3.VALUE OF ENDOSCOPIC IN THE TREATMENT OF ACUTE SEVERE PANCREATITIS: CLINICAL ANALYSIS OF 26 CASES

Objective:To assess the value and indication of endoscopic intervention for acute severe pancreatitis(ASP).Methods:A series of consecutive patients with (26 cases) was treated mainly with endoscopic naso-biliary drainage(ENBD),sphincterotomy (EST) as well as drugs,Abdominal pain duration:raised serum amylase period surgical intervention rate mortality rate and average be in hospital number of days.Results:Abdominal pain duration was (2.5±1.5) days in treatment group with endoscopic and (5.1±2.4) days in control group respectively (P<0.05),raised serum amylase period was (2.8±1.3) days in treatment group with endoscopic and (4.8±1.8) days period in control group respectively(P<0.05),mortality rate was 3.85% (1/26) in treatment group with endoscopic while 16.67% (5/30),P<0.05,average be in hospital number of days was (17.2±5.2) days in treatment group with endoscopic and (26.2±18.2) days in control group respectively.Conclusion:ENBD+EST the intraductal pressure can be lowered and the cause of occlusion be relieved with therapeutic effect.It is considered that endoscopic intervention is a practic of choice for selected.
4.Effects of driving fatigue on working memory

Objective To investigate effects of driving fatigue on working memory. Methods The control group included 12 taxi drivers after adequate rest while the fatigue group included 27 taxi drivers who had been driving for 10h. Digit memory span, words span, Digit Subtraction Test, Random Number Generation test were used. Results Compared with the control(6.60 ±0.40,90.67 ±6.65,158.27 ±29.12,0.30 ±0.06,24.29 ±10.59,35.90 ± 10.64 ), driving fatigue group were significantly( P < 0.05 )different in the number of backward number memory span( 5.37 ± 0.72 ), percentage of number of right reaction of the number of total reaction of Digit Subtraction Test(79.95 ± 8.04), total number of RNG( 88.33 ± 19.48 ), RNG(0.40 ± 0.05 ), Coupon( 12.35 ±12.88), NSQ(49.72 ± 8.06). Conclusion Driving fatigue can decline the working memory.
5.Study on Determination of Total Isofraxidin in Ciwujia Tablets by HPLC

Objective: To establish the determination method of total isofraxidin in Ciwujia Tablets. Methods: In this method phenomenex C 18 column was used, acetonitrile -0.05mol?L -1 potassium dihydrogen phosphate solution (15∶85) was used as a mobile phase. The detection wavelength was at 344nm.Results: The recovery of the added sample was 102.8% and RSD was 2.2%. Conclusion: This method is simple and the result is reliable.
6.Treatment of intra articular fractures of distal radius

Objective To review the results of operative treatment of the distal radius fractures. Methods The group of 12 cases were classified accorded to universal classification. There was 1 case of type Ⅲ, 3 cases type ⅣA,2 cases type ⅣB, 5 cases type ⅣC and 1 case type ⅣD. Five of them were checked preoperatively by spiral CT and three dimensional evaluation. They were given respectively treatment by external fixation, by volar or dorsal buttress plate, with or without percutaneous K wires. Results The average period of follow up was 12 months. 9 cases were excellent and 2 cases were good. The excellent and good rate was 91.7%. Conclusions Operative interventions for intra articular fractures of distal aspect of the radius should be considered when non operation management failed. The spiral CT and three dimensional evaluation can reveal enough information about the fracture preoperatively. The proper operative method should be chosen according to the fracture type.
7.Clinical observation of GEM+NVB regimen in retreatment of 24 Advanced NSCLC treated by TAX+DDP regimen

Objective To appraise the efficacy,survival rate and toxicity of GEM+NVB regimen in retreatment of 24 Advanced NSCLC treated by TAX+DDP regimen.Methods 24 patients with Advanced NSCLC received chemotherapy of GEM+NVB regimen.We evaluate the efficacy,survival rate and toxicity after two periods.Results Among these 24 patients,there was no complete response.Rates of partial response is 29.2 %,the average survival period is 28 weeks.One year survival rate is 34.3 %.Conclusions GEM+NVB regimen is effective as second-line chemotherapy for the Advanced NSCLC patients that have ever been treaded by TAX+DDP regimen.
8.Caesalpinia sappan aqueous extract's growth inhibiting and apoptosis-inducing effects on human ovarian cancer cell

Objective To observe caesalpinia sappan aqueous extract's growth inhibiting and apoptosis inducing effects on human ovarian cancer cell line SKOV3. Methods Cell growth inhibiting effect was detected by cytometry; Examined were by trypan blue staining under a light microscope; Electronic microscopy was used to observe ultrastructure of SKOV3 cell affected by caesalpinia sappan aqueous extract's;With different concentration of caesalpinia sappan aqueous extract's acting on cells, cells cycle and apoptosis rate were analyzed by flow cytometry (FCM). Results The growth of SKOV3 was inhibited effectively by caesalpinia sappan aqueous extract's; typical morphology of apoptosis (cell shrinkage, chromatin condensation and apoptotic body formation) were observed and in a concentration-dependent manner. Flow cytometry detection showed: the experiment in vitro revealed growth of SKOV3 after treatment by caesalpinia sappan aqueous extract's at the different concentration. Apoptosis rate was increased with the increasing of the concentration of caesalpinia sappan aqueous extract's. Conclusion Caesalpinia sappan aqueous extract's can obviously inhibit growth of human ovarian cancer cell line SKOV3 and induce its apoptosis.
9.Three-dimensional finite element numerical analysis of the Ni Ti shape memory alloy clutching internal fixator

Objective Clutching internal fixtor(CIF)loose and the fixed part weakly heal up are often found in orthopedic clinic.In the present paper,biomechanics methods were used to try to explain and analyze these issues,providing a helpful suggestion for the application of CIF in clinic.Method Commerical finite element models(FEM)Program ANSYS was applied to set up the finite element models of orthopedic CIF and bone tissue to analyze and evaluate the biomechanical performances of the NiTi shape memory alloy CIF.Results There is an interaction force between embracing force of CIF and resistant force of bone tissue during the orthopedic clinical treatment.The embracing force along two semi-circular arms of CIF is increasing from the open position and reached the maximum value at the open symmetry position where the deformation of the bone occurred.Conclusion It is the key to choose the force loading position during the practical treatment,as the concentration force is the main force when there is an interactive force between the bone and the CIF.
10.Application of contact heat-evoked potentials in acute myelitis

Objective To study the characteristics of the central part of the nociceptive system in acute myelitis (AM) with contact heat evoked potentials (CHEPs) and to document the potentials in patients with AM.Methods Twenty patients with AM were recruited in this study as an experimental group,and twenty healthy subjects were chosen as a control group.A heat foil was used to elicit pain and CHEPs.Thermal stimuli were applied at 54.5 ℃ at five sites:the dorsum of the hand,the proximal volar surface of the forearm,the skin of the leg 5 cm proximal to the medial malleolus,and at the C7 and T12 acupuncture locations.The latency and waveform of the evoked potentials were recorded.The conduction velocity of the A8 fibers of the peripheral nerves and of the spinal part of the spinothalamic tract were analyzed.The somatosensory evoked potential (SEP) and sensory conduction velocity (SCV) of the limbs were also examined,and the results were compared with the CHEP results.The results were compared between the two groups.Results The N 550 latencies of the CHEP on the dorsum of the hand,the inside of the leg,and at C7 and T12 were prolonged significantly in the patients with AM compared to the healthy controls.There were no significant differences in the nerve conduction velocity of the Aδ fibers and the velocity or amplitude of sensory nerve conduction in the limbs between the groups.The conduction velocities of the spinothalamic tract were significantly reduced in the patients with AM compared to the control group,while the peak latencies of N13 and the interpeak latencies of N9-N13 and N13-N20 in the AM patients were significantly prolonged compared to the healthy persons.In the patients with AM,CHEP abnormality in the lower limbs (17/20,85%) was significantly higher than in the upper limbs,total CHEP abnormality and CHEP abnormality in the lower limbs were significantly greater than SEP abnormality.Conclusion Persons with AM have abnormalities in the central part of the nociceptive system.When used with MRI and other electrophysiological examinations,CHEP may contribute to diagnosing AM.It could be helpful in the differential diagnosis of AM from motor neuron diseases and peripheral nerve lesions.It is of great potential value in clinical practice.
