1.Chinese herbal medicinal products in Europe:sales prospect and legal issues

In general EU legislation on pharmaceutical products for human use also applies to herbal medicines.In 2004 EU adopted a Directive to introduce a new pathway for marketing traditional herbal medicinal products,the "simplified registration" procedure.Subject to satisfying certain conditions,it allows the registration of traditional herbal medicinal products without requiring particulars and documents on tests and trials on safety and efficacy,provided that there is sufficient evidence of a minimum 30 years medicinal use,including at least 15 years in the EU.Accordingly pharmaceutical companies should to be able in theory to save considerable expenditure of time,effort and money when applying for marketing authorization.But in practice the new pathway doesn't work out as smoothly as expected.This article analyze the various pieces of European legislation that affect the development of both modern and traditional herbal medicine,the continued growth of their demand,EU's response to the latest market trend,and how Chinese herbal medicine companies should grasp the opportunities to penetrate into the European market.It also stresses the fact that publication of Chinese medicine scientific research articles in international medical journals will be the key to the sector's future success.

Herbal products,either in the form of drugs,cosmetics or health food are increasingly popular in Europe.There are wide variations throughout Europe with regard to the quality and safety control of these products.In recent years EU begins to adopt a new wave of legislation aiming at harmonizing the law.While small and medium herbal companies may feel very difficult to tackle the legal restriction problems newly imposed upon them,some large pharmaceutical companies with adequate capital and technical know-how are showing keen interest in investing in herbal products for supply to the European market.Strategic alliance,merger and acquisitions,and industry restructuring activities will frequently take place in the foreseeable future.This article argues that although facing fierce market competition Chinese herbal companies are well positioned to expand their business in Europe.Besides it makes an in-depth analysis of a set of important rules and regulations concerning borderline products,labeling,packaging,health and nutrition claims;all of which are important legal issues deserving special attention of Chinese herbal companies.
3.Comparison of effective outcome of application of Ruyi golden powder and magnesium sulfate in the treatment of phlebitis caused by amiodarrone

Objective To observe the effective outcome of application of Ruyi golden powder and magnesium sulfate in the treatment of phlebitis caused by amiodarrone.Methods 60 patients were randomly divided into the observation group and the control group (30 cases in each group).The external application of Ruyi golden powder which was prepared with 75% alcohol into a paste was used in the observation group and 50% magnesium sulphate was used in the control group.Results The total effective rate of two groups of patients was 93.3% in the observation group and 66.7% in the control group,the significant difference was found by the comparison of the two groups.Conclusions The external application of Ruyi golden powder with alcohol in the treatment of phlebitis caused by amiodarrone is better than magnesium sulfate.
4.Study of the effect of Shuanghongtong Decoction on diabetic neuropathy peripheral

Objective: To investigate the curative effects of Shuanghongtong Decoction (Flos carthami, Radix Ginseng Rubra and Radix Ophiopogonis) diabetic neuropathy peripheral (DPN) and its mechanism.Methods:28 DPN patients were treated with Shuanghongtong Decoction for 3 weeks. The symptom and the peroneal nerve conduction velocity (SNCV) were observed before and after the experiments respectively. The diabetic rats reduced by STZ were treated with the same decoction for 4 weeks, at the end of the experiments the nerve conduction velocity (NCV) and the activity of aldose reductase (AR) were measured.Results: Some symptoms such as acra pain, numbness, asthenia and the peroneal SNCV were significantly improved after treatment with decoction. The animal experiments showed that the NCV were significantly increased and the activity of AR significantly decreased in diabetic rats as compared with the normal rats.Conclusion: Shuanghongtong Decoction could improve the symptom and increase the NCV of DPN. It's mechanism may be through inhibiting the activity of AR then improving the diabetic metabolism disorder.
5.Effect of STAT3 gene silencing on epithelial-to-mesenchymal transition of human pancreatic cancer cells

Objective To investigate the effect and mechanism of RNAi-mediated STAT3 gene silencing on epithelial-to-mesenchymal transition (EMT) of human pancreatic cancer cells.Methods Lentivirus vector mediating RNA interference targeting STAT3 was constructed in SW1990 cell line.The invasion ability of SW1990 cells was determined by cell invasion assay in vitro.Cell proliferation and cell cycle of SW1990 cells were also detected.The expression of EMT related genes such as STAT3,P-STAT3,Twist,Snail and E-cadherin were analyzed by reverse transcription-PCR,real-time PCR,and Western blotting.Results Silencing of STAT3 with RNAi not only markedly reduced proliferation but also greatly decreased the invasion ability of SW1990 cells.The mRNA level and protein expression of Snail decreased significantly (P < 0.05),but those of E-cadherin increased significantly (P < 0.05),compared to parental cells.However,no difference was on the expression of Twist in SW1990 cell line.Conclusions STAT3 signaling pathway plays an important role in the process of EMT.Silencing of STAT3 with RNAi can significantly inhibit EMT by downregulating expression of Snail and E-cadherin in pancreatic cancer cells.
6.Neuroprotective effect of Dengzhan Shengmai capsule on ischemia/reperfusion injury in rats

Objective To investigate the neuroprotective effect of Dengzhan Shengmai capsule (DZSM) in rat cerebral ischemia-reperfusion injury and to explore the mechanism. Methods Rats were divided into Sham group, MCAO group, DZSM group, carbenoxolone (CBX) group and DZSM + CBX group. Each group was assessed for neurological function , infarct volume and the expression of Caspase-3 48 h after reperfusion. Connexin 43 (Cx43) expression of MCAO group was detected 3, 12, 24, 48 h after reperfusion. Results There were lower neurological deficit scores , infarct volume and the expression of Caspase-3 in DZSM , CBX and DZSM + CBX group 48 h after reperfusion when compared with those in MCAO group (P < 0.05) but Cx43 expression level in each group increased after reperfusion at each time point (P < 0.05). Expression of Cx43 was lower in DZSM, CBX and DZSM + CBX group than that in MCAO group (P < 0.05). Lower expression of Cx43 was also seen in CBX and DZSM + CBX group when compared with that in DZSM group (P < 0.05). Conclusion DZSM capsule can improve neurological function , reduce infarct volume and inhibit the expression of Caspase-3. The mechanism may be related to its inhibition of Cx43 expression.
7.Meta analysis on the tolerance and prognosis of nasogastric feeding versus nasojejunal feeding in severe acute pancreatitis

Objective To compare the safety and efficiency of nasogastric ( NG ) feeding with nasojejunal (NJ) feeding in treating severe acute pancreatitis ( SAP).Methods The terms NG tube,NJ tube,SAPand enteral nutritionwere used for literature search in PubMed , Embase, Cochrane Library, WanFang and CNKI databases and the publication deadline was June 1, 2016.Random effect model was used for Meta analysis .Results A total of 5 random clinical trials involving 264 patients ( 136 in NG group and 128 in NJ group ) were included .There was no statistical difference on the incidence of adverse events (mortality:RR=0.77, 95%CI 0.42~1.41, P=0.39; infection complications: RR=0.77, 95%CI 0.45~1.30, P=0.39; digestive complications: RR=1.26, 95%CI 0.73~2.16, P=0.41; stopping nasogastric proportion:RR=0.66, 95%CI 0.10~4.10, P=0.65;MODS rate:RR=0.98, 95%CI 0.71~1.35, P=0.90; the percentage of energy balance: RR=1.00, 95%CI 0.97~1.03, P=0.39 and the average length of hospital stay:RR=0.98, 95%CI 0.71~1.35, P=0.90).Conclusions NG feeding was safe and effective , which was comparable with NJ feeding .NG feeding was more convenient with a higher clinical value .
8.Efficiency and safety of simultaneous resection vs staged resection on in colorectal carcinoma with synchronous liver metastasis:a meta-analysis

Objective To compare the safety and efficiency between simultaneous resection and staged resection of synchronous colorectal liver metastases.Methods A systematic literature search was performed in PubMed,Embase,Cochrane Library,OVID,Web of Science,WanFang Data,CBM,VIP and CNKI databases for relevant articles published from January 2000 to July,2017.The meta-analysis was performed by RevMan 5.3.The primary outcomes were peri-operative complications,postoperative mortality,five-year survival rate.Results 25 studies with a total of 5 631 patients were finally included.The results of meta-analysis showed that simultaneous and staged resections were similar in peri-operative complications (OR =0.90,P =0.39),postoperative mortality (OR =1.23,P =0.29) and five-year survival rate (OR =1.03,P =0.68).Conclusions Simultaneous resection is safe and efficient in the treatment of patients with SCLMs.However,the results should be interpreted and applied prudently due to the lower level of evidence and the existence of heterogeneity.
9. Application value of intracranial vascular hemodynamics in neonatal subependymal hemorrhage

Objective:
To explore the related factors of subependymal hemorrhage (SEH) and cerebral hemodynamic changes.
Methods:
From October 2012 to October 2017, 200 cases of children with subependymal hemorrhage diagnosed by ultrasound in our department of pediatrics were selected as the observation group , and a total of 150 children who were admitted to the Department of Pediatrics in the same period due to craniocerebral diseases and other serious diseases were selected as control group. The independent risk factors of the children in the observation group were analyzed, and the difference of the maximum systolic blood flow velocity (SV), the diastolic maximum flow velocity (DV), the systolic and diastolic velocity ratio (S/D), the resistance index (RI), and the pulsatile index (PI) were compared between the two groups.
Results:
Neonatal asphyxia, preterm birth, acidosis, neonatal respiratory distress syndrome (NRDS), patent ductus arteriosus and coagulation dysfunction were independent risk factors for subependymal hemorrhage. 10.An investigation on parents’ knowledge of dental trauma in primary school of chancheng district, Foshan City

Objective:
To investigate the knowledge of pupils’ parents for the emergency treatment of dental trauma in Chancheng of Foshan.
Methods :
By the method of random sampling, a total of 320 parents from 5 primary schools of Chancheng District was selected for questionnaire survey on the treatment method of children’s dental trauma. Chi square test was used for statistical analysis
Results:
40.6% of the parents didn’t know that the anterior teeth of a nine-year-old child was permanent. 66.2% of parents didn’t know suitable methods to deal with the avulsed tooth. 72.2% parents didn’t selected the corrected preservation medium for the avulsed tooth. 95.6% of parents believed they needed to learn more knowledge of emergency treatment for dental trauma.
Conclusion
The present study showed that the knowledge of parents in Chancheng of Foshan about the emergency treatment of dental trauma is inadequate and related education is necessary.
