1.A Case of Livedo Reticularis Associated with Decompression Sickness.

Livedo reticularis is a mottled bluish discoloration of the skin which occurs in a netlike pattern and is not a diagnosis in itself, but is a nonspecific reaction pattern. It may be classified as idiopathic and secondary livedo reticularis. Decompression sickness can occur during decompression after diving into deep sea water or during a rapiid ascent from sea level, and is one of the many causes of the secondary livedo reticularis. We report a case of livedo reticulris which developed in a patient with decompression sickness. A biopsy from the purpuric patch revealed an unusual histopathologic finding that resembles those of bullae and sweat gland necrosis in drug induced coma.

2.A Case of Transient Acrodermatitis Enteropathica in a Full-Term Breast-Fed Infant.

We herein report a case of transient acrodermatitis enteropathica in a 3-month-old, breast-fed, full-term infant. The patient was presented with a 2-month history of diarrhea and crusted erythema- tous patches on the periorificial area. Similar lesions were seen in his siblings. His serum zinc level and the zinc level in his mother's breast milk were markedly reduced. Diarrhea and skin lesions disappeared promptly with oral zinc supplementation and did not recur when zinc was discontinued after three weeks. Our case indicates that even full-term infants, who feed excl-usively on mothers milk, run a risk of developing zinc deficiency, if the concentration of zinc in the breast milk is very low.

6.Therapeutic Efficacy of Ciclopirox Nail Lacquer in the Treatment of Onychomycoses.

BACKGROUND: Ciclopirox 8% nail lacquer is a new topical antifungal agent which is character ized by excellent penetration into the nail plate to the infected koratin and remains there for a prolonged period. We per formed a st,udy to assess the therapeutic efficacy and safety of ciclopirox 8% nail lacquer in the treatment of onychomycoses. METHODS: We used 20 satients with onychomycoses, which was confirmed by KOH ancl fungus culture. The ciclopirox nail lacquer was applied once daily for 1 month, and followed by two times weekly for the next 5 months. The clinical observat.ion was perforrned every month for any clinical improvement and side-dffects. The involved percent of a target nail was measured by using a adhesive tape at.ached to the selected nail plate. All involved nails(n=68) were also evaluated by a severity score(sever>50%. moderate 25-50%, mild<25%). RESULTS: Seventeen patients were finally assessed because of the deletion of 3 patients througl loss of contact. The involved percent of target nail was significantly decreased from 57.4% at baseline to 27.9% througli the 6 month-treatment(p<0.01, ANOVA). The total reduced precent of target nail involvement at the end of the treatment was 52+22% on average. The precent of severly involved nail(>50%) in all 68 involved nails was also decreased from 56% at baseline to 7% by the end of the treatment The mycologieal cure rate was 82.4% by the end of the treatment. The overall efficaor assessed by the physician and patients was improvement by 82.4% Tolerability was excellent or good in all patients, but cosmetic acceptance was good or excellen by 53%. Only one patient complained of a prickling sensation, but no systemic adverse effect were found. CONCLUSION: These data suggest that ciclopirox 8% nail lacquer may be safe and effective ir the treatment of onychomvcoses.

7.Multiple Dermatofibromas in a woman with Systemic Lupus Erythematosus.

Multiple dermatofibromas have been reported in patients with various autoimmune disorders such as systemic lupus erythematosus in receiving immunosuppressive therapy. We report a case of systemic lupus erythematosus in a 30-year-old woman who developed 23 dermatofibromas while she was treated with systemic corticosteroid. The mechanism for the development of multiple dermatofibromas in the setting of immune disturbance remains unclear. The altered immune system may play a role in the pathogenesis of this cutaneous condition.

8.Erythema Multiforme Induced by Topical Application of Viru-Merz® Ointment.

Viru-Merz® ointment,1% tromantadine hydrochloride, is a topical antiviral agent used for recurrent herpes simplex. There are many reported cases of contact dermatitis by tromantadine.But erythema multiforme-like eruptions in association with Viru-Merz® Ointment has not been reported. A 31-year-old woman, who applied Viru-Merz® ointment for a recurrent herpes simplex labialis, developed an allergic contact dermatitis. Erythema multiforme like eruptions with typical target feature followed around the eczematous lesions on the face, and spread progressively on the neck and trunk. A patch test revealed positive reaction to Viru-Merz® ointment.

9.A Case of Traumatic Epidermal Cyst Containing Glass Pieces.

We report a case of a 40-year-old man with a traumatic epidermal cyst on the lateral malleolus. It was unusual that the cyst contained glass pieces as a foreign body, which did not induce foreign body reaction in both clinical and histopathological aspects. To our best knowledge, this is the first case of an epidermal cyst to manifest as a foreign body cyst containing glass pieces in cystic contents.

10.Expression of Involucrin and Filaggrin in Various Skin Disorders: Immunohistochemical Study.

BACKGROUND: The cornified cell envelope (CE) is a unique structure found in the terminal differentiation of the skin. The precursor proteins of the CE are composed of many candidate structural proteins, among which invalucrin and filaggrin are important ones to participate in the complicated process of forming, the complex structure. OBJECTIVE: The purpose of this study was to evaluate the usefulness of expression of involucrin and filaggrin as markers of terminal differentiation in various skin disorders including tumors. METHODS: Immunohistochemical studies were performed in the formalin-fixed, paraffin-embedded skin samples of non-tumors (41 cases) and tumors (43 cases).
