ObjectiveTo study the clinical and pathological characteristics of tubulointerstitial nephropathy associated with Chinese herb patent Guan Xin Su He Wan(GXSHW-TIN).Methods15 patients with GXSHW-TIN were studied.Clinical and pathological data were semi-quantitatively assessed.Relationships between medication and the incidence,clinical characteristics and the outcome of the disease were analyzed.ResultsAll the patients had chronic renal failure when GXSHW-TIN was diagnosed.They all got the disease after long-term taking of GXSHW in routine dosage.Most of the patients presented gastrointestinal dysfunction,abnormal urine analysis and mild to moderate anemia as onset symptoms.The pathological characteristics were similar to those of Guanmutong(Aristolochia manshuriensis Kom) induced chronic aristolochic acid nephropathy(AAN).ConclusionLong-term taking of GXSHW,which contains Radix Aristolochiae,might induce AAN.It indicates that GXSHW should be causious for clinical use,the ban of Radix Aristolochiae in the pharmaceutical market needs to be considered for prevention of AAN.

Objective To explore the influence of maternal free position combined with duole tools during the first labor stage on birth outcomes.Methods 600 normal pregnant women who were routinely examined in our hospital from May 2016 to December 2016 were selected.They were randomly divided into observation group and control group according to the number of prenatal date of pregnant women,each group in 300 cases.The control group was taken routine supine position in the first labor stage.The observation group used free position with duole tools during the first labor stage.The maternal first labor time,delivery mode,neonatal asphyxia,postpartum hemorrhage,maternal reproductive tract injury were observed.The maternal postpartum within 2 hours,the pain score,maternal childbirth response scale (CCB) and anxiety by comprehensive hospital anxiety and depression table (HAD) were assessed.Results The spontaneous delivery rate and tolerable pain of the observation group were 90.0％(270/300) and 73.7％ (221/300) respectively,which were higher than those of the control group [83.3％(250/300) and 62.0％ (186/300)].The maternal perineum II degree laceration and neonatal asphyxia rates of the observation group were 8.3％(25/300) and 1.7％ (5/300) respectively,which were lower than thsoe of the control group [12.3％(37/300) and 4.3％ (13/300) respectively],the differences were statistically significant(χ2=9.31,9.84,9.05,8.39,all P＜0.05).The maternal first labor time and postpartum hemorrhage of the observation group were (10.87±3.06)h and (121.60±15.11) mL,respectively,which were significantly lower than those of the control group [(11.69±2.48)h and (180.52±14.76)mL],the differences were statistically significant (t=8.92,9.74,all P＜0.05).The HAD score of the observation group was (6.04±1.95)pionts,which was lower than (8.76±1.21)points of the control group.CCB score of the observation group was (49.82±7.61)points,which was higher than (45.33±6.90)points of the control group,the differences were statistically significant (t=10.15,11.32,all P＜0.05).ConclusionTaking the free position with duole tools during maternal first labor stage can improve the natural delivery rate,tolerate pain and postpartum response ability significantly,the first labor stage significantly shortened,postpartum adverse reactions and psychological anxiety decreased.

OBJECTIVE:To study in vitro anticancer activity of conjugated linoleic acid-gemcitabine conjugate (CLA-GEM). METHODS:IC50 of different tumor cells (breast cancer MCF-7 cell,breast cancer MDA-MB-231 cell,lung cancer A549 cell, small cell lung cancer NCI-H446 cell,glioma C6 cell)were investigated after treated with different concentrations(0.001-100μmol/L)of CLA-GEM and gemcitabine(GEM)for 72 h;survival rates of MCF-7 cell were investigated after treated with above so-lution for 24,48 and 72 h. The dependence of 0.001-100 μmol/L CLA-GEM and GEM to nucleoside transporter was investigated (by IC50)through MCF-7 cells and MDA-MB-231 cells treated with nucleoside transporter inhibitors(NBMPR,100 μmol/L)and di-pyridamole(4 μg/ml). The change of MCF-7 cell cycle was investigated after treated with 1 μmol/L CLA-GEM and GEM for 24 h. RESULTS:Compared with GEM,IC50 of MCF-7,MDA-MB-231 and NCI-H446 cells became lower after treated with CLA- GEM (P<0.01),there were no statistical significances in IC50 between A549 and C6 cells(P>0.05). Survival rate of MCF-7 cells de-creased significantly after treated with GEM for 48 h and CLA-GEM for 24 h. Survival rate of MCF-7 cells was the lowest,being 21% after treated with GEM for 72 h,while tumor cells were sacrificed by CLA-GEM completely. Compared with GEM or CLA-GEM,IC50 of MCF-7 and MDA-MB-231 cells increased significantly after treated with NBMPR,dipyridamole combined with GEM (P<0.01);there were no statistical significance in IC50 after treated with NBMPR,dipyridamole combined with CLA-GEM (P>0.05). Compared with GEM,CLA-GEM could prolong 6% of S stage of MCF-7 cells (P<0.01). CONCLU-SIONS:Compared with GEM,CLA-GEM exhibits significant antitumor activity and rapid action,and it isn’t influenced by nucle-ic acid transportation.

Objective To observe the effects of intravenous injection of fentanyl and tramadol on postoperative analgesia in laparoscopic cholecystectomy of clinical efficacy.Methods 80 cases treated with laparoscopic cholecystectomy were randomly divided into the fentanyl group (group F) and tramadol group (group Q).VAS score,adverse reaction incidence and patient satisfaction,were observed after the administration of 0.5,1,2,4,8,12h.Results In group F after administration of 0.5,1,2h,VAS score was better than that of group Q; group F after administration of the drug 4,8 h VAS score was inferior to group Q;two groups of patients in postoperative 12h VAS scores showed no significant difference in group F;the overall incidence of adverse reactions was higher than that of group Q,patients' satisfaction was higher than that of group Q.Conclusion Intravenous fentanyl in laparoscopic cholecystectomy operation postoperative overall are superior to the analgesic effect of tramadol,with high patient's satisfaction.

Objective:To prepare fenofibrate PEG2000-DSPE micelles in order to improve the solubility of fenofibrate, and study the oral pharmacokinetics of the micelles in SD rats. Methods:Fenofibrate PEG2000-DSPE micelles were prepared and characterized. The rats were administrated with fenofibrate PEG2000-DSPE micelles and fenofibrate suspension, respectively. The blood samples were collected from eye socket and determined by HPLC. The compartmental pharmacokinetics was analyzed by DAS software. Results:Fenofibrate PEG2000-DSPE micelles were prepared successfully. The mean particle size was (23. 40 ± 3. 62) nm, the drug loading and the entrapment efficiency was (97. 65 ± 3. 32) % and (1. 33 ± 0. 32) %, respectively. The mean plasma concentration-time curves of fenofibric acid were both in accordance with two-compartment mode after oral administration of fenofibrate PEG2000-DSPE micelles and fenofibrate suspension in rats. After the oral administration, AUC(0-24) and Cmax of fenofibrate PEG2000-DSPE micelles was respectively 7-fold and 14-fold higher than that of fenofibrate suspension [(61. 41 ± 5. 71)μg·h·ml-1 vs (8. 49 ± 0. 66)μg·h·ml-1, and (9.67±1.65) μg·ml-1 vs (0.71 ±0.09) μg·ml-1]. The relative bioavailability of fenofibrate PEG2000-DSPE micelles was 723. 3%. Conclusion:The bioavailability and absorption rate of fenofibrate are both increased by the micelles remarkably when com-pared with those of fenofibrate suspension after oral administration. The PEG2000-DSPE micelles served as drug carrier for oral delivery present promising application perspectives.

BACKGROUND: The association between the ingredients of perfusate and its protection on corneal endothelium is always the hot issue in ophthalmology and pharmacology.OBJECTIVE: To observe the influence of different perfusates on the structure and function of rabbit corneal endothelium during phacoemulsification.DESIGN: A randomized grouping designed and controlled animal trial.SETTINGS: Laboratory of experimental animal center of Jinzhou Medical College and the laboratory of experimental animal operation of an urban hospital.MATERIALS: The experiments were carried out in the laboratory of experimental animal center of Jinzhou Medical College and the laboratory of experimental animal operation of Jinzhou Yadong Ophthalmology Hospital from September 2004 to March 2005. Sixteen pure Japanese big-ear rabbits of 3.5 months old, clean degree, were randomly divided into four groups with 4 rabbits in each group: normal control group, saline group, shike group and balanced salt solution group. Shike was produced by Shenyang Qixing Pharmaceutical Co.,Ltd.; Balanced salt solution by Alcon Company (USA).METHODS: The rabbits were intraperitoneally anesthetized with 100 ml/L chloral hydrate (3 mL/kg), 4% oxybuprocaine hydrochloride eye drops were used for surface anesthesia of eyes, and both eyes were operated. Alcon phacoemulsification apparatus (USA) and routine microsurgical instruments were used. A 3.5-mm incision was made on sclerotic tunnel at 2 mm posterior to superior limbus of sclera, punctured into the anterior chamber, then 0.25 mL Viscoat (Alcon) was infused. Curvilinear capsulorhexis was performed with the diameter of about 5 mm. The phacoemulsification head was placed in the center to suck out the crystal nucleus and cortex, and the incision was closed after the operation. The morphology of the corneal endothelium was quantitatively determined using contact specular microscope preoperativley and 6 hours postoperatively, including the density and area of corneal endothelium,percent of hexagonal cells and the coefficient of variation. At 6 hours postoperatively, trypan blue-alizarin red active staining was performed, and the changes of slight structures of corneal endothelium were observed under light microscope.MATN OUTCOME MEASURES: ① Quantitative analysis of the forms morphology of corneal endothelium (density and area of corneal endothelium, percent of hexagonal cells and the coefficient of variation); ② Characters of forms and structures of corneal endothelium.RESULTS: All the 16 rabbits were involved in the analysis of results. ① There were no significant differences in the morphological indexes among the four groups preoperatively. ② At 6 hours postoperatively, density and areas of endothelial cells, percent of hexagonal cells and the coefficient of variation were significantly lower in the saline group,shike group and balanced salt solution group than in the normal control group (P ＜ 0.05), and no significant difference was observed between the shike group and balanced salt solution group (P ＞ 0.05). ③ The structural changes of corneal endothelium in the shike group and in balanced salt solution group were alleviated more significantly than those in the saline control group, no necrosis was observed.CONCLUSTON: In phacoemulsification, the damage of perfusate to corneal endothelium is a chemical one. Under the same surgerical conditions, domestic perfusate of shike is as effective as balanced salt solution in protecting endothelial cells.

Aim To investigate the expression of Notch pathway and Nephrin in angiotensin Ⅱ ( AngⅡ)-stimulated mice podocyte. Methods Mice podo-cyte was stimulated by AngⅡ, and then was treated with valsartan. The levels of Notch1, Notch intracellu-lar domain 1 ( NICD1 ) , Hes1 and Nephrin were deter-mined by immunofluorescence, Western blot and Real-time PCR. Results AngⅡincreased Notch1, NICD1 and Hes1 expression, and decreased Nephrin expres-sion in a time-dependent manner ( P<0. 01 ) . Valsar-tan inhibited AngⅡ-induced activation of Notch path-way and enhanced Nephrin level ( P <0. 01 ) . Con-clusion AngⅡdecreases Nephrin expression in podo-cyte by activating Notch pathway.

Objective To compare the surgical outcomes between laparoscopic and open surgeries in treatment of ectopic oviduct pregnancy, and investigate the clinical value of laparoscopic surgery. Methods Two hundred and forty-six patients with ectopic oviduct pregnancy in our hospital from June 2009 to March 2013 were retrospectively analyzed. 134 cases were treated with laparoscopic operation and 112 with open surgery. Some parameters such as operative time, blood loss, usage of pain-killer, hospital stay were compared between both groups. Results All operations were successful. Laparoscopic surgery was shown to be superior to open operation in the parameters of operative time, blood loss, usage of pain-killer and hospital stay ( < 0.05) . Conclusion Compared with open operation, laparoscopic surgery has the advantages of less trauma and rapid postoperative rehabilitation. It may become the first line treatment for ectopic oviduct pregnancy.

Objective To investigate the features and diagnostic value of computed tomography ( CT) in cys-titis glandularis .Methods The CT images and clinical data of 9 patients with cystitis glandularis confirmed by histol-ogy were studied retrospectively .Results The characteristics of cystitis glandularis ( location,shape,density and the relationship with adjacent tissues)were clearly displayed on CT.The lesion was located in the bladder trigone(n=5), right anterior wall(n=1),left posterior wall(n=2),diffuse lesion(n=1).According to the shape,8 lesions showed mass type,the range of these lesions were limited and some lesions were combined with cystic degeneration ,only one lesion showed a diffusely thickened bladder wall .After enhancement ,CT value increased from 14.0-45.0 HU to 25.0-62.0 HU.Conclusion CT has high sensitivity and accurate in diagnosis of cystitis glandularis .

An enrichment culture showing specific algae-lysing activity was isolated from the mixtures of different samples and Microcystis aeruginosa. The process of algal lysis was monitored by chlorophyll measurement, PCR, and denaturing gradient gel electrophoresis (DGGE). The result showed that the enrichment culture had still high algicidal activity against M. aeruginosa after 1/100000 dilution. Rubritepida sp. C1, Pseudomonas sp. C2 and Sphingomonas sp. C3, as accompanying bacteria, existed in M.aeruginosa. The bacterial community in M. aeruginosa showed significant change after adding the enrichment culture, where uncultured Flavorbacterium sp. A2, Sphingomonas sp. C3 and Hydrogenophaga sp. A3 were observed, and A2 became a dominant species. The obvious correlation can be seen between change of bacterial population and extinction of M. aeruginosa. Compared identification of pure bacterium with sequencing of DGGE band, it was inferred that uncultured bacteria were probably play an important role in controlling the growth and abundance of M. aeruginosa.
