2.A Histopathologic, Histochemical and Immunocytochemical Study of Cardiac Myxoma.

The immunoreactivities of surgically removed 16 cardiac myxomas were studies for factor VIII-related Ag (F VIII-RA), Ulex europaeus agglutinin I (UEA-I) and desmin alpha1-antichymotrypsin, and this study was accompained by a clinicopathologic review. More than 50% of the patients with cardiac myxomas were in their fourth and fifth decades, and cardiac myxomas were much more common in women than in men. All but on occurred in the left atrium, and the majority were attached to the atrial septum, usually in the region corresponding to the fossa ovalis. In one case, an atrial myxoma recurred 37 months after the initial excision. Microscopically, the myxomas contained a myxoid matrix composed of acid mucopolysaccharides within which were embedded polygonal cells. The cells forming both the surface and complicated vascular like channels throuhout the myxoid stroma tested positive for F VIII-RA and UEA-I. The outer cell layers of the complex vascular structures demonstrated variable staining for F VIII-RA, while isolated bundles of smooth muscle cells were present and stained for desmin. A small number of the so-called myxoma cells, immunoreactive for alpha1-antichymotrypsin which were not laden with hemosiderin pigment but were similar to histiocytes, were present particularly around the areas of hemorrhage. These findings support the current view that cellular and histologic heterogeneity arose from the divergent differentiation of multipotential mesenchymal cells. In particular, it remains to be confirmed by further study whether or not true histiocytic differentiation occurs.

3.Detection of Major bcr/abl mRNA from Stored Bone Marrow Aspirate Smears Using the Reverse Transcriptase Polymerase Chain Reaction.

BACKGROUND: The verse transcriptase polymerase chain reaction (RT-PCR) has been widely used to analyze the bcr/abl fusion mRNA in chronic myelogenous leukemia (CML). Fresh or cryopreserved cells may not always be available for molecular diagnosis. So we investigated the value of stored bone marrow aspirate smears as the sources of material for the detection of bcr/abl mRNA. METHODS: We extracted RNA using modified Chomczynski method, and amplified bcr/abl mRNA by RT-PCR from the 70 cases of bone marrow smear slides stored from 7 days to 7 years, which were comprised of 49 CML, 11 other chronic myeloproliferative disorders (CMPD) and 10 acute lymphoblastic leukemia (ALL). Sensitivity of RT-PGR was tested using the slide smears prepared with 10(0)-10(6) K562 cells, and RT-PCR results losing each fresh bone marrow cellular suspension and slide smears in 24 patients were compacted. For major bcr/abl rearrangement, RT-PCR was performed by nested PGR afters GDNA synthesis losing downward primer and beta2-microglobulin was used as RNA controls. RESULTS: The sensitivity of RT-PCR for detecting bcr/abl mRNA was l02 cells per slide. Sixty one cases (86%) of 70 bone marrow aspirate smears showed positive results of beta2-micyoglobulin cDNA as an indicator of intact RNA. Thirty nine cases of 42 beta2-microglobulin cDNA positive CML bone marrow aspirate smears showed 29 b3a2 type mRNA and 10 b2a2 type mRNA. Nine cases of 11 bone marrow aspirate smear with other CMPD showed negative results of bcr/abl mRNA. Two cases of 10 ALL bone mallow aspirate smears had b2a2 type mRNA and b3a2 type mRNA, respectively. The results for detection of bcr/abl mRNA with fresh cell suspensions of 24 patients were same as the bone marrow aspirate smears storied for 7 days to 1 year. CONCLUSIONS: These data indicated that RNA obtained from bone marrow smears storied for less than 1 year was valuable as the source of RT-PGR for the detection of bcr/abl mRNA in CML and the bone marrow smears stored for much longer period ould be assailable as the specimens for retrospective analysis of specific gene alter-ation in other hematologic malignancy.

4.Leiomyoma of the Ovary A report of two cases.

Ovarian leiomyoma is a rare form of the ovarian mesenchymal neoplasm and about 50 cases have been reported in the literature. It is believed that many cases may go unnoticed because they are usually small in size and frequently mistaken for the more common fibroma or fibrothecoma. Its origin is still controversial and many possibilities are considered including the smooth muscle in the blood vessel wall of the hilum or the multipotential ovarian stromal cell. Herein we describe two cases of ovarian leiomyoma with its characteristic histologic finding.

6.Operative Treatment for Cubital Tunnel Syndrome

Thirteen patients were operated for cubital tunnel syndrome and followed for an average of 26 months postoperatively. Ten patients had a history of relevant trauma and three patients had degenerative osteoarthritis of the elbow. The average duration of symptoms was 18 months (range, 2 to 96 months). Diagnosis was made by physical examination, electromyography and nerve conduction study. Among these, nerve conduction study was found to be the most valuable diagnostic method for the patients with atypical clinical findings. Most of the operations were performed by anterior transposition of the ulnar nerve. At the most recent follow-up, the result was excellent in two patients, good in eight, and fair in three; thus ten patients(77%) showed satisfactory results. The rating system for ulnar neuropathy based on sensory, motor dysfunction and pain was useful for evaluating the operative results. The postoperative gain of score for pain and sensory function were larger than that of motor function. Factors known to influence the result of the operation (age, duration of symptom, history of trauma, method of operation) did not effect the outcome in this study. For successful operation, the ulnar nerve must be thoroughly examined, all possible levels of compression must be released and new foci of compression must be created.

7.Evaluation of Child Health Information Articles in Newspapers.

The purpose of this study was to take the right direction and meet the requirements of newspaper function about child health through evaluation of child health information articles in newspapers. Data were collected 4 main daily newspaper by selecting child health information articles during 1 year from January 1 to December 31, 1998. The results of this study are as follows. The frequency according to health category, disease treatment(47.7%) topped followed by health maintenance?promotion(28.8%), growth?development(12.1%), disease prevention(11.4%). The frequency according to WHO international disease classification, infectious disease (23.6%) take most. In evaluation area of child health information, practical usage(3.78) topped followed by accuracy(3.68), comprehensiveness(3.64), clearness (3.48), concreteness(3.33).

9.Effects of Transforming Growth Factor-beta on Proliferation, Collagen synthesis, Migration and Metalloproteinase Secretion of Human Retinal Pigment Epithelial Cells.

PURPOSE: This study was performed to evaluate the effect of TGF-beta on proliferation, collagen synthesis, migration, and matrix metalloproteinase (MMP) secretion of human RPE cells in vitro. METHODS: The cultured human RPE cells were treated with either TGF-beta1 or TGF-beta2 in concentrations of 0, 0.1, 1, 10 ng/ml respectively. The cell number was measured in 3, 6, 9 days, and the collagen synthesis and cell migration was measured. [3H]-thimidine uptake assay was done to evaluate the change of DNA synthesis. And the secretions of MMP1, MMP2, MMP3, MMP9, TIMP1 (tissue inhibitor of metalloproteinase), and TIMP2 were measured by electrophoresis and Western blot analysis. RESULTS: TGF-beta1 and TGF-beta2 significantly inhibited the proliferation of RPE cells in a concentration -and time-dependent manner (p<0.05). [3H]-thymidine uptake was decreased by TGF-beta1 and TGF-beta2 in a concentration-dependant manner. The collagen synthesis of RPE cells was significantly increased by high concentration of TGF-beta1 and TGF-beta 2. However, the migration of RPE cells was not affected by TGF-beta. As the concentration of TGF-beta1 and TGF-beta2 increased, the secretions of MMP1, MMP2, MMP3 and MMP9 decreased, while the secretion of TIMP1 and TIMP2 increased after 72 hours. CONCLUSIONS: These results suggest that the TGF-beta1 and TGF-beta2 may have critical effect on the development of PVR and provide clues to possible therapeutic solutions for controlling PVR process.

10.The Degree of ZMother's Accident Prevention Practices for Their Children.

Accidents are the leading cause of death in children worldwide. The purpose of this study is to use basic data of safety education, counseling, and information available regarding accident prevention to examine the degree of mother's accident prevention practices for their children. Data were collected from June to October, 1998 from 587 mothers including 2 University hospitals located in Seoul and KyoungKi-Do. By using the 30 item questionnaire, which was created by researchers through literature review, the degree of mothers' accident prevention practices for their children was measured. The degree of accident prevention practices of mothers shown ranged 70-118 and averaged 3.19. Compared to the composit area average score, drug keeping had the highest score of 3.57. The next highest scores and traffic safety(3.41), and super vision of child(3.30). Prevention against burning was the lowest point at 2.58. With the respect to the demographic characteristics, there were statistically significant differences in the mothers' education(F=4.291, p=.014), type of residence(F=3.979, p=.008), and child developmental age(F=5.275, p=.001). The degree of accident prevention practices of mothers were relatively high. But the area which showed the low degree of accident prevention practices, required nurses' active education, counseling, social interest and support, and mass media participation.
