1.Surgical therapy of senile gastric cancer complicated with portal hypertension

Objective To investigate the surgical treatment method for senile gastric cancer complicated with portal hypertension. Methods Retrospective analysis was used for 88 elderly patients who suffered from gastric cancer complicated with portal hypertension.Among the cases,35 cases were treated with standard radical resection,38 cases treated with cytoreductive surgery,9 cases with palliative operation,and 6 cases cured with jejunal fistula.Operation complications,death and mortality,the risk factors of postoperative complications were observed. Results For all the patients,the operation time was (170±33) min,and the intraoperative bleeding was (490±100) ml.In 35 patients (39.8%) occurred operation complications,among which the incidence of incision infection and anastomotic leakage was the highest, which accounted for 14.3% and 11.4%,respectively.In 5 years,69 patients were in death,the mortality rate was 78.4% which most resulted from liver failure (21.7%).Logistic regression analysis showed that age,tumor diameter,operation time and perioperation bleeding were the risk factors for death ( 95% CI:6.92-13.33,7.39-18.26,11.34 24.85,8.69-19.27). Conclusions The surgical approach for elderly gastric cancer patients complicated with portal hypertension should be chosen carefully,otherwise it will bring serious operation complications,and even endanger the lives of patients.

3.Efficacy research of self-made traditional Chinese medicine for hand-foot-mouth disease

Objective To explore clinical effect of self-made traditional Chinese medicine for hand-foot-mouth disease.Methods 118 cases with hand-foot-mouth disease were randomly divided into two groups.The control group(n=59)were treated by regular western medicines.On that basis,the study group(n=59)were treated bys elf-made traditional Chinese medicine.And the curative effects of two groups were observed.Results Fever subsidise time of the study group after treatment was(2.0±0.7)d,which was significantly shorter than that of the control group,with P<0.05.Rash subsidise time of the study group after treatment was(3.9±0.9)d,was significantly shorter than that of the control group,with P<0.05.Length of hospital stay of the study group after treatment was(3.5±1.3)d,was significantly shorter than that of the control group,with P<0.05.Total effective rate of the study group after treatment was 94.9%,which was significantly higher than that of the control group(81.4%),with P<0.05.Conclusion Self-made traditional Chinese medicine for hand-foot-mouth disease has good clinical curative effect,has high security and reliability.
4.The study of a differential induced FOXP3 expression by transforming growth factor-β1 between CD4+and CD8+ T lymphocytes

Objective To study the different induced expressions of FOXP3 by transforming growth factor-β1 (TGF-β1) between CD4+ and CD8+ T lymphocytes.Methods After the cultures of CD4+ cells and or CD8 + cells under an anti-TCR stimulation condition with an addition of TGF [β1 or not for 4 days respectively,the induced FOXP3 expressions were detected through the flow cytometric intracellular staining.Meanwhile we also examined the proliferative activity of TGF β1 induced FOXP3 expressing lymphocytes through a CFSE labeling experiment and the effect of TGF-β1 on cell apoptosis by annexin V staining.Results TGF-β1 selectively induced the CD4+ T lymphocytes to express FOXP3 (PBMC:29.66±3.624 vs 7.430±0.643; NIL:31.74±2.612 vs 8.637±1.146); The induced cells also had a proliferative activity (94.39 ± 1.179) and could secrete IFN-γ efficiently after activation (39.58±1.611); TGF-β1 could reduce the apoptosis rate of CD8+T lymphocytes(25.39±2.158 vs 9.320±0.3219).Conclusions Consistent with the discovery that >95% of the FOXP3+lymphocytes in TIL (tumor infiltrating lymphocytes) of HCC (human hepatocelluar carcinoma) patients were concentrated in CD4+ T lymphocytes (97.15±0.3807,n=10),TGF-β1 induced the CD4 +T lymphocytes to express FOXP3 in preference to CD8+T lymphocytes; But with different to natural Tregs,the induced cells still could proliferate and secrete IFN-γactively after an effective stimulation;CD8+ T lymphocytes were more easily suffering cell apoptosis after activation than CD4+ T lymphocytes,and TGF-β1 could rescue the more cell apoptosis.
5.Pro. Xu Xiaofeng’s Experience in Treating Primary Amenorrhea Combined with Infertility with Combination of TCM and Western Medicine (WM)

[Objective]Exploring from the specific case analysis of Chinese and western medicine treatment of primary amenorrhea with the clinical curative effect of infertility. [Method]In2013 January by Professor Xu Xiaofeng, doctor diagnosed primary amenorrhea with infertility and 1 patient of typicalcase, combined traditional Chinese medicine(TCM) and western medicine artificial menstrual cycle for 3 months, 3 months after the Chinese medicine Tiaozhou method combined with low dose of HMG with HCG inovulation induction treatment. [Result]After the treatment, the patients had pregnancy 70D, blood hormone and B Ultrasound examination were normal. [Conclusion]Combined treatment of TCM and western medicine is better than pure Chinese medicine or western medicine, suggesting that combination of TCM and western medicine is effective for the treatment of primary amenorrhea with infer-tility.
6.Diagnosis and therapy status of mesenteric venous thrombosis

Mesenteric venous thrombosis (MVT) is a kind of rare intestinal anemic disease.Because the clinical manifestations of MVT do not have high specificity,early diagnosis is difficult.In recent years,with huge progress of medical imaging technology,particularly the clinical application of spiral CT and MRI,the early diagnosis of MVT became possible.With the development of interventional therapies,especially the maturity of the interventional treatment technology such as mesenteric artery perfusion with papaverine,thrombectomy and thrombolysis via percutaneous transhepatic portal vein and so on,MVT mortality rate reduced greatly.The diagnosis and therapy status of MVT were reviewed in this article.
7.Determination of telomerase activity in exfoliated cells in urine for the diagnosis of bladder carcinoma

Objective In order to find a more sensitive and specific noninvasive assay for the diagnosis of bladder carcinoma, the authors tested the exfoliated cells from patient′s voided urine for the presence of telomerase activity and evaluated its clinical significance.Methods PCR-ELISA assay was used to determine the presence of telomerase activity in voided urine samples from patients with bladder carcinoma, fhematuria of benign causes and from normal, healthy volunteers.Results Telomerase activity was detected in 14 of 18 tumor samples, while none of the hematuria samples and normal, healthy volunteers′ samples. Conclusions Telomerase activity could be detected in most urine samples of bladder carcinoma patients and it is more sensitive and specific than other assays, therefore, would be a good diagnostic marker.
8.Clinical application of subcutaneous nephrovesical bypass system

Objective:To evaluated the subcutaneous nephrovesical bypass carried out in patients with ureter obstruction and improve the quality of life of patients with palliative definitive percutaneous nephrostomy.Methods:A total of 36 patients with advanced cancer receiving 45 subcutaneous nephrovesical bypass tubes were included in the study.Uronephrosis ranged from 1.8-5.2 cm,serum creatinine 73-760 ?mol/L,and glomerular filtration rate(GFR) 19-26 mL/min.The obstruction was proved to be complete by retrograde pyelography.The nephrovesical stents were inserted utilizing percutaneous access to both of the kidney and bladder,and a subcutaneous tunnel was created between the two sites.B-ultrasound,KUB(kidneys,ureters,and bladder),serum creatinine test and GFR were carried out 72 hours after operation.Results:Urinary diversion operations were successful in the 36 patients,uronephrosis ranged from 0-1.8 cm,all stents were in position,serum creatinine 72-173 ?mol/L,GFR 25-40 mL/min after the operation,and the bypass functioned well in 45 stents for 67 to 481 days of the follow-up.Conclusion:Subcutaneous nephrovesical bypass can protect renal function of patients suffering from ureteral obstruction by simple and minimum invasive operation;it can also reduce complications caused by percutaneous nephrostomy and improve the patient's life quality.
9.Development of advanced gastric cancer treated with weekly administration of paclitaxel

Benefits in survival and quality of life have been demonstrated in patients with advanced gastric cancer receiving chemotherapy plus optimal supportive care versus optimal supportive care alone.The taxanes, which are among the most promising cytotoxic agents in clinical use have considerable activity in a number of tumor types.Paclitaxel,especially low-dose weekly administration,has shown encouraging activity in the treatment of advanced gastric cancer as a single agent.Patients treated with the low-dose weekly paclitaxel regimen had a statistically superior response rate and time to disease progression as well as clinically significant prolongation of survival.And it offers a new option for those advanced gastric patients such as the aged,poor performance status(PS) or unsuitable for combination chemotherapy,and those even with no indications for rountine chemotherapy.This review is focused on the clinical reports of low-dose weekly administration of paclitaxel for patients with advanced gastric cancer.
10.Study on histopathology of the mouse forestomach neoplasm inducd by B(a)P

Purpose:To explore the development change of histopathology of the mouse forestomach neoplasm inducd by B(a)P.Methods:A part of the mice were killed each four weeks after they were given benzo(a)pyrene (concentration 5mg/ml) by lavage for four weeks, twice per week. The histopathology of the forestomach was then observed.Results:Forestomach neoplasm was noticed from the 17~(th) week in the mice. Local gland hyperplasia, arrangement disorder gradually developing into glandular abnormality hyperplasia and then transformed into early stomach cancer and finally into aggressive stomach cancer could be observed.Conclusions:Latent period of the mouse forestomach neoplasm induced by B(a)P is about three months. The development course is atrophic gastritis, atypical hyperplasia and stomach cancer.[
