PURPOSE: For the synthesis of surfactant protein(SP) peptides and production of next generation artificial pulmonary surfactant(PS), we have isolated SP-B, C from bovine PS, and studied the biochemical properties and amino acid sequences of these protein-peptides. METHODS: Crude surfactant and purified surfactant were isolated from materials extracted from the bovine lung lavage. The hydrophobic SP-B, C were purified by Sephadex LH 60 column chromatography from PS. The purities of SP-B, C were assessed by tricine buffer SDS-polyacrylamide gel electrophoresis and the amino acid sequences of these proteins were determined using Beckman PI-2090. RESULTS: The molecular weights of SP-B, C shown in SDS-polyacrylamide gel electrophoresis were as follows; 15,000-18,000(oligomer) Da for SP-B, 3,500-5,000 Da for SP-C. The amino acid sequences were; FPIPLPYCWL LRTLIKKIQA VIPKGVLAMT VAQCHVVPL LVGGIQQLV IEYSVILLTD TLLGRLPNLV CGLRLRCSG in SP-B, LIPCCPVNIK RLLIVVVVVV LLVVVIVGAL LMGL in SP-C, respectively. These results indicated that the amino acid sequences of bovine SPs were different. CONCLUSION: The SP-B, C were purified from bovine PS, and amino acid sequences of SP-B, C were determined. Further studies are needed for the development and use of next generations of exogenous PS preparation based on synthetic SP-peptides for the treatment of neonatal RDS in the future.

We had previously reported the purification of bovine lung natural surfactant-phospholoipd (PNSL)and the assessment of it's surface physical properties in vitro. To observe the clinical effectiveness of PNS-L in vivo, the degree of improvement of thoracic pressure-volume, compliance and histological changes following PNS-L instilation in premature rabbits with respiratory distress syndrome(RDS)were investigated. Rabbits, delivered prematurely by cesarean section at 27 days, treated with PNS-L via tracheostomy, were enrolled the study group. Two control groups were premature RDS rabbits (no treatment with PNS-L)and mature rabbits. We compared the results of thoracic pressure-volume changes during deflation phase and lung compliance changes among PNS-L treated study group and two control groups, and compared the results of aefated lung area ratio (%) on histologic samples among PNS-L treated study group and two control groups by the methods of IBSA-2000. There were significant improvements of thoracic pressure-volume during duflation phase(p<0.001), lung compliance (p<0.01, p<0.005, p<0.001) and increased acrated area histologically (p<0.005)in PNS-L treated study group compared with premature control group. It was suspected that PNS-L had contained the effective survace physical properties as a function of pulmonary surfactant. And improvement of pulmonary ventilatory functions and histological characteristics, were observed in PNS-L treated RDS model in vivo study.

Total 36 reports on the mortality rates (MRs) of low birth weight infants (LBWI) and very LBWI (VLBWI) in Korea from the 1967 through 2001 were analyzed. We compared the changes in the MR by 5 and 10-yr interval. The MRs observed by 5-yr intervals from the early 1960s through the 1990s have drastically decreased. TheMRs of LBWI are as follows: 23.1% and 23.6% in the 1960s, 17.3% and 16.8% in the 1970s, 14.1% and 14.4% in the 1980s, and 8.1% in the early 1990s. The MRsof VLBWI have also fallen and were reported as follows: 68.2% and 63.7% in the 1960s, 55.8% and 57.6% in the 1970s, 56.2% and 48.1% in the 1980s, 33.5% and 24.5% in the 1990s, and 11.7% in the early 2000s. In every 10-yr period, the MRs of LBWI have decreased from 23.4% in 1960, to 17.0% in 1970, to 14.2% in 1980, and to 8.1% in 1990. The MRs of VLBWI also have decreased from 66.2% in 1960, to 56.7% in 1970, to 50.8% in 1980, to 32.9% in 1990, and to 11.7% in 2000. TheMR of LBWI and VLBWI has gone down remarkably due to improvements in neonatology in Korea as shown above.
