2.A Case of Small Bowel Obstruction Due to a Paracecal Hernia.

Internal hernias are rare causes of small bowel obstruction, and one such internal hernia is the paracecal hernia. We report a case of a small bowel obstruction related to a paracecal hernia in which a preoperative diagnosis was made on computed tomography. A laparotomy was performed for definitive diagnosis and treatment. The surgery achieved a good outcome.

Primary or idiopathic greater omental torsion is an uncommon cause of an acute surgical abdomen. The etiology of omental torsion is as yet unknown. The preoperative diagnosis is usually hard, and generally made at laparotomy, due to the objective rareness and absence of typical symptoms. Resection of the infarcted segment is the treatment of choice, offering rapid recovery, and reducing the possibility of adhesion formation. A case of primary omental torsion is reported, with a review of the literature.

9.Intracorporeal Anastomosis in Laparoscopic Gastric Cancer Surgery.

Laparoscopic gastrectomy has become widely used as a minimally invasive technique for the treatment of gastric cancer. When it was first introduced, most surgeons preferred a laparoscopic-assisted approach with a minilaparotomy rather than a totally laparoscopic procedure because of the technical challenges of achieving an intracorporeal anastomosis. Recently, with improved skills and instruments, several surgeons have reported the safety and feasibility of a totally laparoscopic gastrectomy with intracorporeal anastomosis. This review describes the recent technical advances in intracorporeal anastomoses using circular and linear staplers that allow for totally laparoscopic distal, total, and proximal gastrectomies. Data that demonstrate advantages in early surgical outcomes of a total laparoscopic method compared to laparoscopic-assisted operations are also discussed.

10.A case of trapped placenta in uterine sacculation after normal delivery.

Uterine sacculation is a very rare functional malformation associated with pregnancy in which a part of the uterine wall balloons. It is a transitory pouch or sacculation and resolves once the uterine contents are delivered. The uterine sacculation frequently contains the placenta. If the aperture of the sacculation is small, manual delivery of the placenta is impossible. It explains why many patients with uterine sacculation ends up with having a laparotomy with or without a hysterectomy. We report a case of a trapped placenta in uterine sacculation after normal delivery which is removed by laparotomy.
