Journal of Chongqing Medical University 2010;35(1):113-115
Objective: To investigate the relationship between glycated albumin levels and diabetic nephropathy in patients with type 2 diabetes mellitus. Methods: From January 2006 to June 2009,78 patients with type 2 diabetes mellitus in our hospital were enrolled in the study. We used liquid enzymatic to detect glycated albumin, and compare with 50 cases of healthy in the same period as the control group. Results: The glycated albumin was different between the control group and the diabetic nephropathy groups(F=8.953,P=0.000). Glycated albumin in each diabetes group was higher than that in the control group;glycated albumin in diabetic nephropathy groups was higher than that in non-diabetic nephropathy group;glycated albumin in end-stage diabetic nephropathy group was higher than that in the rest of groups. The severity of diabetic nephropathy and level of glycated albumin were related,rs was 0.754(t=6.547,P=0.008).Logistic regression showed that glycated albumin was an independent risk factor impacting on the incidence of diabetic nephropathy and the severity of the disease,multi-adjusted OR value was 2.326,and 95%CI was 2.114-2.538.Conclusions:Glyeated albumin plays an important role in the pathogenesis of diabetic nephropathy ,and it is closely related to the phases of diabetic nephropathy.
5.Effect of chemokines, activity enhanced by mobilization of bone marrow cells on solution and absorption of venous thrombosis
Journal of Chongqing Medical University 2010;35(1):110-113
Objective: To discuss the effect of bone marrow mobilization on chemotaxis of aggregation of macrophages in venous thrombosis. Methods:SD adult rats were divided randomly into normal group, sham operation group, thrombus group and rhG-CSF group.Peripheral blood was harvested at 30min before operation and on the 7th and 14th day after operation.Level of MCP-3 in peripheral blood was detected by ELISA.Inferior vena cava specimens of thromus group and rhG-CSF group were collected at postoperative 7th and 14th day. With the application of RT-PCR,expression of MCP-3mRNA in thrombus was detected. Results:The level of MCP-3 in peripheral blood of rhG-CSF group was significantly higher than that before operation and also higher than the levels of MCP-3 in peripheral blood of sham operation group and thrombus group on the 14th day after operation(P<0.05),and the difference was significant. On the 7th and 14th day after operation, compared with thrombus group, the expression of MCP-3 mRNA in rhG-CSF group remarkablely increased, and the difference Was significant beween these two groups(P<0.01).Conclusion:Brone marrow cell mobilization enhances the chemotaxis of MCP-3 on macrophages.
6.The value of enteral immunonutrition in therapeutic hepatectomy for patients with primary liver cancer
Journal of Chongqing Medical University 2010;35(1):100-102
Objective: The aim of this study was to investigate the immune function and prognosis for patients with primary hepatic cancer who implemented the perioperative enteral immunonutrition support for hepatectomy. Methods: 58 cases of patients with primary hepatic cancer in our hospital in 2003 treated with liver resection were randomly divided into nutritional support fur immune nutrition group and conventional control group.Immune function was detected on admission,1st day and 7th after surgery;5-year follow-up was performed to compare the prognosis of the two groups. Results: Concentration level of CD3+,CD4+,CD8+and CD4+ICD8+in preoperative hepatectomy were the same between the two groups. On the first day after surgery,CD3+,CD4+,CD8+content decreased significantly; there was no difference between the two groups. On the 7th day after surgery,CD3+,CD4+,CD8+content increased;and the intervention group increased more significantly.1,2,3,4,and 5-year survival rates in intervention group and control group were 82.8%,70.2%, 39.6%,22.6%.20.6%and 74.6%,50.1%,25.4%,21.5%,19.5%.The survival rate was higher in the intervention group than the control group at 1-year,2-year and 3-year,while the 4 and 5-yearsurvival rate of the two groups was same, and the intervention group had better overall survival compared with the control group(X~2=3.547,P=0.025).The OR value for enteral immunonutrition support impacting postoperative survival was 0.649 after multivariate analysis adjusted.with 95.0%CI of 0.614-0.686.Conclusion:Enteral immunonutrition support can ameliorate immune suppression in hepatectomy for patients with primary liver cancer during perioperative period, and it also Can improve postoperative survival.
7.Hospital-acquired pneumonia due to acinetobacter baumannii in RICU- a report of 49 cases
Journal of Chongqing Medical University 2009;34(11):1584-1587
Objective: To investigate the prevalence, antibiotic resistance and risk factors of Acinetobacter Baumannii hospital-acquired pneumonia (ABHAP) in Respiratory Intensive Care Unit (RICU) of our hospital for guidance of clinical prevention and treatment of ABHAP. Methods: Clinical information and results of Antibiotic susceptibility test for all of the patients with hospital-acquired pneumonia (HAP) in RICU of our hospital from May 2007 to October 2008 were collected. Antibiotic resistance and risk factors of ABHAP were analyzed retrospectively. Results: 159 patients suffered from HAP. Of which, 49 patients had ABHAP and accounted for 30.8% of all cases. Acinetobacter Baumannii was the second commonest pathogen of HAP. The resistance rate of Acinetobacter Baumannii strains to the third generation of Cephalosporin and Fluoroquinolone were over 90%. And the resistance rate to Imipenen was 42.7%. Pan-resistant rate was 23.5%. Univariate analysis showed that APACHEII score≥20, hospitalization in RICU ≥2 weeks, invasive mechanical ventilation, invasive mechanical ventilation ≥7 days, trachea intubation or tracheotomy, trachea intubation or tracheotomy ≥7 days and broad-spectrum antibiotic therapy ≥7 days were associated with BAHAP. Multivariate logistic analysis showed that hospital ization in RICU ≥2 weeks, invasive mechanical ventilation ≥7 days, APACHEII score ≥20 and broad-spectrum antibiotic therapy ≥7 days were independent risk factors of ABHAP. Conclusions: The present study found high incidence rate, severe drug-resistance and therapeutic difficulties of ABHAP in RICU. Shortening duration of hospitalization and invasive mechanical ventilation, rational use of antibiotics are helpful to prevent and treat ABHAP.

9.Magnetic resonance neurography of lumbar plexus in healthy people
Journal of Chongqing Medical University 2008;0(S1):-
Objective:To Compare the difference between the data of normal lumbar plexus nerve measured through magnetic resonance neurography(MRN) with specimen. Methods:Volunteers of 30 healthy lumbar plexus nerve cases(19 males and 11 females) were selected, to measure the short shaft line and neuromuscular signal ratio of the right normal lumbar nerve root, ganglia, nerve and the femoral nerve stem, anatomy and measurements of the right lumbar plexus L2~5 nerve root, ganglia and nerve stem, femoral nerve morphology and size. And comparison of MRN measurement data and autopsy measurement data were conducted. Results:MRN could clearly show L2~5 nerve course, ganglion being circle or Oval enlargement, with the size about 3~6mm;a nerve root and stem displayed strips and smooth edge, with the size about 2~5mm, nerve Festival signal is more than the nerve root and stem. There was no significant statistical difference with the comparability between the obtained data and autopsy ones. Conclusion:MRN can clearly show lumbar plexus and major branches; its measured value can reflect changes in lumbar plexus. It is possible for diagnostic criteria of the lumbar plexus nerve diseases and dysplasia.
