2.Hepatocellular Carcinoma with Immature T-Cell (T-lymphoblastic) Proliferation.

Indolent T-lymphoblastic proliferation has been rarely reported in the upper aerodigestive tract. The lymphoid cells associated with this condition have the morphological and phenotypical features of immature thymocytes. However, their pathogenesis and biology are unknown. We present an unusual type of tumor infiltrating lymphocytes in a case with hepatocellular carcinoma, presumed to be a T-lymphoblastic proliferation. A 58-yr-old female patient presented with indigestion and a palpable epigastric mass. The abdominal computed tomography revealed a mass in the S6 region of the liver. A hepatic segmentectomy was performed. Microscopic examination showed dense isolated nests of monomorphic lymphoid cells within the tumor. Immunohistochemically, the lymphoid cells were positive for CD3, terminal deoxymucleotide transferase (TdT) and CD1a. In addition, they showed dual expression of CD4 and CD8. The polymerase chain reaction used to examine the T-cell antigen receptor gamma gene rearrangement showed polyclonal T-cell proliferation. This is the second case of hepatocellular carcinoma combined with indolent T-lymphoblastic proliferation identified by an unusual tumor infiltrating lymphocytes.

3.A Foreign Body in the Urethra: A Case Report.

Most foreign bodies in the lower genitourinary tract are self-inserted through the urethra for masturbation. It can be diagnosed by clinical histories and physical examinations of the patient, endoscopy and radiologic evaluations. Extraction should be tailored as the surgery, endoscopic manipulation and dissolution according to the nature of the foreign body and should minimize bladder and urethral injury. We report a case of a long wire in the bladder and urethra with brief review of literatures.

4.Influence of Pain and Urinary Symptoms by Chronic Prostatitis-like Symptoms on Quality of Life in Young Men.

PURPOSE: To estimate the influences of pain and urinary symptoms on quality of life and determine the item that has the most predicting ability for quality of life in young men with chronic prostatitis-like symptoms. MATERIALS AND METHODS: Chronic prostatitis-like symptoms were measured by the National Institutes of Health-Chronic Prostatitis Symptom Index. of 28,841 men aged 20 years dwelling in the community, 16,321 men(a response rate 56.6%) completed self-administered questionnaires. A total of 1,057men who complained symptoms suggestive of chronic prostatitis were included in the study. The influences of pain and urinary symptoms on quality of life were determined by the logistic regression analysis. The receiver operating characteristic(ROC) curve was used to provide the predicting ability of each item quality of life. RESULTS: In multivariate analysis, pain and urinary symptoms were associated with an increased likelihood of impaired quality of life, and pain contributed more than urinary symptoms. Among items of pain and urinary symptoms, pain severity had the most predicting ability for impact, overall quality of life and quality of life impact domain. The areas under the ROC curves for impact, overall quality of life and quality of life impact domain were 71.3%, 69.3% and 67.0%, respectively. CONCLUSIONS: Urinary symptoms as well as pain may be associated with an increased likelihood of impaired quality of life in young men with chronic prostatitis-like symptoms. In addition, our findings suggest that pain severity is the most influencing factor for quality of life in this population.

5.A Case of Anuria and Urinary Ascites in a Premature Infant due to Bilateral Ureteropelvic Fungal Bezoars.

A 3-month-old female presented with anuria and abdominal distention for 1 day. The patient was twin and was delivered at 30 week-term with low birth weight. Emergent ultrasonography revealed both hydronephrosis and hyperechogenic mass within the both renal pelves. Urinoma around left kidney was also found. Computed tomography showed high density material filled dilated pelvises. Urinoma measured 3x4cm in the left prerenal space and urinary ascites in the right abdomen were found. Candida albicans was cultured from catheter drained urine. We started the care with percutaneous nephrostomy with amphotericin B irrigation, coupled with systemic antifungal therapy. And this management had a major role in the successful outcome.

6.The Preventive Effect of Systemic Treatment with Interferon-alpha2B for Infertility of Mumps Orchitis.

Postpubertal mumps may result in ochitis and permanent testicular atrophy may develop following infection. This present study was initiated to evaluate the preventive effect of interferon-alpha2B on infertilty after mumps orchitis. There were 21 patients with mumps orchitis between May 1990 and June 1997. Patients were randomly distributed into 2 groups: group 1 patients (n=13) maintained therapy with interferon-alpha2B (3x10(6) IU per day) and group 2 were managed by conservatively. All of the patients were evaluated with testis size measurement, mumps virus titer, hormone level, and if possible semen analysis. For group 1 patients symptoms disappeared within 2 to 3 days and the volume of testis returned to normal within 11 days and testis atrophy was not observed in all patients in follow up. But asthenospermia was continued in 4 patients (unilateral 2, bilateral 2). For group 2 patients symptoms disappeared within 5 to 6 days and the volume of testis returned to normal within 10 days and testis atrophy was observed in 3 patients (unilateral 2, bilateral 1) in floow up. Asthenospermia was continued in 4 patients (unilateral 2, bilateral 2). Sperm count and morphology were recovered all the recover in group 1, 4 patients had persistent reduced sperm count and morphology in group 2, respectively. These observations suggest that systemic interferon-alpha2B treatment is highly effective in preventing infertility as well as testicular atrophy after mumps orchitis.

7.Resting Tremor during Low-dose Tricyclic Antidepressant Treatment: A case report.

Tricyclic antidepressant (TCA) is a useful drug for treating neuropathic pain. However, tremors are one of the relatively frequent side effects of TCA. A female patient, who was suffering from postherpetic neuralgia, was treated with amitriptyline starting with 10 mg/day. She developed resting tremors on the second day after increasing the dose to 30 mg/day. This case highlights the need for the careful use of amitriptyline in the treatment of neuropathic pain in elderly patients.

8.Questionnaire Study about Circumcision Performed in Young Men in Daejeon City and Chung-nam Area.

PURPOSE: We surveyed a population of 20-year-old Korean men and gathered epidemiologic data regarding circumcision. MATERIALS AND METHODS: Between May and December 2001, 2,700 men aged 20 years dwelling in the community were randomly selected at a 10.0% sampling fraction after a sampling process by census district. A total of 1,674 (a response rate 62.0%) men completed the self-administered questionnaire which consisted of demographic information, circumcision status, age at the time of circumcision, reasons that influenced the circumcision decision, and perspectives about circumcision. RESULTS: The incidence of circumcision was 78.0% and the peak period for circumcision was during their late elementary and middle-school years. When asked why they felt that they had been circumcised, most (89.1%) mentioned hygienic benefits. However, about more than half mentioned that they did not know whether circumcision had other medical benefits. Most men were circumcised on the recommendation of their parents or by their own will. CONCLUSIONS: Most Korean men are circumcised at puberty. There were no predominant medical reasons for circumcision in these men except hygienic benefits. Contrary to previous reports, the physician's role does not seem to be a leading contributory factor toward the unusual status of circumcision in South Korea.

9.Risk of connective-tissue disease in men with testicular or penile prostheses: a preliminary study.

AIMTo help clarifying the possibility of connective-tissue diseases in men with penile or testicular prostheses.
METHODSEight patients underwent inflatable penile prostheses and 15, testicular prostheses consented to the study. Their medical records were reviewed and a follow-up interview and physical and serological examinations were performed.
RESULTSIn patients with penile prostheses, there was no abnormal antinuclear antibody (ANA) or IgM elevation. The serum levels of the rheumatoid factor (RF), C4, IgA and IgG were abnormal in one patient, and the levels of erythrocyte sedimentation rate (ESR) and C3, abnormal in two. Four had elevated IgE. In patients with testicular prostheses, there was no abnormal RF, ANA or IgM. The serum levels of ESR and IgA were abnormal in two, and three had abnormal C4, ten abnormal C3, and eleven decreased IgG. All had increased IgE. Men with penile prostheses had higher serum levels of IgG and IgM than those with testicular prostheses (P=0.001, P=0.016, respectively). The rates of abnormal values of IgE and IgG were higher in men with testicular prostheses than in men with penile prostheses (P=0.008, P=0.009, respectively). Physical examination was normal in all patients and nobody had documented symptoms pertinent to connective-tissue diseases.
CONCLUSIONOur findings suggest that the risk of connective-tissue diseases is not higher in patients wearing prostheses as the ANA is negative and there is no apparent manifestation suggestive of connective-tissue diseases.

10.Influence of Age, Rate of Obesity, Hepatic Function and Renal Function on Serum Prostate-Specific Antigen in Men Older than 50 Years.

PURPOSE: We investigated the influence of age, rate of obesity, hepatic function and renal function on serum prostate-specific antigen in healthy men older than 50 years. MATERIALS AND METHODS: From January 1999 to December 2000, 2,296 healthy Korean men older than 50 years who visited Health Promotion Center at our hospital for a routine health checkup were entered into a prospective study. All men underwent detailed clinical examinations, including serum prostate-specific antigen (PSA) determination using Hybritech Tandem-R assay, anthropometric measurements, tests for liver disease, tests for renal function, and other constitutes. We determined the risk factors of the increase of serum PSA among these clinical parameters using univariate and multivariate logistic regression analyses. RESULTS: The univariate logistic regression analysis indicated that age, alanine aminotransferase (ALT), hemoglobin, potassium, albumin, high-density lipoprotein (HDL) were the risk factors of the increase of serum PSA. In the multivariate model used, age, ALT, alkaline phosphatase, albumin, HDL and cholesterol were an independent predictors of the increase of serum PSA, whereas hemoglobin and potassium lost their statistical significance. CONCLUSIONS: Our findings suggest that the health status as well as may influence on serum PSA level of healthy men older than 50 years. Additional work is needed to determine and validate the clinical significance of these findings.
