1.Proteinuria and Hypothyroidism: Two cases illustrating a bidirectional thyroid-kidney relationship

Introduction:
The thyroid-kidney axis represents a clinically significant bidirectional relationship. Nephrotic syndrome (NS) may induce
hypothyroidism via urinary hormonal and binding protein losses, while severe hypothyroidism may mimic NS and
contribute to renal dysfunction.
Cases:
A 64-year-old male with gout, presented with facial puffiness, pedal edema, weight gain and frothy urine. Investigations
revealed severe hypothyroidism FT4 <0.5 pmol/L (12–22 pmol/L) TSH 314 mIU/L (0.27–4.2 mIU/L) with NS – UPCR
7.7 3 g/L (0.04–0.15 g/L), albumin 22 g/L, total cholesterol 15.9 mmol/L – and renal impairment (eGFR 58 mL/min/1.73
m²). Positive anti-TPO antibodies suggested Hashimoto’s thyroiditis. He was initiated on liothyronine, hydrocortisone,
levothyroxine and diuretics. Although FT4 normalized (14.4 pmol/L) a month later, renal function worsened requiring
dialysis. Further evaluation including renal biopsy demonstrated IgA nephropathy as the underlying cause of NS and
renal failure. Treatment with prednisolone led to gradual improvement in renal function and proteinuria (UPCR 0.21 g/L).
A 73-year-old male with hypothyroidism diagnosed 6 months prior (FT4 11.9 pmol/L, TSH 7.23 mIU/L) for thyroxine
replacement, diabetes, hypertension, dyslipidemia and CKD presented with anasarca, decompensated heart failure, pleural
and pericardial effusions requiring ventilatory and inotropic support. On admission, he had marked hypothyroidism (FT4
2.9 pmol/L and TSH 97.2 mIU/L) and moderate proteinuria (UPCR 0.81 g/L). He was similarly treated with liothyronine,
hydrocortisone and levothyroxine. Renal function initially deteriorated (creatinine 439 µmol/L, eGFR 11 mL/min/1.73 m²)
but later returned to baseline with clinical recovery. Despite normalization of FT4 months later, proteinuria persisted; but
renal function remained stable.
Conclusion
These cases highlight the overlapping manifestations of severe hypothyroidism and renal dysfunction with proteinuria.
Each may mimic or exacerbate the other. NS may unmask underlying hypothyroidism. While inadequately treated
hypothyroidism can worsen proteinuric CKD exacerbating hypothyroidism. Concurrent evaluation of both systems
is essential to prevent misdiagnosis and guide timely management.

2.Impact of Glycemic Burden on Renal Filtration Decline and Proteinuria in Type 1 Diabetes Mellitus

Introduction:
Diabetic nephropathy is a major complication of type 1
diabetes mellitus (T1DM) and a leading cause of endstage renal disease. This study aims to correlate long-term
glycemic control and disease duration with renal decline,
measured via estimated glomerular filtration rate (eGFR)
and urinalysis proteinuria.
Methodology:
A retrospective cohort study was conducted among 15
patients with T1DM at Hospital Bentong. Independent
variables were 3-year mean glycated hemoglobin A1c
(HbA1c) and duration of diabetes. Dependent variables
included the change in eGFR derived from baseline and
latest serum creatinine (calculated using the CKD-EPI 2021
equation), and the shift in urine protein severity, mapped
to an ordinal scale, over a 3-year follow-up. Multivariate
linear regression and Spearman’s rank correlation were
utilized for analysis.
Results:
The cohort consisted of 5 male patients and 10 female
patients with a mean initial presentation HbA1c of 11.4%, a
mean 3-year HbA1c of 9.15%, and a median disease duration
of 11 years. Baseline eGFR was preserved (mean 117.15 mL/
min/1.73 m²). Regression analysis revealed that for every 1%
increase in mean HbA1c, eGFR declined significantly by 9.6
mL/min/1.73 m² (p = 0.002). Furthermore, each additional
year of disease duration independently reduced eGFR by
1.1 units (p = 0.008). Spearman validation confirmed the
strong negative correlation between HbA1c and eGFR (Rho
= -0.664, p = 0.006). Conversely, the correlation between
3-year mean HbA1c and worsening proteinuria was not
statistically significant (Rho = -0.067, p = 0.827), and patient
gender did not significantly impact progression (p = 0.129).
Conclusion
Poor long-term glycemic control and longer duration of
diabetes mellitus are strong independent predictors of
decline in functional glomerular filtration in T1DM. While
worsening proteinuria may not be detected during a short
3-year window using a standard urinalysis (UFEME), a
more sensitive test such as Urine Albumin-to-Creatinine
Ratio may better detect early structural kidney damage.

3.Recognizing RENI Syndrome: A Case of Adrenal Insufficiency, Ichthyosis, and Proteinuria

Introduction:
RENI syndrome (renal, endocrine, neurologic, and immune
syndrome), also known as sphingosine-1-phosphate
lyase insufficiency syndrome (SPLIS), is a rare autosomal
recessive disorder caused by pathogenic variants in
the SGPL1 gene. This gene encodes sphingosine-1-
phosphate lyase, an enzyme responsible for the final step
in sphingolipid degradation. Impaired enzyme activity
results in the accumulation of sphingolipid intermediates,
leading to multisystem involvement affecting the kidneys,
adrenal glands, skin, immune system, and nervous system.
Frequently reported manifestations include steroid-resistant
nephrotic syndrome, primary adrenal insufficiency,
ichthyosis, and neurological abnormalities.
Case:
We report a 15-year-old male with a history of primary
adrenal insufficiency diagnosed at age three after
presenting with recurrent vomiting and abdominal pain.
He was started on steroid replacement therapy and
remained stable without episodes of adrenal crisis. He
also had ichthyosis requiring dermatological follow-up.
During routine surveillance, persistent proteinuria was
detected, and urinary protein excretion increased over time. Ultrasound showed renal parenchymal changes
despite preserved renal function. Whole-exome sequencing
identified a homozygous variant of uncertain significance
in the SGPL1 gene, consistent with RENI syndrome. He
was started on enalapril for his proteinuria and continues
to undergo multidisciplinary follow-ups. Both parents are
consanguineous, but were not screened.
Mutations in SGPL1 have increasingly been recognized as a
monogenic cause of syndromic steroid-resistant nephrotic
syndrome, associated with endocrine and dermatological
features. Renal histopathology in cases reported often
shows focal segmental glomerulosclerosis, reflecting
podocyte injury due to disruption of sphingolipid signaling
pathways. Early recognition is crucial, as patients need
multidisciplinary care.
Conclusion
This case highlights the importance of considering RENI
syndrome in patients presenting with early-onset primary
adrenal insufficiency accompanied by renal and dermatological symptoms. Increased clinical awareness and prompt
genetic testing can enable earlier diagnosis, guide long-term
monitoring, and support appropriate genetic counseling.

4.Establishing the diagnostic accuracy of point-of-care ratiometric urine protein:creatinine test with 24-h total protein measurement for preeclampsia

INTRODUCTION
Preeclampsia, a multisystemic, multifactorial disorder, is the second leading cause of maternal deaths in the Philippines. It is diagnosed by the presence of hypertension and proteinuria or significant end-organ damage in a parturient carrying at least 20 weeks age of gestation. Proteinuria, in preeclampsia, is diagnosed by having 300 mg protein in a 24-h urine sample, a 0.3 mg/mg urine protein:creatinine ratio, or 2+ protein on a urine dipstick. All currently available diagnostic tests have their advantages and disadvantages. A novel diagnostic test, the spot ratiometric urine protein:creatinine dipstick test kit, was developed to meet the limitations of the currently available methods. Early diagnosis of preeclampsia will help in the prompt management to decrease maternal and neonatal complications.
OBJECTIVESThe objective of this study was to compare the diagnostic accuracy of the spot ratiometric urine protein:creatinine dipstick test (SUPCR) in comparison to 24-h urine protein (24HUP) in the diagnosis of preeclampsia.
MATERIALS AND METHODSA non-experimental cross-sectional study comparing spot ratiometric urine protein:creatinine dipstick test (SUPCR) to 24HUP and urine dipstick among parturients with elevated blood pressure in a tertiary hospital to diagnose preeclampsia.
RESULTSA total of 190 parturients were included. SUPCR showed a sensitivity of 88.36%, a specificity of 93.18%, and a likelihood ratio (LR) of 12.96. Urine dipstick (2+) showed a sensitivity of 26.03%, a specificity of 95.45%, and an LR of 5.73.
CONCLUSIONSSUPCR can be an alternative to 24HUP in detecting preeclampsia among pregnant patients due to its high sensitivity, specificity, and LR values. This novel diagnostic can be used in low-resource settings due to its fast results, low cost, and ease of use.

5.PLCE1 mutation-induced end-stage renal disease presenting with massive proteinuria: a family analysis and literature review.

OBJECTIVES:
To summarize the clinical and genetic characteristics of end-stage renal disease caused by PLCE1 gene mutations.
METHODS:
A retrospective analysis of the clinical and genetic features of three children from a family with PLCE1 gene mutations was conducted, along with a literature review of hereditary kidney disease cases caused by PLCE1 gene mutations.
RESULTS:
The proband was an 8-year-old male presenting with nephrotic syndrome stage 4 chronic kidney disease. Renal biopsy showed focal segmental glomerulosclerosis. Two years and five months after kidney transplantation, the patient had persistent negative proteinuria and normal renal function. Whole-exome sequencing identified two pathogenic heterozygous variants: c.961C>T and c.3255_3256delinsT, with c.3255_3256delinsT being a novel mutation. Family screening revealed no renal involvement in the parents, but among five siblings, one brother died at age of 4 years from end-stage renal disease. A 7-year-old sister presented with proteinuria and bilateral medullary sponge kidney, with proteinuria resolving after one year of follow-up. A 3-year-old brother died after kidney transplantation due to severe pneumonia. The literature review included 45 patients with hereditary kidney disease caused by PLCE1 gene mutations. The main clinical phenotype was nephrotic syndrome (87%, 39/45), and renal pathology predominantly showed focal segmental glomerulosclerosis (57%, 16/28). No mutation hotspots were identified.
CONCLUSIONS
Compound heterozygous mutations in the PLCE1 gene can lead to rapid progression of the disease to end-stage renal disease, with favorable outcomes following kidney transplantation. Family screening is crucial for early diagnosis, and medullary sponge kidney may be a novel phenotype associated with these gene mutations.

6.Clinical and Laboratory Characteristics of Cold Agglutinin Disease Patients with Positive Results of Acidified-Serum Lysis Test.

OBJECTIVE:
To analyze the clinical features and laboratory characteristics of patients with cold agglutinin disease (CAD)/cold agglutinin syndrome (CAS) who were positive for acidified-serum lysis test (Ham test), and to compare them with Ham test negative CAD/CAS patients and paroxysmal nocturnal hemoglobinuria (PNH) patients, in order to provide references for the differential diagnosis of these diseases.
METHODS:
53 patients diagnosed with CAD/CAS and 67 patients diagnosed with classic PNH in our hospital from January 2015 to December 2020 were retrospectively analyzed. The patients were grouped according to clinical diagnosis and results of cold agglutinin test (CAT), direct antiglobulin test (DAT), Ham test and PNH clone detection. The clinical and laboratory characteristics of each group were compared.
RESULTS:
The patients were grouped as follows: Ham- CAD/CAS group, CAD/CAS patients negative for Ham test (n=36); Ham+ CAD/CAS group, CAD/CAS patients positive for Ham test (n=17); classic PNH group (n=67). Compared with the classic PNH group, the Ham+ CAD/CAS group had a higher median age (P =0.024), weaker positivity of Ham test, higher positive rates of CAT and DAT, and lower positive rate of PNH clone detection (all P <0.001). The proportions of patients with splenomegaly and cyanosis in Ham+ CAD/CAS group were significantly higher than those in classic PNH group (P =0.002 and P <0.001). Ham+ CAD/CAS group displayed lower red blood cell count (RBC) and lactate dehydrogenase (LDH) level (P =0.007 and P <0.001), and higher mean corpuscular hemoglobin (MCH), mean corpuscular hemoglobin concentration (MCHC), and indirect bilirubin (IBIL) level (P =0.003, P =0.004 and P =0.006) than those in classic PNH group. The levels of serum complement C3 and C4 in Ham+ CAD/CAS group were lower than those in classic PNH group (P =0.001 and P <0.001). The positive rate of urinary occult blood in Ham+ CAD/CAS group was lower than that in classic PNH group (P =0.010). The clinical and laboratory characteristics of Ham+ CAD/CAS group were similar to those of Ham- CAD/CAS group, except for median age, hemoglobin (Hb), MCHC, mean corpuscular volume (MCV), reticulocyte ratio (Ret), Ham test results, DAT positive types, and proportion of splenomegaly.
CONCLUSION
Some clinical features and laboratory indicators of CAD/CAS patients with positive results of Ham test are different from those of classic PNH patients, but relatively similar to those of CAD/CAS patients with negative results of Ham test. These results may provide a reference for differential diagnosis of related diseases.

7.Modified Hu-Lu-Ba-Wan Alleviates Early-Stage Diabetic Kidney Disease via Inhibiting Interleukin-17A in Mice.

OBJECTIVE:
To identify the underlying molecular mechanism of Modified Hu-Lu-Ba-Wan (MHW) in alleviating renal lesions in mice with diabetic kidney disease (DKD).
METHODS:
The db/db mice were divided into model group and MHW group according to a random number table, while db/m mice were settled as the control group (n=8 per group). The control and model groups were gavaged daily with distilled water [10 mL/(kg·d)], and the MHW group was treated with MHW [17.8 g/(kg·d)] for 6 weeks. After MHW administration for 6 weeks, indicators associated with glucolipid metabolism and urinary albumin were tested. Podocytes were observed by transmission electron microscopy. Kidney transcriptomics was performed after confirming therapeutic effects of MHW on DKD mice. The relevant target of MHW' effect in DKD was further determined by enzyme-linked immunosorbent assay, Western blot analysis, immunohistochemistry, and immunofluorescence staining.
RESULTS:
Compared with the model group, MHW improved glucose and lipid metabolism (P<0.05), and reduced lipid deposition in the kidney. Meanwhile, MHW reduced the excretion of urinary albumin (P<0.05) and ameliorated renal damage. Transcriptomic analysis revealed that the inflammation response, particularly the interleukin-17 (IL-17) signaling pathway, may be responsible for the effect of MHW on DKD. Furtherly, our results found that MHW inhibited IL-17A and alleviated early fibrosis in the diabetic kidney.
CONCLUSION
MHW ameliorated renal damage in DKD via inhibiting IL-17A, suggesting a potential strategy for DKD therapy.

8.Bioinformatics analysis of a CLCN5 geneframeshift mutation in a patient with Dent disease.

Dent disease is a rare X-linked recessive inherited renal tubular disorder characterized by low molecular weight proteinuria (LMWP), hypercalciuria, nephrocalcinosis, and other clinical features, and can lead to progressive renal failure. It is primarily caused by mutations in the CLCN5 gene. This article reports the case of a 10-year-old male patient of Chinese descent who was incidentally found to have asymptomatic proteinuria during a routine health examination. Comprehensive biochemical testing and clinical evaluation revealed significant LMWP and hypercalciuria, while renal biopsy showed mesangial cell and matrix proliferation. Whole exome sequencing identified a novel deletion mutation in the CLCN5 gene (NM_001127899.4, c.1158delC, p.F387Lfs*42) causing a frameshift and premature termination, which is likely to disrupt its role in chloride/hydrogen ion exchange and endosomal acidification. Bioinformatic analysis indicated the variant is pathogenic. Genetic testing plays an important role in diagnosing rare kidney diseases. Early identification of pathogenic mutations is essential for facilitating timely intervention and appropriate management, potentially enhancing patient outcomes. This report expands the CLCN5 mutation spectrum and contributes to understanding the genetic and molecular mechanisms of Dent disease.

9.Diagnostic accuracy of urine protein-creatinine ratio dipstick test in the diagnosis of preeclampsia

INTRODUCTION
Hypertension disorders in pregnancy cause significant number of maternal morbidity and mortality. In local statistics for the years 2019–2022, hypertension causes 13.8% of the maternal mortality. Thus, accurate diagnosis of Preeclampsia is crucial to prevent disease progression and to provide timely intervention for improved maternal outcomes. It is widely accepted that 24-h urine protein is the gold standard for detecting proteinuria in patients with preeclampsia, but since the process of collection is too long and complicated, recent studies focus on other less complex yet reliable methods of determining proteinuria for the diagnosis of preeclampsia, including the protein–creatinine ratio (PrCr) dipstick tests.
GENERAL OBJECTIVEThis study aims to determine the diagnostic accuracy of urine protein detection in patients with preeclampsia, using a urine PrCr dipstick test.
MATERIALS AND METHODSA prospective, cross-sectional study using purposive sampling was used in this study. A total of 153 admitted pregnant patients with gestational hypertension and preeclampsia, without other comorbidities or significant past medical history, were tested for proteinuria using the 24-h urine protein test and urine PrCr dipstick test. Statistical analysis to assess diagnostic accuracy used was the sensitivity, specificity, positive predictive value, and negative predictive value.
CONCLUSIONSThe urine PrCr dipstick test has comparable diagnostic accuracy with 24-h urine protein test in detecting proteinuria, with a sensitivity of 88%, a specificity of 64%, and a high positive predictive value of 94%. It is a simpler, faster, yet useful alternative to a more tedious, time and resource consuming process of urine collection in the 24-h urine protein in identifying patients with proteinuria, and therefore, preeclampsia.
