2.A Case of Papillary Adenoma of the Nipple Showing Histopathologic Findings of Syringocystadenoma Papilliferum.

A 24-year-old woman was seen for evaluation of an skin lesion on thiright nipple. Examination revealed a solitary 1.5 x 1cm-sized, erythematous, oozing, eroded andruted, verrucous nodule on the right nipple. A small amount of serous discharge was found but there was no pain or tenderness. Histopathologic findings showed cystic invaginations extending down vard from the epidermis and papillary projections extending into the lumina of the invagination. Pupiae were lined by two rows of cells. In stroma, there were marked inflammatory infiltrates containing any plasma cells. The overlying dermis showed an adenomatous growth pattern with a prolifertion of round, oval or irregular shaped ducts and fibrotic stroma.

3.Two Cases of Pseudoepitheliomatous , Keratotic and Micaceous Balanitis.

Pseudoepitheliomatous, keratotic and micaceous balanitis is a rare distictive clinical entity that represents a histologic spectrum ranging from hypertrophic hyperpalstic penile dystrophy to verrucous carcinoma. This condition is thought to be a malignant growth potential by resistance to treatment and its tendency toward local recurrence. We report two cases with similar clinical presentation of hyperkeratotic plaque and micaceous scaly patches on the glans penis that were compatible with pseudoepitheliomatous, keratotic and micaceous balanitis. Histopathologically, case 1, 48 year-old male was progressed to squamous cell carcinoma and case 2, 78 year-old male, was shown pseudoepitheliomatous hyperplasia without malignant changes.

4.A Case of cytophagic Histiocytic Panniculitis.

We describe a case of typical cytophagic histiocytic panniculitis occuring in 10-year-old male. He had recurrent subcutaneous nodules, fever, pancytopenia and abnormal liver function tests since 5 months of his age, and recently showed hemorrhagic diathesis. The histologic picture showed lobular histiocytic panniculitis with "bean bag" cytophagic cells.

5.A Case of Factitial Panniculitis.

We report a case of factitial panniculitis in a 50-year-old female patient who had multiple variable sized subcutaneous nodules on area accessible to the hands and disappeared by only prohibition of habitual skin massage. Histologic findings showed mostly lobular panniculitis with hemosiderin deposits, many foamy histiocytes and pseudomicrocysts.

6.Dermographism ( IV ): The Prevalence in Atopic Dermatitis and Allergic Rhinitis.

The prevalences of dermographism in 106 patients with atopic dermatitis and 45 patients with allergic rhinitis were studied using a dermographic tester. The results are as follows : 1. Dermographism was more common in atopic dermatitis(17.0%) than in the general population(6.7%), but no significant difference was observed between allergic rhinitis(6.7%) and general population. 2. The prevalence rates of dermographism were not significantly correlated with the level of serum IgE in both atopic dermatitis and allergic rhinitis.

7.A Case of Epidural Abscess Complicated from Acute Mastoiditis Caused by Streptococcus pneumoniae.

Acute otitis media (AOM) is one of the most common childhood infectious diseases. Despite antibiotic treatment for AOM, AOM and its complication still continue to develop. Acute mastoiditis is a serious complication of AOM and epidural abscess constitutes the commonest of all intracranial complication of AOM. Neurological complication of acute mastoiditis are rare but can be life threatening. Their presentation may be masked by the use of antibiotics. We report the rare case of acute otitis media progressing to acute mastoiditis, epidural abscess formation and lateral sinus thrombophlebitis caused by Streptococcus pneumoniae in a child. She was admitted with acute otitis media with fever. Despite proper antibiotics, acute mastodititis and epidural abscess were developed, and after surgical drainage and antibiotics therapy she was recovered without sequalae.

8.Epinephrine-induced lactic acidosis in orthognathic surgery: a report of two cases.

Submucosal infiltration and the topical application of epinephrine as a vasoconstrictor produce excellent hemostasis during surgery. The hemodynamic effects of epinephrine have been documented in numerous studies. However, its metabolic effects (especially during surgery) have been seldom recognized clinically. We report two cases of significant metabolic effects (including lactic acidosis and hyperglycemia) as well as hemodynamic effects in healthy patients undergoing orthognathic surgery with general anesthesia. Epinephrine can induce glycolysis and pyruvate generation, which result in lactic acidosis, via β2-adrenergic receptors. Therefore, careful perioperative observation for changes in plasma lactate and glucose levels along with intensive monitoring of vital signs should be carried out when epinephrine is excessively used as a vasoconstrictor during surgery.

10.A Case of Treatment-Refractory Foreign Body Reaction after Autologous Hair Transplantation

Complications in autologous hair transplantation are rare, with minor complications that can follow any surgical intervention. A 37-year-old female presented with pruritic, solitary, 0.6-cm-sized, skin-colored nodule on the forehead at one month after autologous hair transplantation. Histopathological findings revealed diffuse dermal granulomatous reactions with multinucleated giant cells, histiocytes, and lymphocytes around the implanted hair follicles, suggestive of foreign body reaction. The patient was treated with 3 courses of 2.5 mg/mL triamcinolone intralesional injections at 4-week intervals. Despite treatment, the lesion remained, and a novel lesion appeared near the existing lesion.Total punch removal of the lesions was performed, and the lesions resolved without recurrence for 6 weeks. A foreign body reaction in hair transplantation surgery is thought to be a rare complication after implantation of the patient’s hair has replaced synthetic fibers. Herein, we report a case of foreign body reaction caused by hair transplantation treated with total removal of the lesion.
