1.A Case of Megameatus Intact Prepuce Hypospadias Variant.

Megameatus intact prepuce variant is an unusual anterior hypospadias variant.This type of hypospadias is the combination of widely splayed coronal or subcoronal megameatus, deep glandular groove, and an intact prepuce.There is no chordee with this variant. The over all incidence of the MIP variant is approximately 3 percent (6 percent of anterior hypospadias). Intact prepuce can lead to possible late recognition of this malformation. MIP variant is one of the most technically challenging types of hypospadias to repair. So, recognition is important because of uncommon combination and technical challenge of this malformation. I report a very rare case of Megameatus Intact Prepuce hypospadias variant and reviewed .the relevant literatures.

3.Antibiotic Sensitivity to the Causative Organism of Acute Simple Urinary Tract Infection for Recent 3 Years.

BACKGROUND: We studied the antibiotic sensitivities to the causative microorganisms of acute simple urinary tract infection for recent 3 years. METHODS: We analyzed 112 microorganisms and their antibiotic sensitivities of the 104 patients who were admitted to or visited the Department of Urology, Catholic University St. Marys Hospital and had more than 10cfu/mL on urine culture from June 1996 to January 1999 retrospectively. RESULTS: The chance of gram negative and positive as causative microorganisms was 72.3% and 27.7% respectively. The most common pathogenic microorganisms were Escherichia coli (67.0%) followed by a-hemolytic streptococci, Entercoccus. In gram negative acute UTI, imipenem, sulperanzone showed relatively higher sensitivity, while cotrimoxazole, ampicillin showed relatively lower sensitivity. In gram positive, vancomycin, penicillin showed relatively higher sensitivity, while ampicillin, imipenem showed relatively lower sensitivity. CONCLUSIONS: We consider that gram negative microorganisms, especially E. coli, is the main cause of acute simple UTI. But, we should be concerned about the increase of gram positive organisms and other gram negative organisms besides E. coli. Regarding to the choice of adequate drug in the treatment of UTI, it is necessary to consider the change of pathologic microorganisms.

6.Evaluation of Reduced Port Laparoscopic Distal Gastrectomy Performed by a Novice Surgeon

Purpose:
Reduced port laparoscopic distal gastrectomy (RPLDG) using 3 ports is less invasive than conventional laparoscopic distal gastrectomy (CLDG) using 5 ports. Although RPLDG performed by expert surgeons is safe and feasible, novice surgeons have difficulty performing this procedure. This study evaluated the surgical outcomes and feasibility of RPLDG performed by a novice surgeon.
Materials and Methods:
The records of 136 patients who underwent laparoscopic distal gastrectomy for gastric cancer performed by a single novice surgeon between May 2016 and December 2018 were retrospectively reviewed. Among these 136 patients, 52 underwent RPLDG and 84 underwent CLDG. The clinicopathological characteristics, operative outcomes, and short-term postoperative outcomes of the 2 groups were compared.
Results:
The percentage of women was significantly higher in the RPLDG group than in the CLDG group (48.1% vs. 31%; P=0.045), but other baseline characteristics did not differ significantly between the groups. Billroth II anastomosis was performed significantly more frequent (90.4% vs. 73.8%, P=0.015) and operation time was significantly shorter (207.1±43.3 min vs. 225.5±44.6 min, P=0.020) in the RPLDG group than in the CLDG group. The time to first flatus, postoperative pain score, length of postoperative hospital stay, and incidence and severity of complications did not differ significantly between the groups. Analysis of the learning curve based on the operation time showed that performing RPLDG on 20–30 patients was required to achieve technical proficiency.
Conclusions
RPLDG is a safe and feasible surgical procedure for the treatment of gastric cancer, even when performed by a novice surgeon.
7.The Effects of Human Muscle Derived Stem Cells on the Induction of Peripheral Nerve Regeneration.

PURPOSE: In this study, we evaluated the extent of functional and histological axonal regeneration after resection of the sciatic nerve. The nerve was repaired with silicone tubes filled with human muscle derived stem cells(MDSCs) and neuronal progenitor cells(NPCs) in nude mice. MATERIALS AND METHODS: The human muscle samples were obtained from the rectus abdominis muscle of 12 patients that underwent a laparotomy. The MDSCs were isolated using a modified preplate technique. Using the MDSCs, neurogenic differentiation was induced by dissociating neurospheres produced in a neurosphere culture medium containing neuronal induction agents. A part of the sciatic nerve, approximately 7 mm in length, was excised bilaterally, and a 9mm long silicone tube guide was placed at the resulting gap in 40 nude mice. The transplanted sites were divided randomly into three groups according to the type of grafting cells: silicone tube guides filled with PBS(P group, n=20), silicone tube guides filled with MDSCs(M group, n=40) and silicone tube guides filled with NPCs(N group, n=20). Histological observations and a nerve conduction study were performed 12 weeks after the graft. RESULTS: The number and diameter of the myelinated axons were significantly increased in the M and N groups(p<0.001). In a nerve conduction study, the amplitude of the compound muscle action potential(CMAP) and motor latency of response were significantly higher and shorter in the M and N groups(p<0.001). Moreover, reaction with neuronal class beta-tubulin(Tuj1, a neuronal marker) and antiglial fibrillary acidic protein(GFAP, a glial marker) was observed in the regenerated nerve that originated from the M and N groups.onclusions: These results show that MDSCs can differentiate into NPCs and improve the peripheral nerve regeneration rate after transplantation into a nerve guide.

8.Urodynamic Evaluations According to Clinical Findings and Clinical Significance of Glycosylated Hemoglobin (HbA1C) in Patients with Diabetic Cystopathy.

PURPOSE: In an effort to characterize the complex voiding dysfunction associated with symptomatic diabetic patients, the results of urodynamic studies of diabetic patients were retrospectively analyzed. MATERIALS AND METHODS: We retrospectively reviewed the clinical information, laboratory findings and urodynamic data, of patients with diabetic cystopathy (DCP) (20 males and 40 females) between January 1997 and December 2001. The urodynamic findings were classified as detrusor instability (DI), impaired detrusor contractility (IDC), detrusor areflexia (DA) and normal detrusor contraction (ND). RESULTS: The mean duration of diabetes in irritative symptom group (40%), combined symptom group (26.7%) and obstructive symptom group (33.3%) was 3.4 years, 8.3 years and 15.2 years, respectively (p<0.05). Of the 20 patients with serum HbA1C<7%, 15 were noted in the irritative symptom group. Of the 20 patients with serum HbA1C>or=7%, 17 were noted in the obstructive symptom group. The urodynamic findings of the 60 diabetic patients were distributed as follows: 22 (36.7%) detrusor instability, 20 (33.3%) impaired detrusor contractility, 16 (26.7%) detrusor areflexia and 2 (3.3%) normal detrusor contraction. CONCLUSIONS: The shorter the duration of diabetes, the more predominant were the signs of irritative symptoms, whereas the longer the duration of diabetes, the more predominant were the obstructive voiding symptoms. The patients with serum HbA1C<7% mostly complained of irritative symptoms, but the patients with serum HbA1C>or=7% mostly complained of obstructive symptoms.

9.Effect of Prostatectomy on Detrusor Instability Associated with Benigh Prostatic Hyperplasia: 11 Cases.

PURPOSE: Detrusor instability is common in men with benign prostatic hypertroplasia(B.P.H.) and known to be reversed in about two thirds of patients after operation. We evaluated the fate of detrusor instability in B.P.H patients after operation. MATERIALS AND METHODS: In 11 patients with urgency or urge incontinence combined with B.P.H., cystometries were performed at baseline and 3 months after operation. RESULTS: Detrusor instability associated with B.P.H was reversed postoperatively in 7 patients (63.6%). CONCLUSIONS: Detrusor instability with urgency or urge incontinence can be managed by operation for B.P.H. This will improve the quality of life for B.P.H. patients.

10.The Characteristics of Voiding Difficulty in Women and the Effect of Tamsulosin.

PURPOSE: The aim of this study was to define the characteristics of female voiding difficulty, and evaluate the effects of the alpha-blocker, tamsulosin, on the symptoms in those patients. MATERIAL AND METHODS: 148 patients, who complained of voiding difficulty between March 2002 and September 2004, were retrospectively evaluated. 32 patients with anatomical and neuropathic causes were excluded, with the remaining 116 assigned to 4 groups from their urodynamic evaluations: group I, bladder outlet obstruction (BOO) only; group II, BOO plus an overactive bladder (OAB); group III, detrusor underactivity (DU) only; and group IV, DU plus an OAB. After 2 weeks of observation, tamsulosin, 0.2mg/d, was prescribed in all groups, with the patients re-evaluated after 3 months. RESULTS: 58 (50%), 23 (19.8%), 20 (17.3%) and 15 (12.9%) of the 116 study subjects were placed into groups I, II, III and IV, respectively. In group I, 45 (77.5%) had symptomatic improvement after taking tamsulosin for 3 months, and in group II improvement was observed in 73.9% (17/23) of patients. In groups III and IV; however, improvements were seen in only 25 (5/20) and 13.3% (2/15) of cases, respectively. Dizziness, postural hypotension and urinary incontinence occurred in some patients, but these disappeared after the medication was discontinued. CONCLUSIONS: 81 patients (69.8%) had a bladder outlet obstruction, without detrusor underactivity, and 62 (76.5%) of these exhibited a voiding improvement after taking tamsulosin for three months. However, in patients with detrusor underactivity, the response rate was very low (20%).
