Twenty newly admitted acute schizophrenic patients were treated with haloperidol for 6 weeks. HVA and 5-HIAA were sampled at baseline, 3days after initial neuroleptic dose, and after 1, 2, 3, 4, 6 weeks of treatment. Nine patients were classified as responders in this prospective haloperidol treatment trial. They had a score of change in the BPRS total scores of 25% or greater. Eleven patients were classified as nonresponders, based on a score of changes in the BPRS total scores of less than 25%. 1) There was no significant difference in plasma HVA/5-HIAA ratio between responder and non-responder before and after haloperidol treatment. 2) There was no significant correlations between plasma HVA/5-HIAA ratio and BPRS total scores. This study could not support the hypothesis that neuroleptic treatment would be effective by changing dopamine and serotonin function and/or by altering their interaction.

Conventional neuroleptic treatment of organic delusional disorder can induce many serious side effects including extrapyramidal symptoms, sedation and tardive dyskinesia, especially in elderly patients. Risperidone is an atypical neuroleptics that is lack of severe extrapyramidal symptoms. This 68-year-old male case demonstrated that elderly patients with organic delusional disorder could be treated with risperidone without serious side effects.

This report describes the efficacy of combined use of aripiprazole in the treatment of a patient with clozapine induced enuresis. Aripiprazole acts as a potential dopamine partial agonist and the dopamine blockade in the basal ganglia might be one of the causes of urinary incontinence and enuresis. We speculate that aripiprazole functioned as a D2 agonist in hypodopaminergic state of basal ganglia caused by clozapine and maintained dopamine level that would improve enuresis ultimately.

To assess the efficacy and safety of combining electroconvulsive therapy(ECT) and clozapine in patients with treatment-resistant schizophrenia, the authors reviewed use of this combination in one treatment-resistant schizophrenic inpatient and to explain the mechanism of this combination, we measured the blood level of clozapine and its metabolite with high performance liquid chromatography(HPLC) before, during and after this combination. The combination of clozapine and bilateral ECT was mildly effective and the authors saw no adverse effects during combined treatment. The blood levels of clozapine and its metabolite did not significant change before, during and afyer this combination. Combining ECT and clozapine appears to be safe and is effective without significant change of the blood levels of clozapine and its metabolites in some patients with treatment-resistant schizophrenia.
