Purpose: Anaphylaxis is a life-threatening condition that requires prompt recognition and treatment. Particularly in children, anaphylaxis often occurs in the child care facilities, making the role of teachers crucial. The aim of this study is to evaluate the extent of improvement in anaphylaxis awareness among child care facilities teachers both online and offline education programs. Methods: On June 22 and October 18–19, 2022, a total of 387 teachers from child care facilities in Busan participated. Among them, 271 individuals received education and completed surveys online in June, and 116 participated offline in October. We administered 9 items of questionnaire survey on knowledge, and management skills for anaphylaxis before and after the educational sessions were provided by an allergy specialist. Results: The overall correct answer rates for awareness were improved from 57.1% before to 67.3% after education. Awareness of anaphylaxis symptoms was the lowest (10.1%) before education, it has improved after education, but it remained the lowest (22.5%).Awareness of self-injectable epinephrine was significantly improved from 79.8% to 93.8%, and awareness of the injection site increased from 55.8% to 86.8%. Based on the education methods, the awareness improvement rate was 5% (56.6%→61.5%) for online and 14% (57.5%→73.0%) for offline (P < 0.01). Conclusion The correct awareness of anaphylaxis is important, so repetitive, systematic and continuous education is necessary to improve and promote. Additionally, the results suggest that an educational method combining practice and feedback in offline services may be more effective than online methods in enhancing awareness of anaphylaxis.

Objective: To evaluate the effectiveness of a squatting posture grading system established to screen for limited ankle dorsiflexion. Methods: The squat posture grading system categorizes subjects’ squat posture into three grades. Grade 1 is defined as being able to maintain a squatting posture with heels on the ground in full ankle dorsiflexion without effort. Grade 2 is defined as being able to perform the same position, but unable to maintain the position for more than 5 seconds or requiring trunk and leg muscle efforts to maintain the position. Grade 3 is defined as being unable to maintain the same position and falling backwards immediately if attempted to touch the ground with heels. Next, subjects’ ankle dorsiflexion angles were directly measured in knee flexed and extended position by goniometer. Results: Out of the 92 total subjects, 35 were in grade 1, 18 were in grade 2, and 39 were in grade 3. The average ankle dorsiflexion angle with knee flexed position were 23.13° for grade 1, 16.03° for grade 2, and 9.31° for grade 3. The average ankle dorsiflexion angle with knee extended position were 15.16° for grade 1, 7.92° for grade 2, and 3.40° for grade 3. Ankle dorsiflexion angles showed a significant decrease from grade 1 to 3 (p<0.05). Conclusion The squatting posture grading system defined in this study effectively graded the subjects based on the difference in their average ankle dorsiflexion angle. This system could be used as a quick screening method for limited ankle dorsiflexion.

Purpose: This study aimed to analyze nationwide trends and regional disparities in inguinal hernia surgeries in South Korea between 2016 and 2022. Additionally, we aimed to evaluate changes in surgery frequency, including urban concentration and the introduction of robotic surgery. Methods: This retrospective review used nationwide data on inguinal hernia surgeries from the Health Insurance Review and Assessment Service database. Results: From 2016 to 2022, 254,367 inguinal hernia surgeries were performed in South Korea, with males accounting for 88.9% of cases. The annual number of surgeries fluctuated, particularly in 2020, owing to the coronavirus disease 2019 pandemic. Medical costs increased from $1,218.4 to $1,970 on average, whereas patient copayments rose from $180.2 to $293.3. Robotic inguinal hernia surgeries, introduced in 2019, increased to 226 cases in 2022. Pediatric surgeries steadily declined, whereas adult surgeries remained stable, with a slight increase in 2022. The average hospital stay did not change significantly but varied between pediatric and adult patients. Regional disparities were notable, especially in pediatric surgery rates between metropolitan areas, such as Seoul and the surrounding provinces. Conclusion This study highlights stable overall surgery rates, a decline in pediatric cases, and an increase in robotic inguinal hernia surgeries. The persistent concentration of healthcare services in metropolitan areas suggests a need for policy interventions to address regional disparities and ensure equitable healthcare access. The findings underscore the importance of ongoing efforts to improve healthcare distribution and the need for long-term strategies to address changing surgical trends.

Axial spondyloarthritis (axSpA) is a chronic inflammatory disorder affecting the sacroiliac joints and axial spine. Along with pharmacotherapy, non-pharmacological interventions for axSpA are crucial and constitute the cornerstone of treatment. Here, we review the evidence for non-pharmacological treatment of axSpA as a basis for the 2023 Korean treatment recommendations for patients with axSpA. The effectiveness of the core non-pharmacological approaches, such as education, smoking cessation, and exercise, has been reaffirmed. High-quality research on surgical treatment is limited. However, total hip replacement is advised in patients with ongoing pain or disability and visible structural damage to the hip on imaging. Urgent spinal intervention should be considered in cases of acute spinal pain with neurological deficiency or concurrent unstable fractures. Evidence for complementary therapies, including spas and acupuncture, remains insufficient.

Methods: This randomized open-label crossover trial included 30 patients receiving step 3 inhaled medication according to the Global Initiative for Asthma (GINA). Patients received beclomethasone/formoterol (BDP/F) for maintenance and reliever therapy for 6 weeks, followed by budesonide/formoterol (BUD/F) for 6 weeks, or vice versa, with a 4-week washout period in between. Assessments at each visit included the Asthma Control Test (ACT), Asthma Control Questionnaire, Quality of Life Questionnaire for Adult Korean Asthmatics, and pulmonary function test. The primary endpoint was the change in forced expiratory flow between 25% and 75% of vital capacity (FEF25–75% pred). Results: Twenty-four patients (15 females, mean age 39.3 years) completed the study. The changes in FEF25–75% pred were comparable between BDP/F and BUD/F (5.79 ± 38.34 vs. -1.36 ± 14.93, p = 0.399). No significant differences were observed between the BDP/F and BUD/F groups in terms of improvement in asthma control or quality of life. However, in the subgroup of patients with positive methacholine bronchial provocation tests, BDP/F significantly improved ACT scores compared to BUD/F (0.92 ± 2.25 vs. -1.31 ± 3.04, p = 0.044). Conclusions Our study demonstrated that extrafine ICS treatment provided no significant advantage over non-extrafine ICS in improving small airway obstruction or overall asthma control in moderate asthma. This suggests that factors other than particle size may contribute to treatment outcomes.

Purpose: This study evaluated the postoperative quality of life (QoL) after various types of gastrectomy for gastric cancer. Materials and Methods: A multicenter prospective observational study was conducted in Korea using the Korean Quality of Life in Stomach Cancer Patients Study (KOQUSS)-40, a new QoL assessment tool focusing on postgastrectomy syndrome. Overall, 496 patients with gastric cancer were enrolled, and QoL was assessed at 5 time points: preoperatively and at 1, 3, 6, and 12 months after surgery. Results: Distal gastrectomy (DG) and pylorus-preserving gastrectomy (PPG) showed significantly better outcomes than total gastrectomy (TG) and proximal gastrectomy (PG) with regard to total score, indigestion, and dysphagia. DG, PPG, and TG also showed significantly better outcomes than PG in terms of dumping syndrome and worry about cancer. Postoperative QoL did not differ significantly according to anastomosis type in DG, except for Billroth I anastomosis, which achieved better bowel habit change scores than the others. No domains differed significantly when comparing double tract reconstruction and esophagogastrostomy after PG. The total QoL score correlated significantly with postoperative body weight loss (more than 10%) and extent of resection (P<0.05 for both).Reflux as assessed by KOQUSS-40 did not correlate significantly with reflux observed on gastroscopy 1 year postoperatively (P=0.064). Conclusions Our prospective observation using KOQUSS-40 revealed that DG and PPG lead to better QoL than TG and PG. Further study is needed to compare postoperative QoL according to anastomosis type in DG and PG.
