3.Mycological and Clinical Study on Dermatophytoses in Infants and Preschoolers.

The clinical and mycological study was made with 38 cases of dermatophytoses in infants and preschoolers in dermatologic clinic of St. Pauls Hospital, Catholic Medical College, from September, 1982 to April, 1984. Following results are obtained: The incidence of dermatophytoses under the age 6 was 2. 2% among 1, 711 out- patients of the sarne age group. 2. The incidence of involving sites was the highest in tinea(T.) corporis(37. 2%) and T. faciale(37, 2%) and then T. capitis(18. 7g). In 0-under 2 groups, the highest incidence in T. corporis(54. 5%) and then T. faciale(3l. 8%.). In 4-under 5 group, the highest incidence in T. capitis(55. 6%). 3 Fungi cultured from their skin lesions were 36 strains belonging to 6 species. These were M. canis(55. 6%), T. rabrum(27. 8%), granular type of T.mentagrophyte(8.3%) and interdigital type of T. mentagrophyte(27. 8%), M gypseum (2.8%). and E. floccosum(2.8%) in order. 4. Arnong 1Q strains of T. rabrum, anthropophilic fungi, 8 strains(80. 0%) were frorn 0-under 2 age groups, E. floccosum, and interdigital type of T. mentagrophyte were also from 0-under 1 age group. Among ZO strains of M, canis, zoophilic fungi, 13 strains(65. 0%) were from the age groups of 2-under 5. Granular type of T. mentagrophytc, 2 among 3 strains were from 4-under 5 age group, 5. The incidence of causative fungi in each sites were as follows: In T. corporis, T. rubrum(50. O%), and M canis(18. 8%) in order. In T. faciale, M. canis(62. 5%), T. rabrum(12. 5%), T. mentagrophyte(12. 5%), and M. gypseum(6. 3%) in order. In T. capitis, all cultured fungi were M. canis

4.A Case of Epidermolytic Keratosis Palmaris et Plantaris.

Epidermolytic keratosis palmaris is a rare disease which shows clinical findings of Unna Thost keratoderma and histopathologic of epidermolytic hyperkeratosis. We report herein a case of epidermolytic keratosis palmaris et plantaris in a 16-month-old female baby. Light microscopy shows marked hyperkeratosis, large irregular keratohyalin granules, and large clear spaces in the granular and upper spinous layers. Eletron microscopic findings shows that the clear spaces are areas of cytoplasm filled with a fibrillar material and cellular organelles. Abnormal clumping of tonofilament and keratohyalin is also present.

5.Two Cases of Solitary Leiomyoma Cutis.

Leiomyoma Cutis is an unusual benign tumor of smooth muscle derivecl either from the arrector pili muscle, from the media of blood vesseles, or from smooth muscles of scrotum, labia or nipple. Two casea of leiomyoma cutis confirmed by H&E stain and Massons stain is presented, one with lesion on the left thigh and the ather with lesion of the left ankle. The first case was a 49 year-old female with solitary pinhead sized subcutaneous firm mass with intermittent pain and tenderess for 10 months, The second case waa a 36 year-old female with painful solitary peanut sized firm nodule which had pinkish center of 1 year duration.

6.A Case of Piezogenic Pedal Papules with Family History.

We present herein a case of painless piezogenic pedal papules occurring on the both heels of 25-year-old female patient, Histopathologic findings showed normal adipose tissue surrounded by loose fibrous tissue in the nid-dermis and thickening of venous wall. Hyalinized thick collagen bundles were distributed below the herniated adipose tissue in the nodular pattern. Her parents, one brother and sister had the sarne clinical features.

7.Two Case of Sudoriparous Angioma.

Sudoriparous angioma is an unusual form of angioma with pain and hyperhidrosis. We present two cases of typical sudoriparous angioma. Case 1: 9-year-old female showed 0. 8 to 2. 0cm multiple dark brownish tender palpable hypertrichotic masses on the right lower leg after birth. Muscular atrophy and flexion contracture was also developed on the right lower leg for 2 years. Case 2: 4-year-old rnale showed 3 * 3cm tender elevated mass on the left shin after birth. Marked hyperhidrosis was developed when even slight pressure applied on the lesion in these cases. On histologic examination, these two cases showed proliferation of varible sized blood veasels hyperplasia and dilated hyperplastic eccrine glandular and ectopic adipose tissue on the mid and deep dermis. Muscular biopsy from case 1 showed fibrotic and degenerative change. Electromicroscopic examination done from case 1. The secretory epithelial cell of eccrine gland showed cytoplastic degradation and slight dilated intercellular canaliculi (Fig. 11). The lumen of subepithelial capillaries were dilated and some of endothelial cells showed degenerative changes evidenced by RER dilation, mitochondrial swelling and detachment from underlying tissue(Fig. l2). The cutaneous nerve showed many non-myelinated axons enclosed in cytoplasm of Schwann cells.

8.Unilateral Frontal Hyperhidrosis.

Unilaterl frontal hyperhidrosis, a rare disorder of the sweat gland with localized hyperhidrosis, ia discribed. A 13-year-old female patient had increased sweating on the left half of the forehead, recurring in summer season for 2 years. Quantitative sweat collection demonstrated that the sweat glands responsive to those stimuli, which were thermal (direct and indirect heat), gustatory and cholinergic stimulation. A marked increase in sweat gland activity was noted on the affected areas as compared to the contaralateral normal sites, in thermal and gustatory effect. But, carbachol stimulation was not responsive. Biosy specimen which taken from the positive site of starch iodine technique showed increase number of sweat glands and ducts with somewhat dilated sweat ducts.

10.Facial Cutaneous Mycosis by Paecilomyces lilacinus.

A 19-year-old man noticed a very slowly spreading erythematous macular lesion on his cheek when he was 5 years old. This asymptomatic and well circumscribed patch was covered with fine scales. He sought first medical attention at the age of 12 years and diagnosis was cutaneous tuberculosis was made by skin biopsy. Seven years after the second biopsy was made and it revealed a few spores within giant cells in the upper dermis. By mycologic study, it was identified as Paecilomyces lilacinus. The patient was put on the treatment of topical and oral antifungal agents. The skin lesion showed signs of improvement after 12 weeks of treatment.
