL positive cells by 71.87% (P<0.001). The expression of cleaved caspase-3 protein in isoflurane group increased by 90.20% (P<0.001) compared with control group, while Dex at dose of 25, 50 and 75 μg·kg-1 reversed isoflurane-induced increase of cleaved caspase-3 by 30.54% and 53.71%(P<0.05)and 96.71% (P<0.001)separately. Isoflurane increase the ratio of p-CRMP2/CRMP2 in the hippocampus by 119.41% (P<0.001), and didn't influence the expression of CRMP2; while Dex at dose of 50 and 75 μg·kg-1 inhibited isoflurane-induced increase of p-CRMP2/CRMP2 ratio by 63.75% (P<0.01) and 77.70% (P<0.01) and increased the expression of CRMP2 by 44.30% (P<0.05) and 62.13% (P<0.01). CONCLUSION: Dex Attenuates isoflurane-induced neurodegeneration through inhibitory effect of hippocampal neuroapoptosis in neonatal rats, decrease the ratio of p-CRMP2/CRMP2 and increase the expression of CRMP2 may be one of the mechanisms for neuroprotection and this Conclusion may provide the theoretic foundation for reasonable selection of anaesthetics.
9.Effective penetration of cell-permeable peptide mimic of tyrosine residue 654 domain of beta-catenin into human renal tubular epithelial cells.

Phosphorylation of beta-catenin tyrosine residue 654 plays an important role in the epithelial to myofibroblast transition (EMT). Introducing mimic peptide of tyrosine residue 654 domain of beta-catenin into cells may influence phosphorylation of beta-catenin tyrosine residue 654. To deliver this mimic peptide into renal epithelial cells, we used penetratin as a vector, which is a novel cell permeable peptide, to deliver hydrophilic molecules into cells. A tyrosine 654 residue domain mimic peptide of beta-catenin (PM) with fused penetratin was constructed, purified and then detected for the penetration of the mimic peptide into human renal tubular epithelial cells (HK-2). The results showed that purified fusion mimic peptide could efficiently and rapidly translocate into human renal tubular epithelial cells. It is concluded that a cell-permeable peptides mimic of tyrosine residue 654 domain of beta-catenin was successfully obtained, which may provide a useful reagent for interfering the human renal tubular epithelial-mesenchymal transition.

10.Clinical effect of self made Jianpi Yiqi Xiaoyu decoction combined with FMC chemotherapy for treatment of elderly patients with moderate to advanced gastric cancer: results of a randomized controlled study

Objective To discuss the clinical effect of self made Jianpi Yiqi Xiaoyu decoction combined with FMC chemotherapy for treatment of elderly patients with moderate to advanced gastric cancer and its impact on their quality of life.Methods One hundred and two elderly patients with moderate to advanced gastric cancer accompanied by the Chinese medicine dialectical syndrome belonging to deficiency of spleen and stomach qi and blood stasis admitted to Zhengzhou Traditional Chinese Medicine Hospital from January 2013 to January 2016 were enrolled,they were divided into observation group and control group according to random number table method and 2 patients who missed the follow-up in each group were excluded,resulting in 49 patients in each group;the patients in the control group were treated with FMC chemotherapy [including mitomycin 15 mg/m2,cisplatin 60 mg/m2 and fluorouracil 1000 mg/m2,slow intravenous drip (drop) after dilution with 60 mL 0.9% sodium chloride injection];the patients in observation group were treated with FMC chemotherapy combined with Jianpi Yiqi Xiaoyu decoction (consisting of astragali 30 g,codonopsis pilosula 20 g,atractylodes macrocephala 15 g,Chinese yam 15 g,hoelen 15 g,ligustici 10 g,moutan cortex 10 g,aurantii 10 g,rehmannia dride rhizome 10 g,caulis dendrobii 10 g,500 mL water was added to make a decoction,boiled and concentrated to 200 mL decoction,100 mL taken in the morning and another 100 mL in the evening orally in one day).The clinical efficacy,the change of quality of life and incidence of adverse reactions were compared between the two groups.Results The proportion of patients whose quality of life improved in the observation group was significantly higher than that in the control group [55.10% (27/49) vs.16.33% (8/49),P < 0.05].The total effective rate was obviously higher in the observation group than that in the control group [51.02% (25/49) vs.22.45% (11/49),P < 0.05];the incidence of reduction of hemoglobin [30.61% (15/49) vs.63.27% (31/49)],aleucocytosis [28.57% (14/49) vs.67.35% (33/49)],thrombocytopenia [28.57% (14/49) vs.69.39% (34/49)],nausea and vomiting [44.90% (22/49) vs.71.43% (35/49)],diarrhea [32.65% (16/49) vs.63.27% (31/49)] and neurotoxicity [34.69% (17/49) vs.55.10% (27/49)] were significantly lower in observation group than those in the control group (all P < 0.05).Conclusions Self made Jianpi Yiqi Xiaoyu decoction combined with FMC chemotherapy can significantly improve the clinical efficacy of theelderly patients with moderate to advanced gastric cancer.It can improve the patients' quality of life and reduce the incidence of adverse reaction of chemotherapy.
