Objectives: This study aimed to evaluate the effectiveness of a multidisciplinary pulmonary rehabilitation program at Ewha Womans University Mokdong Hospital, Korea that was designed to augment respiratory function, promote patient recovery, and improve discharge outcomes.

Methods: This study was conducted as part of quality improvement activities from March to August 2023. A multidisciplinary pulmonary rehabilitation team was established, comprising specialists in rehabilitation medicine, pulmonology, and thoracic surgery, as well as physical and occupational therapists, and the nutrition team. We developed a comprehensive rehabilitation program that included stretching, strengthening exercises, aerobic training, and respiratory retraining, supplemented by individualized treatments such as bedside and ambulatory physical therapy. Evaluations before and after rehabilitation were performed using the Modified Medical Research Council Dyspnea Scale and the Chronic Obstructive Pulmonary Disease Assessment Test. The primary indicators of success were an increase of over 10% in pulmonary rehabilitation consultations and an improvement in discharge-to-home rates.

Results: The total number of pulmonary rehabilitation cases rose by 79.16%, from 24 to 43, while consultations saw a 21.15% increase, from 52 to 63. Of the 63 pulmonary rehabilitation cases, 32 (50.8%) were transferred to other hospitals, 25 (39.7%) were discharged home, and 6 (9.5%) remained hospitalized.

Conclusion: The study demonstrated the effectiveness of a multidisciplinary pulmonary rehabilitation approach in improving program participation and achieving meaningful discharge outcomes. These findings underscore the potential for expanding multidisciplinary efforts and highlight the need for further studies to increase discharge-to-home rates and evaluate the long-term impacts of such programs.

However, the relationship between caffeinated beverages and sleep quality, considering frequency or types of caffeinated beverages, were not extensively studied. Methods: Data were collected from 160 urban workers (75 men [46.9%] aged 20–69 years; with an average age of 41.8±12.3 years) using a structured, self-administered online questionnaire. Sleep quality, time, satisfaction; types and frequency of caffeinated beverages (number of cups per week; Q1: 0 cup, Q4: 14 or more cups per week), demographics, and health behaviors were asked. Sleep quality were evaluated using the Korean version of the Pittsburgh Sleep Quality Index (PSQI-K). Multiple regression analysis was conducted on the association between the frequency of caffeinated beverages consumption and sleep quality. Results: The most frequently consumed beverages were unsweetened coffee (22.8%) and the most common time for caffeine was between 12 pm to 5 pm (58.2%). The average sleep quality score based on the PSQI-K was 6.0±2.0 overall, 5.3±1.6 in Q1, and 6.6±2.2 in Q4 (frequent caffeinated beverage drinkers), indicating poorer sleep quality in Q4 (P=0.022). In Q1, 13.3% rated their sleep quality as ‘very good,’ while in Q4, only 2.5% gave the same rating. Poor sleep quality was significantly associated with the frequency of caffeinated beverages per week (β=0.232, P=0.004) and self-reported stress level (β=0.256, P=0.002). Conclusions Frequent consumption of caffeinated beverages appears to be associated with poor sleep quality among urban workers. While reducing caffeine intake may contribute to improvements in sleep quality as a health promoting behavior, this hypothesis requires validation through future studies employing personalized intervention approaches.

Background: The World Health Organization has declared the end of the coronavirus disease 2019 (COVID-19) public health emergency. However, this did not indicate the end of COVID-19. Several months after the infection, numerous patients complain of respiratory or nonspecific symptoms; this condition is called long COVID. Even patients with mild COVID-19 can experience long COVID, thus the burden of long COVID remains considerable. Therefore, we conducted this study to comprehensively analyze the effects of long COVID using multi-faceted assessments. Materials and Methods: We conducted a prospective cohort study involving patients diagnosed with COVID-19 between February 2020 and September 2021 in six tertiary hospitals in Korea. Patients were followed up at 1, 3, 6, 12, 18, and 24 months after discharge. Long COVID was defined as the persistence of three or more COVID-19-related symptoms. The primary outcome of this study was the prevalence of long COVID after the period of COVID-19. Results: During the study period, 290 patients were enrolled. Among them, 54.5 and 34.6% experienced long COVID within 6 months and after more than 18 months, respectively. Several patients showed abnormal results when tested for post-traumatic stress disorder (17.4%) and anxiety (31.9%) after 18 months. In patients who underwent follow-up chest computed tomography 18 months after COVID-19, abnormal findings remained at 51.9%. Males (odds ratio [OR], 0.17; 95% confidence interval [CI], 0.05–0.53; P=0.004) and elderly (OR, 1.04; 95% CI, 1.00–1.09; P=0.04) showed a significant association with long COVID after 12–18 months in a multivariable logistic regression analysis. Conclusion Many patients still showed long COVID after 18 months post SARS-CoV-2 infection. When managing these patients, the assessment of multiple aspects is necessary.

Purpose: To report a case of ocular inflammation after bisphosphonate use in a patient with osteoporosis.Case summary: A 54-year-old female presented to our clinic with eye pain and conjunctival and episcleral vessel injection in both eyes for 1 month. The patient was diagnosed with osteoporosis by the Department of Endocrinology 7 weeks ago and prescribed risedronate, a bisphosphonate. After using risedronate for 3 weeks, the patient developed generalized body pain, eye pain, and conjunctival injection. Slit lamp examination demonstrated conjunctival injection, whereas fundus examination did not reveal any cells in the anterior chamber or any other abnormal findings. Subsequently, risedronate was discontinued, which relieved the pain. At the 2-month follow-up visit in the outpatient department, the patient was asymptomatic, and there was no conjunctival injection. Conclusions Bisphosphonates can lead to ocular adverse effects, which should be considered in patients with osteoporosis presenting with nonspecific ocular inflammation.

Purpose: This study investigated the effects of a nursing leadership program on self-leadership, interpersonal relationships, clinical performance, problem-solving abilities, and nursing professionalism among nursing students in South Korea. Methods: A quasi-experimental study was conducted. The Practice-Driven Nursing Leadership Program for Students (PDNLP-S) was developed based on the ADDIE model (analysis, design, development, implementation, and evaluation). This quasi-experimental study design included 60 nursing students. The experimental group (n=30) participated in the PDNLP-S for 120-minute sessions over 5 weeks, while the control group (n=30) received usual lectures. The PDNLP-S included lectures, discussions, and individual and group activities to cultivate core nursing leadership competencies such as individual growth, collaboration, nursing excellence, creative problem-solving, and influence. Data were analyzed using descriptive statistics, the Mann-Whitney U-test, and the independent t-test with IBM SPSS Windows ver. 26.0. Results: The experimental group demonstrated significant improvements in self-leadership (t=3.28, p=.001), interpersonal relationships (t=3.07, p=.002), clinical performance (U=268.50, p=.004), and problem-solving abilities (t=2.20, p=.017) compared to the control group. No significant difference was observed in nursing professionalism (t=0.50, p=.311). Conclusion This study demonstrates that the PDNLP-S improved nursing students’ self-leadership, interpersonal relationships, clinical performance, and problem-solving abilities. The PDNLP-S can play a significant role in cultivating future nurse leaders by enhancing these nursing leadership competencies among nursing students.

Background: and Purpose We aimed to determine the proportion of Korean patients with early Alzheimer’s disease (AD) who are eligible to receive lecanemab based on the United States Appropriate Use Recommendations (US AUR), and also identify the barriers to this treatment. Methods: We retrospectively enrolled 6,132 patients with amnestic mild cognitive impairment or mild amnestic dementia at 13 hospitals from June 2023 to May 2024. Among them, 2,058 patients underwent amyloid positron emission tomography (PET) and 1,199 (58.3%) of these patients were amyloid-positive on PET. We excluded 732 patients who did not undergo brain magnetic resonance imaging between June 2023 and May 2024. Finally, 467 patients were included in the present study. Results: When applying the criteria of the US AUR, approximately 50% of patients with early AD were eligible to receive lecanemab treatment. Among the 467 included patients, 36.8% did not meet the inclusion criterion of a Mini-Mental State Examination (MMSE) score of ≥22. Conclusions Eligibility for lecanemab treatment was not restricted to Korean patients with early AD except for those with an MMSE score of ≥22. The MMSE criteria should therefore be reconsidered in areas with a higher proportion of older people, who tend to have lower levels of education.
