2.Research progress of lower limb muscle strength training in the treatment of lliotibial band syndrome.

Iliotibial band syndrome (ITBS), as an overused injury of the lower extremities, has developed into a common cause of lateral knee pain. At present, the treatment of ITBS includes drug therapy, muscle strength training, physical therapy, and surgical treatment. Among these methods, physical therapy, drug therapy, and surgical treatment can only alleviate the symptoms of patients. As a safe and effective treatment, lower limb muscle strength training can improve patients' muscle strength, correct abnormal gait, and reduce the recurrence rate of the disease by paying attention to the dynamic changes of patients' recovery process. At present, the pathogenesis of ITBS remains unclear, and the treatment methods are not unified. It is necessary to further study the biomechanical factors related to the lower extremities and develop more scientific and comprehensive muscle strength training methods.

5.Intensivists' Direct Management without Residents May Improve the Survival Rate Compared to High-Intensity Intensivist Staffing in Academic Intensive Care Units: Retrospective and Crossover Study Design

running ICUs without residents in the near future.METHODS: We performed a retrospective observational study (intensivist crossover design) of medical patients who were transferred to two ICUs from general wards between September 2017 and February 2019 at one academic hospital. We compared the ICU outcomes according to the ICU type (ICU with resident management under high-intensity intensivist staffing vs. ICU with direct management by intensivists without residents).RESULTS: Of 314 enrolled patients, 70 were primarily managed by residents, and 244 were directly managed by intensivists. The latter patients showed better ICU mortality (29.9% vs. 42.9%, P = 0.042), lower cardiopulmonary resuscitation (CPR) (10.2% vs. 21.4%, P = 0.013), lower continuous renal replacement therapy (CRRT) (24.2% vs. 40.0%, P = 0.009), and more advanced care planning decisions before death (87.3% vs. 66.7%, P = 0.013) than the former patients. The better ICU mortality (hazard ratio, 1.641; P = 0.035), lower CPR (odds ratio [OR], 2.891; P = 0.009), lower CRRT (OR, 2.602; P = 0.005), and more advanced care planning decisions before death (OR, 4.978; P = 0.007) were also associated with intensivist direct management in the multivariate cox and logistic regression analysis.CONCLUSION: Intensivist direct management might be associated with better ICU outcomes than resident management under the supervision of an intensivist. Further large-scale prospective randomized trials are required to draw a definitive conclusion.]]>

6.Surgical Resection of Intermetatarsal Coalition of the Fourth and Fifth Founded in Long Distance Running

Intermetatarsal coalition (IC) is very rare; although few cases have been reported in foreign orthopedic journals, these have not originated in our country. We report the case of a 20-year-old man who complained of pain in the left forefoot only during long distance running (3 km). On examination, his foot shape, skin appearance, and gait were normal, with no plantar keratosis; however, the radiograph revealed coalition between the fourth and fifth metatarsals. Surgical excision was performed. In the histopathologic study, fibrous coalition was confirmed. This paper reports an uncommon case involving surgical excision of IC in the military service, involving active sport activity.

8.Current Status of the Operations of Clinical Microbiology Laboratories at Night, on Weekends, and during Public Holidays in Korea: Proposing a Consensus Guideline

BACKGROUND: Clinical microbiological tests are conducted at night, on weekends, and during public holidays in various manners due to the fact that both manual processes and the form of working type, are not well known. Therefore, we surveyed the current running condition of these laboratories and made some suggestions for better-quality clinical microbiology tests. METHODS: We conducted a survey, both online and offline, focusing on the operating styles of clinical microbiological tests within laboratories that had participated in an external quality assessment program, conducted by the Korean Association of External Quality Assessment Service (KEQAS). RESULTS: Out of 341 laboratories that participated in the microbiology program of KEQAS, 128 replied to our questionnaires. In Korea, various types of operations occur within clinical microbiology laboratories. Those in night duty operate in either shifts or dedicated duties. In the case of weekend shifts, they either operated on single-day schedules (either on a Saturday or a Sunday), or over the entire weekend. For public holidays, the laboratories operated in various manners, depending on the number of days off. Among the clinical microbiological tests conducted at night, on weekends, and during public holidays, Gram staining and inoculations are the most common tasks carried out, with some laboratories conducting antibiotic susceptibility tests as well. CONCLUSIONS: Rapid reporting of clinical microbiological test results is currently inadequate due to both cost and labor constraints, despite its many advantages. It would be ultimately beneficial for both the patient and the hospital to switch to a 24/7 operating schedule through the utilization of a variety of methods, including cost control, coordination of a fine workforce, and prioritization of tests needing to be reported.

9.Association between DIO polymorphism and aerobic endurance.

OBJECTIVE:
To study whether polymorphisms in the iodothyronine deiodinase (DIO) gene region contribute to endurance exercise capacity and to validate whether TSHR gene can be used as genetic marks associated with aerobic endurance performance.
METHODS:
Three SNPs (C785T in DIO1 gene regions, Thr92Ala and Gly3Asp in DIO2 gene regions) were selected. The genotypes of the 123 elite long running athletes(EEA) and 127 college students from northern China(CG) were analyzed using the matrix assisted laser desorption ionisation-time of flight mass spectrometry method(MALDI-TOF). The athletes were divided into different groups according to the sports level and the items, which are international masters and masters (43 vs 80), 5/10 km and marathon (92 vs 31).
RESULTS:
There was no significant difference of the C785T loci in DIO1 gene and the Thr92Ala loci in DIO2 gene between the EEA and the CG(P>0.05), while at Gly3Asp loci, the frequency distributions of the 3 genotypes were remarkably different in the groups of control and international masters of sports, as well as in the groups of control and marathon athletes(P<0.05). The genotype TT only existed in EEA not in CG, however, the frequency distribution was very low. The Thr92Ala and Gly3Asp loci of DIO2 gene were in strong linkage disequilibrium. The frequency distributions of the haplotype CT were significantly different in the male CG and the female CG, the male CG and the male EEA(P<0.05), the male CG and the male masters of sports, as well as in the male CG and the male marathon athletes(P<0.05). The frequency distributions of the haplotype TC were remarkably higher in the groups of female international masters of sports and female 5 000 m/10 000 m than those in the female CG(P<0.05).
CONCLUSION
The frequency distributions of the haplotype CT were different in male and female CG, and haplotype CT could be used as a genetic mark associated with aerobic endurance performance of the male EEA, especially for the long running athletes of masters of sports and marathon, while the haplotype CT was associated with the aerobic endurance performance of the female long running athletes of international masters of sports and 5 000 m/10 000 m.

10.Experience of Hospital Administrative Nurses' Work Adjustment

PURPOSE: This study was aimed to explore the work experience of nurses who transitioned from the patient nursing department to administrative departments.METHODS: The participants were 11 nurses who had worked in departments of human resources, education and training, medical insurance review, information technology, and quality improvement. Data were collected through in-depth individual interviews done between December 2017 and July 2018 and were analyzed in terms of Giorgi's phenomenological methodology. The research question was ‘What kind of experience did you have as you transition to a new role from a nurse to a hospital administrative nurse? What are the challenges in carrying out a new role? How do you overcome obstacles and adjust to being a hospital administrative nurse?’RESULTS: The main essence of the administrative nurses' work adjustment experience is represented by six components; 1) Beginning of a new challenge in conflict, 2) Running into barriers, 3) Struggling to take root, 4) Recognizing changes in the hospital, 5) Gradually adjusting to change, 6) Becoming a proficient hospital administrative nurse.CONCLUSION: Based on these findings, it is essential to create systems that enable administrative nurses to adjust to their work without confusion, and continuously develop programs to enhance their competence.
