1.Oxygen Desaturation following the Use of Midazolam and Fentanyl during Spinal Anesthesia.

The purpose of this study was to find out the effeet of midazolam and fentanyl on oxygcn saturation by pulse oximetry in surgical patients under spinal anesthesia. The subjects of this study were 83 patients who were operated on under spinal anesthesia, at a general hospital in Pusan. from December 1st, 1997 to March 31st, 1998. The subjects of Group 1 were 53 patients who did not receive either of midazolam and fentanyl. The subjects of Group 2 were 21 patients who received midazolam. The subjects of Group 3 were 9 patients who received both midazolam and fentanyl. Oxygen saturation was measured with a pulse oximeter by time series. The collected data were analyzed by the SPSS / PC~ program and the results of the study were as follows 1. There were no significant differences among the three experimental groups in terms of age or cardiopulmonary disease. 2. Among the three groups, there were statistically significant differences in Sp02 at 5 minutes after starting the operation or after intravenous injection of drugs. The SpO2 mean for Group 3 was 92.4%, for Group 2 it was 97.7%. and for Group 1 it was 98.2%. 3. Hypoxemia cases occurred in 88.9% of Group 3, 28.6% of Group 2, and 17.0% of Group 1, respectively. It is concluded that oxygen saturation mo4oring should be done routinely to all patients under ~spinal anesthesia, especially during operating tim4. and patients receiving midazolam and fentanyl should be monitored closely. After monitoring oxygen should be administered to all patients who developed hypoxemia.

2.Relationships among Attitudes toward Menstruation, Peri-menstrual Symptoms, and Coping in Adolescent Girls.

BACKGROUND: This study aimed to identify the attitudes of adolescent girls towards menstruation, peri-menstrual symptoms, and coping. METHODS: A convenience sample of 521 adolescent girls was obtained from two middle schools and two high schools in M city. Data were collected through questionnaires about menstrual attitudes, menstrual symptoms, and coping. Data was analyzed by descriptive statistics, t-test, ANOVA, Pearson correlation coefficients, and multiple regression using SPSS/WIN 17.0 program. RESULTS: Subjects showed both negative and positive attitudes toward menstruation. There were significant correlation between attitudes toward menstruation and peri-menstrual symptoms. Subjects having a positive attitude toward menstruation needed a nursing mediator to provide relief for menstrual symptoms during the peri-menstrual period. Attitudes toward menstruation explained 11.0% variance of menstrual symptoms, and factor 4, in particular, (event whose onset can be predicted and anticipated) correlated strongest with menstrual symptoms and explained 21.5% variance of menstrual symptoms. CONCLUSIONS: Our results show that evidence-based strategies for positive attitudes toward menstruation, menstrual symptom, and coping should be included in a comprehensive approach to a nursing intervention to help adolescent girls.

4.Cryogenic Treatment of Herpes Simplex Keratits.

The results of 16 cases of herpes simplex keratitis treated by cryogenic method are presented. The cryoapplicators(2 mm and 4 mm in diameter of tip) were applied on the lesions 5-7 seconds and antibiotics and atropine solutions were instilled. No complication by cryogenic treatment was happened. The new vessels in the cornea were faded after cryocautery for the treatment of herpes simplex keratitis.

6.Gender Differences in Behavioral Characteristics and Their Awareness of Obese Children.

The purpose of this study was to investigate gender differences in behavioral characteristics and their awareness of obese school children. The results were as follows : 1. In comparison with eating habits and food preference, the girls had more irregular breakfast and were likely to have vegetables/fruits than the boys. 2. In characteristics related to exercise and activities, boys did more exercise than girls. Particularly, in lunch free time, boys participated in playing various active excercise with friends, but girls participated in more non-active behaviors(such as, chattering, reading, or playing jack-stones). 3. In comparison with their awareness related to obese persons, about a half of boys had positive opinions(e, grong, healthy.), but about only 20% of girls had positive opinions.

7.A Study on Influencing Factors in Health-promoting Lifestyle of Adolescents.

The purpose of this study were to define the degree of performance in health promoting lifestyle and to identify the variable related to performance inn health promoting lifestyle of adolescents. The subjects of this study were 469 adolescents in the 4 high schools. The sample data were collected using a purposive sampling method from July 1 to July 27. The collected data were analysed by using descriptive statistics, Pearson's correlation Coefficients, and stepwise multiple regression with SPSS PC+ program. The results of this research were as follows. First, The means of performance in the health promoting lifestyle of adolescents revealed total 2.478 ; harmonious relationships 3.045, regular diet 2.236, professional health management 1.322, sanitary life 2.910, self regulation 2.558, emotional support 2.696, healthy diet 2.048, rest and sleep 2.651, exercise and activity 2.491, self actualization 2.466, diet control 2.408. The factor with the highest degree of performance was the harmonious relationships, whereas the one with the lowest degrees was the professional health management. Second, the relationship between the degree of performance in health promoting lifestyle and its related variable were as follows. (1) Performance in the health promoting lifestyle was significantly correlated with self esteem, self efficacy, health conception, perceived health status, mother's health promoting lifestyle, mother's health conception. (2) The most important factor that affects performance in the health promoting lifestyle of adolescents was self esteem. The combination of self esteem, health conception, mother's health promoting lifestyle, self efficacy, perceived health status accounted for 45.2% of the variance in health promoting lifestyle of adolescents.

9.Benign Fibrous Histiocytoma of Spinal Cord.

Fibrous histiocytoma composed of fibroblasts and histiocytes is quite variable in histologic pattern. The biologic behavior is unpredictable by histologic ground. This tumor is well-known in subcutaneous tissue and deep soft tissue, but quite rare in central nervous system. We experienced a case of the fibrous histiocytoma involving the dura mater of spinal cord in a 26 year old female patient. In gross findings, the mass was a well demarcated, ovoid mass attached to the dura matter, and measured 2.5x1.5 cm in diameter. The cut surface was rubbery, solid, gray-white or yellow. Microscopically, the lesion consisted of polyhedral cells with round or oval nuclei and faintly eosinophilic or vaculoated cytoplasm, and multinucleated giant cells. Some giant cells were Touton-type. Composing cells were bland-looking. Mitotic figures were average 3 per 10 high power fields.

10.Three Cases of Orbital Rhabdomyosarcoma.

Three cases of orbital rhabomyosarcoma are reported here. Histological studies comfirmed the lesion 1:0 be the alveolar type in all cases. This tumor is a rare disease entity in Orient than Europe and America and accordingly, rarely encountered in our ophthalmologic practice, although it is the commonest primary malignant orbital tumor in children. Of the three cases reported here one occurred in adu1t and the other two in children. Case I. This 15 month old male showed a swelling of the left upper lid toward the medial side for 2 months and visited to our hospital on March 10, '69 with complaints of progressive swelling on the region with ptosis. The eye ball was slightly displaced toward the lower temporal side and ocular movement is limitted to upward. On palpation, the childthumb sized tumor was palpable in the upper nasal portion of the orbit. On excision of the tumor, the mass was found to be originated from the superior oblique muscle. The mass round measuring 2.5 X 2.5 X 1.5 cm in size. Histological diagnosis was alveolar rhabdomyosarcoma without cross-striation. No follow-up could be done. Case II. This patient of 15 month old male showed a swelling on the right lower lid existed 6 months and visited to our hospital on September 8, '69. The physical examination revealed that the left eye and the other routine examinations were within normal limits. The right eye showed the swelling of the lower lid, congestion of the palpebral conjunctiva, limitted movement of the eye ball toward the lower side and a palpable bean-sized mass on the region. The excision of tumor was performed under the general anesthesia. This mass was attached with the inferior oblique muscle with no other recognizable adhesion seemingly originating from that muscle. The mass was round and oval in shape, measuring 2 X 2 X 1 cm in size, diagnosis was alveolar rhabdomyosarcoma. The post-operative follow-up studies were unavailable. Case III. This case in one of adult alveolar rhabdomyosarcoma. At the age of 25 years, this man showed a proptosis of the right eye associated with migrain like headache, ocular pain and visual disturbance. This symptoms existed 6 months and gradually increased in intensity. On the physical examination, the left eye was found to be normal. The skull and orbit x-ray and c.b.c. were with in normal limits. The right eye revealed about 5mm proptosis compared with left eye and the adult thumb sized tumor was palpated on the inner side of the lower orbital rim. The visual acuity was in zero. The right optic disc was edematous and elevated about 4.0 D. The pupil showed marked dilatation and the light reflex was abscent and ocular movement was markedly limitted and eye ball is deviated toward the upper side. Under the general anesthesia, the exenteration of the orbit associated with the removal of tumor was carried out with no untoward complications. Origin of the mass was the inferior oblique muscle penetrating to the deeper part of the optic foraman and optic nerve was surrounded by the tumor completely Histology confirmed alveolar rhabdomyosarcoma. Follow-up study was impossible.
