1.Simultaneous Bilateral Spontaneous Pneumothorax.

The simultaneous bilateral spontaneous pneumothorax is very dangerous, so the chest drain insertion should be done urgency. Contrary to the unilteral pneumothorax, the patient with simultaneous bilateral pneumothorax complains a severe respiratory distress, cyanosis and chest pain without the tension pneumothorax. Now, we present two cases of the simultaneous bilateral pneumothorax. The prompt insertion of chest drains relieved symptoms. One case is resolved by the bilateral thoracotomy, the other by the insertion of chest drains only.

2.Experimental Studies of the Fibrin-Adhesive-System in Osteochondral Fractures

A new fibrinogen system, consisting of highly concentrated fibrinogen and thrombin, is going to be applied in many clinical fields. Especially in injuries of the cartilagenous joint surface as chondral or osteochondral fractures, replantation of the fragments must be considered mandarory in order to preserve joint congruence and to prevent further destruction of the articular cartilage. The authors have an encouraging results by using this fibrin adhesive system (F.A.S.) in the experimental works.

Arthroscopy became a popular techique for the diagnosis and treatment of wrist disorder. But the specific indications for the wrist arthroscopy are not well defined. So we reviewed our experience of 16 wrist arthroscopies of radiocarpal and midcarpal joints. With this review, arthroscopy was of benifit in the diagnosis and treatment of the lesion of the triangular fibrocartilage complex and also in the synovectomy.

6.Unilateral Hydronephrosis Caused by Fibrous Band Around The Ureteropelvic Junction.

A 24-year old man presented with intermittent left flank pain for two years. With urograms this case was diagnosed as unilateral hydronephrosis associated with ureteropelvic obstruction. The cause of the ureteropelvic obstruction was proven as fibrous band surgically and pathologically. He underwent resection of the fibrous band and intubated pyelo-ureterostomy, and was discharged on the 14th postoperative day.

9.Coniparision of Clinical Results Between Keratomileusis In Situ and Laser Assisted In Situ Keratomileusis for High Myopia from -15 Diopter to -23 Diopter.

Photorefractive keratectomy (PRK), Keratomileusis In Situ (KM), Laser Assisted In Situ Keratomileusis (LASIK), and clear lens extraction have been performed for high myopic patients. Among the above surgical procedures KM and LASIK are applied to young patients in general. However, comparison of clinical results of these two procedures was not reported yet in Korea. To evaluate the clinical results of KM (n=16) and LASIK (n=11) for high myopic patients between -15 diopter (D) and -23 diopter (D), spherical equivalent (S.E) and uncorrected visual acuity were studied for more than ten months, retrospectively. The mean S.E. was changed from -17.36 D before operation to -3.70 D ten months after operation in KM, whereas it was from -17.41 D to -0.31 D in LASIK. At ten months after operation, uncorrected visual acuity equal or better than best corrected visual acuity before operation was 43. 8% in KM and 63. 6% in LASIK. This results show that the clinical result of LASIK were superior to that of KM for high myopia.
