2.Measurement of the Lumbar Spinal Canal by the Plain X-Ray Film in the Normal Korean Adults

The significance of measurement of the spinal canal size was well recognized in case of detection of intraspinal tumor as well as diagnosis of spinal stenosis. The measurement of sagittal diameter of the lumber spinal canal by plain x-ray film is not always easy. However, the new method designed by Eisenstein made the measurement of sagittal diameter of the lumber spinal canal by plain x-ray film very easy. In order to study the range of normal values of the sagittal diameter of lumber spinal canal in Korean adults, authors measured antero-posterior and transverse diameter of the lumbar spinal canal in 290 normal Korean adults by Eisensteins method. The results obtained are as follows: 1. Mean values of sagittal diameter of the normal Korean adults male was 17.37±1.05mm at Ll, 16.43±1.12mm at L2, 15.89±1.20mm at L3, 15.45±1.24mm at L4, and 17.34±1.13mm at L5 vertebra and in female, 17.41±1.23mm at Ll, 16.45±1.20mm at L2, 16.14±1.41mm at L3, 15.40±1.32mm at L4, and 17.25±1.20mm at L5 vertebra. 2. Mean values of transverse diameter of the normal Korean adults male was 23.54±1.18mm at Ll, 24.61±1.17mm at L2, 25.78±1.41mm at L3, 27.72±1.86mm at L4, and 30.42±2.23mm at L5 vertebra and in female, 23.06±1.45mm at Ll, 23.90±1.48mm at L2, 25.21±1.53mm at L3, 26.62±1.88mm at L4, and 29.70±2.41mm at L5 vertebra. 3. In transverse diameter, it was increased more and more to the lower lumber level, the widest at L5 vertebra. 4. In sagittal diameter, it was decreased less and less to the lower lumbar level, the narrowest at L4, and increased again at L5 vertebra. 5. The difference of sagittal and transverse diameter of lumbar spinal canal at each level of lumbar spine was very significant statistically. 6. The difference of the transverse diameter of the lumber spinal canal between male and female was significant statistically but that of the sagittal diameter was not. 7. The difference of the sagittal and transverse diameter between each age group was not significant statistically.

5.The Biomechanical Study for the Initial Stability of Porous - coated Acetabular Components in Total Hip Arthroplasty.

The purpose of this study was to investigate the gaps of bone-acetabular component interface, the initial mechanical stability and the effects of adjuvant screw fixation in the press-fitted two different shaped porous-coated acetabular components. Semi-hemispherical Harris-Galante II cup and full hemispherical Trilogy cup were press-fitted with or without adjuvant screw fixation in 40 bovine femur. The gaps of bone-implant interface were measured in each radiography of all specimen and axial compression tests and rotational stability tests were performed using servohydraulic tension-compression machine. There was significantly smaller gap at polar 1/3 region in the specimen implanted with Trilogy cup. But the adjuvant screw fixation did not affect the amount of gap. There was no significant statistical difference in axial load and rotational stability test in each situations of fixation. This study demonstrates that the initial mechanical stability is not influenced by the shape of semi-hemispherical Harris-Galante II cup and full hemispherical Trilogy cup and also independent of adjuvant screw fixation. But smaller gap of Trilogy cup at the polar 1/3 region indicates the possibility that less amout of wear debris of polyethylene or metal will be deposited and therefore less osteolysis or cup loosening will occur, but this hypothesis should be confirmed by the long-term clinical follow-up.

7.Analysis of X-Ray Findings of Osteosarcoma

The diagnosis of osteosarcoma is based on both clinical, rediological and histopathological findings, but the majority of the patients with osteosarcoma can be diagnosed with a reasonable degree of confidence from the appearance of the lesion on the plain X-ray film. The combination of soft tissue mass and calcification, increased intramedullary radiodensity, cortical bone destruction,and marked periosteal reaction such as Codmans triangle and sunburst reaction is the radiographic hallmark of osteosarcoma, Among the above mentioned radiological findings, the presence of soft tissue mass with calcification can be so strongly suspected to be nearly diagnostic on the X- ray examination alone. The author performed radiological analysis of 40 cases of osreosarcoma which was experienced at the Department of Orthopedic Surgery, College of Medicine, Korea University including other hospitals from 1975 to 1980 and particulary observed correlation between the soft tissue mass with calcification and other radiological findings. The results obtained as follow: 1. Of the 40 cases, soft tissue mass with calcification were observed in 32 cases(80%), cortical destruction in 31 cases(77.5%), periosteal reaction in 24 cases(60%), sunburst reaction in 23 cases(57%) and Codmans triangle in 21 cases were observed. 2. Of the 40 cases, 23 cases(57.5%) were osteoblastic, 12 cases(30%) of osteolytic and 5 cases(12.5%) of mixed type. 3. The shadow of soft tissue mass with calcification was observed 11 cases(91.7%) among 12 cases of osteolytic type, 28 cases(91%) among 31 cases of cortical destruction, 21 cases(87.5%) among 24 cases of periosteal reaction, 20 cases(86.9%) among 23 cases of sunburst reaction and 18 cases (85.7%) among 21 cases of Codmans triangle.

8.Emphysematous Cholecystitis: A Case Report.

Emphysematous cholecystitis is an uncommon condition which may mimic acute cholecystitis. But it differs from acute cholecystitis in its relatively greater frequency in men and diabetics and has graver prognosis. The condition is diagnosed by demonstration of air in lumen,wall of gallbladder and/or pericholecystic space using a variety of radiographic techniques: simple abdominal radiography, ultrasonography and CT scanning. One illustrative case is presented herein and the pertinent literature is reviewed.

9.Limbal-Conjunctival Autograft Transplantation for the Treatment of Primary Pterygium.

Pterygium is a common surgical ocular disease which recurs frequently. Various different techniques have been developed for the successful surgical treatment of pterygium and among them, autograft transplantation is known as the best successful surgical method. For the prevention of recurrence, the corneal epithelium must heal before the fibrovascular tissue grows and reaches to limbus. For this, we have believed that limbal transplanta- tion is more suitable than conjunctival transplantation. We had performed limbal-conjunctival autograft trasplata- tion for 52 patients with primary pterygia whose age was under 40 years old,or whose pterygia had involved above 5mm beyond limbus and had shown diffuse subconjunctival fibrovascular tissue. The recurrence rate was 1.9%and therewere no complications and no surgical problems. Therefore,we recommend this surgical technique as a safe and effective method of treating primary pterygia with the risk factors of recurrence.

10.Obturator Nerve Block with Botulinum Toxin Type B for Patient with Adductor Thigh Muscle Spasm: A Case Report.

Obturator nerve block has been commonly used for pain management to prevent involuntary reflex of the adductor thigh muscles. One of several options for this block is chemical neurolysis. Neurolysis is done with chemical agents. Chemical agents used in the neurolysis of the obturator nerve have been alcohol, phenol, and botulinum toxin. In the current case, a patient with spasticity of the adductor thigh muscle due to cervical cord injury had obturator nerve neurolysis done with botulinum toxin type B (BoNT-B). Most of the previous studies have used BoNT-A with only a few reports that have used BoNT-B. BoNT-B has several advantages and disadvantages over BoNT-A. Thus, we report herein a patient who successfully received obturator nerve neurolysis using BoNT-B to treat adductor thigh muscle spasm.
