1.Optimization of Decoction Technology and Establishment of Quality Standard for Moringa oleifera Leaf Standard Decoction

ObjectiveBased on response surface methodology combined with principal component analysis(PCA), the optimal decocting process of Moringa oleifera leaf standard decoction was optimized, and its multi-index quality evaluation system was established, in order to provide scientific basis for the quality control of this standard decoction. MethodResponse surface methodology and PCA were used to optimize the decoction process by taking the relative peak areas of 8 characteristic peaks and dry extract yield as indexes. Based on this, the quality of 15 batches of the standard decoction was evaluated by high performance liquid chromatography(HPLC) characteristic chromatogram, determination of major components(neochlorogenic acid, L-tryptophan, cryptochlorogenic acid, vicenin-2, isoquercetin, astragalin), determination of active parts(total flavonoids, total organic acids, total polysaccharides, total α-amino acids, total sinapine), dry extract yield, specific gravity and pH. ResultThe optimal decocting process was to soak M. oleifera leaves(100.00 g) for 30 min and decoct twice with the first decoction of 12 times the amount of water for 30 min and the second decoction of 10 times the amount of water for 20 min. 2.Network Meta-Analysis of Effectiveness of First-Line Immunotherapy Treatments for Patients with Brain Metastases from Advanced Non-Small Cell Lung Cancer

Objective To conduct a network meta-analysis on the effectiveness of first-line immunotherapy on patients with brain metastases from advanced non-small cell lung cancer(NSCLC).Methods Two investigators conducted a computerized search of Pubmed,Embase,Cochrane,and other databases to screen the literature,extract the information,and assess the risk of bias of the included studies.The included clinical trials were statistically analyzed using R(4.1.3)software.For the study outcome indicators OS and PFS,the risk ratios(HRs),and the 95%confidence intervals(CIs)were extracted from the included studies and logarithmically transformed into effect analysis statistics.Results Six randomized controlled trials were finally included,including 327 patients with non-excludable NSCLC brain metastases.Network meta-analysis suggested that PD-1 inhibitor+CTLA-4 was more advantageous than the conventional chemotherapy for enhancing patients'OS(HR:0.13,95%CI:0.03-0.71),followed by PD-L1 inhibitor(HR:0.17,95%CI:0.04-0.74)and PD-1 inhibitor+chemotherapy(HR:0.36,95%CI:0.2-0.63).PD-1 inhibitor+CTLA-4 was also more advantageous(HR:0.37,95%CI:0.15-0.93)than the conventional chemotherapy for boosting patients'PFS,followed by PD-L1 inhibitor+chemotherapy(HR:0.44,95%CI:0.29-0.66)and PD-1 inhibitor(HR:0.48,95%CI:0.27-0.86).Conclusion Immune checkpoint inhibitor therapy improves the survival of patients with brain metastases from advanced NSCLC.In particular,the combination of PD-1 inhibitor and CTLA-4 inhibitor show excellent survival benefit.
3.Preliminary Study on China ’s Insurance Model for ADR Injury Compensation under MAH System

OBJECTIVE:To construct China ’s Insurance Mode for ADR injury compensation model under marketing authorization holder (MAH)system,and to provide reference for reducing the medical disputes caused by ADR damage and protecting the rights and interests of patients. METHODS :Using the methods of recommendation and “snowball”,the interviewers were invited to conduct semi-structured interviews to obtain qualitative data ,and the results were analyzed by thematic analysis. According to the results of subject analysis ,the ADR injury compensation model was designed from three aspects of insurance type setting,insurance level division and compulsory insurance protection objects ,and the Fuzzy comprehensive evaluation method based on AHP was used to evaluate the model ;questionnaire and multiple factor Logistic regression analysis was adopted to analyze the influential factors of whether drug related institutions were willing to establish ADR injury compensation system. RESULTS:A total of 40 experts were invited and 34 of them completed the interview ,with a response rate of 85.0%. At present , the problems of ADR injury compensation in China mainly included the difficulties of third-party identification and adducing evidence of ADR injury ,the difficulty of timely and fair settlement of medical disputes caused by ADR injury ,the high cost and long time of handling ADR injury disputes ,and the uncertainty of compensation mode. More than half of the experts interviewed agreed with the compensation mode of insurance . The overall score of established model was 89.50. A total of 640 questionnaires were distributed and 559 valid questionnaires were recovered (204 drug manufacturers ,172 drug distributors and 183 medical institutions). The effective rate of the questionnaire was 87.3%. Multiple Logistic regression analysis showed that the factors positively correlated with the approval of establishing ADR injury compensation system after the implementation of MAH system in China included the familiarity of ADR staff in drug manufacturing enterprises with ADR related policies (β=1.697,P<0.05),the types of drugs operated by pharmaceutical companies (β=2.987, P<0.05),the cognitionlevel of medical staff on ADR (β= 2.800,P<0.05). The working years of medical staff were negatively correlated with the approval of establishing ADR injury compensation system after the implementation of MAH system ( β =- 2.544,P<0.05). CONCLUSIONS :ADR injury compensation model designed in this study is feasible. It has reference value for reducing the economic burden of ADR patients , treating patients ’health problems caused by ADR injury in time ,and promoting the development of China ’s drug insurance. Main influential factors for the establishment of ADR injury compensation model under MAH system contain the familiarity of ADR staff in drug manufacturing enterprises with ADR related policies ,the types of drugs operated by pharmaceutical companies ,the cognitionlevel of medical staff on ADR ,the types of drugs operated by pharmaceutical companies ,the cognitionlevel of medical staff on ADR.
4. Epidemiological investigation of a family clustering of COVID-19

Objective To investigate the epidemiological characteristics of a family clustering of COVID-19. Methods Field epidemiological survey was conducted. Result Case 1 of the long-term residents from Hubei province was the source of infection of this family clustering. There were 6 cases (from case 2 to case 7) infected in the whole incubation period. The incubation period was more than 14 days for 3 of the second-generation cases. Routes of transmission include respiratory droplets (from case 1 transmitted to case 6, from case 1 to her family members) and closecontact (from case 1 to other cases in her family). All the age groups were generally susceptible, while elderly were easier to progress to critically ill. Besides respiratory symptoms, there were also gastrointestinal symptoms, of which diarrhea was the most common one. Conclusion Family clustering had been an important part for COVID-19 cases.

OBJECTIVE: To provide reference for constructing ADR damage relief system in China. METHODS: Questionnaire survey was conducted among 217 pharmaceutical manufacturers in Shaanxi province by census method. Fisher’s exact test, Pearson χ2 test and multivariate Logistic regression analysis were used to analyze the factors effecting construction of ADR damage relief systerm unter marketing authorization holder (MAH) system, and the interaction among various factors. RESULTS: A total of 217 drug manufacturers were investigated in this study (217 questionnaires were sent out). 209 questionnaires were collected and 204 valid questionnaires were screened out with recovery rate of 94.01%. Totally 9 independent influential factors screened by Fisher’s exact test and Pearson χ2 test had a significant impact on the attitude of building ADR damage relief system under MAH system, i.e. the last year’s earnings of enterprises, the proportion of pharmaceutical technicians, whether to set up R&D departments, whether to obtain new drugs/generic drugs in recent 3 years, whether to actively carry out post-marketing safety evaluation of drugs, whether ADR commissioners are familiar with ADR-related policies in MAH system, cognition about whether MAH system promotes the development of pharmaceutical industry, the development direction of enterprises after the full implementation of MAH, cognition about the lack of legal basis for ADR damage relief (P<0.05). Multivariate Logistic regression analysis showed that whether the ADR specialist was familiar with the ADR-related policies in MAH system was the main influential factor for constructing ADR damage relief system under the MAH system (P=0.044). It was positively correlated with the attitude of approving the construction of ADR damage relief system under MAH system (B=1.697). CONCLUSIONS: The pharmaceutical manufacturers, in which ADR specialists who are familiar with the ADR-related policies in the MAH system, are more inclined to agree to constructing ADR damage relief system under MAH system.

Objective To investigate CT and MRI features of hepatic sclerosed hemangioma (HSH).Methods CT and MRI findings were retrospectively reviewed in 20 cases of HSH,all of which were confirmed pathologically after hepatic surgery.Twenty patients underwent CT scan,4 patients underwent MRI.Meanwhile,the enhancement pattern and signal intensity were analyzed either.Results Twenty patients showed main part of tumor was hypo-attenuating on CT plain scanning,and 16 patients showed the central area of tumor was markedly more hypo-attenuating on CT plain scanning.After administration of intravenous contrast media,multifocal linear or small nodular enhancement in the peripheral area was seen during the arterial phase on 16 patients of HSH.Venous phases showed centripetal enhancement or spread around the nodules enhancement which continued to delayed phases with low density of no enhancement in the lesion area.Four patients showed no obvious enhancement on arterial phases and slight separation sample enhancement at the edge or inside of the lesions with a wide range of non enhancement areas on venous phase and delayed phase.Four patients were performed MRI examination,the lesions demonstrated hypointensity with a lower signal area on T1WI,hyperintensity with a higher signal intensity area on T2WI.The DWI sequence of b value were 0,150,800 s/mm2,all of which were obviously hypointensity.The edge of lesions showed small nodular enhancement on arterial phase,irregular concentric enhancement on venous phase and delayed phase,and there was no enhancement area with lower signal in the center of the lesion.Conclusions The enhancement pattern of HSH different from cavernous hemangioma,with a larger non enhancement area in the center of the lesions and similar to other hepatic masses with central scar,differential diagnosis dependence on CT and MRI dynamic enhanced scan.
8.P2X7R in periaqueductal gray matter participates in nociception modulation of chronic neuropathic pain

Objective To observe the distribution and possible mechanism of P2X7R in periaq-ueductal gray matter (PAG)in a rat model with chronic neuropathic pain in vivo.Methods The in-trathecal catheterization and sciatic nerve injury (SNI)were performed.All animals were randomly assigned into 3 groups with 26 rats in each,which was group Sham,control group (group C)and brilliant blue G (BBG)group (group BBG),respectively.Normal saline or BBG 10 μl were intrathe-cally injected after SNI and repeated for seven days.Paw-withdrawal mechanical thresholds (PWT) were measured on day 0,day 7,day 14,and day 21 after SNI.The rats were sacrificed and PAG tis-sues were collected on day 14 and day 21,separately.The distributions of P2X7R were observed by immunofluorescence.The protein contents of P2X7R and GFAP were assessed by Western blot assays.Results The P2X7R was expressed in PAG in rats.The PWTs of the control group showed a significant decrease during the 21-day period compared with the sham group.The P2X7R signals were predominantly expressed in astrocytes in PAG after SNI.Both P2X7R and GFAP expression remark-ably increased.Administration of BBG increased the PWTs,and inhibited the P2X7R and GFAP ex-pressions compared with those atthe same point of time of the control group.Conclusion These results indicated that P2X7R in PAG might participate in nociception modulation in the midbrain in chronic neuropathic pain.
9.Estimation of excess numbers of viral diarrheal cases among children aged <5 years in Beijing with adjusted Serfling regression model

Objective To estimate the excess numbers of viral diarrheal cases among children aged <5 years in Beijing from 1 January 2011 to 23 May 2015.Methods The excess numbers of diarrheal cases among the children aged <5 years were estimated by using weekly outpatient visit data from two children' s hospital in Beijing and adjusted Serfling regression model.Results The incidence peaks of viral diarrhea were during 8th-10th week and 40th-42nd week in 2011,40th-4th week in 2012,43rd-49th week in 2013 and 45th week in 2014 to 11th week in 2015 respectively.The excess numbers of viral diarrheal cases among children aged <5 years in the two children's hospital were 911(95%CI:261-1 561),1 998(95%CI:1 250-2 746),1 645 (95%CI:891-2 397),2 806(95%CI:1 938-3 674) and 1 822(95% CI:614-3 031) respectively,accounting for 40.38% (95% CI:11.57%-69.19%),44.21%(95%CI:27.66%-60.77%),45.08%(95%CI:24.42%-65.69%),60.87% (95%CI:42.04%-79.70%) and 66.62% (95%CI:22.45%-110.82%) of total outpatient visits due to diarrhea during 2011-2015,respectively.Totally,the excess number of viral diarrheal cases among children aged <5 years in Beijing was estimated to be 18 731(95%CI:10 106-27 354) from 2011 to 23 May 2015.Conclusions Winter is the season of viral diarrhea for children aged <5 years.The adjusted Serfling regression model analysis suggested that close attention should be paid to the etiologic variation of viruses causing acute gastroenteritis,especially the etiologic variation of norovirus.
10.Design and application of skin care informed consent form in critically ill patient

Objective To discuss the application of informed consent of skin care. Methods The informed consent of skin care was designed by nurses themselves. Informed consent of skin care was applied in the nursing of critically ill patients from October 2013 to August 2014. Comparisons were conducted before and after the use of informed consent of skin care. Results After the application of the informed consent of skin care, there was no complaints while the incidence of complaints was 9. 86% before the application of informed consent of skin care (P<0. 05). Family satisfaction of the skin care was higher than before the application of the informed consent of skin care (98. 4% and 87. 3%) (P <0. 05). Conclusions The application of the informed consent of skin care can improve the quality of skin care in critically ill patients, enhance the relationship between nurse and patient, increase the satisfaction of skin care.
