Journal of Biomedical Engineering 2025;42(5):1092-1098
Pan-vascular diseases encompass a range of systemic conditions characterized by sharing a common pathologic basis of vascular deterioration. Due to the complexity of these diseases, a thorough understanding on their similarities and differences is essential for optimizing diagnosis and treatment strategies. Magnetic resonance imaging (MRI), as one of the commonly used medical imaging techniques, has been widely applied in the diagnosis of pan-vascular diseases. Particularly, the integration of MRI with contrast agents and multi-parameter imaging techniques significantly enhances diagnostic accuracy, reducing the likelihood of missed or incorrect diagnoses. Recently, a variety of nano-magnetic resonance contrast agents have been developed and applied to the magnetic resonance imaging diagnosis of diseases. These nanotechnology-based contrast agents provide multiple advantages, ensuring more precise and forward-looking imaging of pan-vascular conditions. In this review, the diverse application strategies of nanomaterials-enhanced MRI techniques in the diagnosis of pan-vascular diseases were systematically summarized, by classifying them into the commonly used MRI sequences in clinical practice. Additionally, the potential advantages and challenges associated with the clinical translation of nanomaterial-enhanced MRI were also discussed. This review not only offers a novel perspective on the precise diagnosis of pan-vascular diseases, but also serves as a valuable reference for future clinical practice and research in the field.

Journal of Central South University(Medical Sciences) 2025;50(1):61-71
OBJECTIVES:
According to the World Health Organization (WHO), infertility rates have been steadily rising worldwide. Identifying risk factors for contrast agent reflux into the myometrium during hysterosalpingo-contrast sonography (HyCoSy) is of clinical significance in reducing this complication and improving infertility treatment. However, there is currently no standardized pre-evaluation method for predicting intra-myometrial contrast reflux, with clinical assessment often relying on physician experience and patient symptoms. This study aims to identify imaging risk factors for contrast agent reflux into the myometrium using four-dimensional (4D) HyCoSy and to construct a nomogram-based predictive model to assist in clinical decision-making.
METHODS:
A retrospective analysis was conducted on 1 274 infertile women who underwent 4D HyCoSy at the Women and Children's Hospital of Hunan and the the Third Xiangya Hospital of Central South University from January 1, 2020, to December 15, 2022. Patients were divided into a reflux group (n=234) and a non-reflux group (n=1 040) based on the presence of intra-myometrial contrast reflux. Univariate and multivariable Logistic regression analyses were used to identify significant predictors, which were then used to construct a nomogram model. Internal validation was performed using 500 bootstrap resamples.
RESULTS:
The age of the reflux group was significantly higher than that of the non-reflux group [(31.82±5.27) years vs (30.66±4.83) years, P=0.001 1]. Primary infertility was more common in the non-reflux group (50.96%), while secondary infertility dominated in the reflux group (76.50%), with 72.65% having a history of gynecological surgery (P<0.001). Abnormal menstrual volume and discomfort during the procedure were more common in the reflux group, while the non-reflux group tolerated higher contrast agent doses (P<0.001). Imaging differences included endometrial thickness, tubal wall smoothness, and peritoneal contrast dispersion, with the non-reflux group showing thicker endometrium and smoother, more patent tubes. The nomogram model yielded an area under the curve (AUC) of 0.854, indicating good predictive performance. The AUC of the decision curve analysis (DCA) for internal validation of the model was 0.737. When the threshold probability for contrast agent reflux into the myometrium ranged from 0.05 to 0.95, the maximum net benefit reached 0.18. The net benefit of applying the nomogram predictive model exceeded that of either full intervention or no intervention, indicating that the model demonstrates good clinical predictive performance.
CONCLUSIONS
The nomogram model, based on infertility type, endometrial thickness, contrast agent dose, and discomfort symptoms, effectively predicts intra-myometrial contrast agent reflux after 4D HyCoSy. It provides a valuable tool for clinicians to implement early preventive measures and reduce the risk of contrast leakage and associated complications.

Journal of Central South University(Medical Sciences) 2025;50(2):301-312
Exosomes are small vesicles commonly found in bodily fluids such as blood, urine, ascites, and breast milk. As essential mediators of intercellular communication, exosomes play pivotal roles in physiological and pathological processes including material transport, signal transduction, homeostasis regulation, immune response, and angiogenesis. They are promising biomarkers for disease diagnosis and key carriers for therapeutic drug delivery. Longitudinal tracking of exosome biodistribution, elucidating their migratory routes and homing effects, determining optimal delivery routes and therapeutic dosages, and evaluating implantation in target tissues are crucial for better understanding their mechanisms of action and guiding clinical applications. Magnetic resonance imaging (MRI), as a non-invasive and repeatable imaging technique, offers an ideal approach for exosome tracking. By labeling exosomes with specific contrast agents or tracers and detecting them via MRI, researchers can trace exosomes in vivo. This facilitates advancements in exosome-based nanomedicine and accelerates the clinical translation of exosome diagnostics and therapeutics.

Journal of Clinical Otorhinolaryngology Head and Neck Surgery 2025;39(9):848-853
Objective:To explore the value of spectral CT parameters combined with dynamic contrast enhanced magnetic resonance imaging(DCE-MRI) parameters in the short-term efficacy of concurrent chemoradiotherapy for nasopharyngeal carcinoma. Methods: A total of 110 cases with nasopharyngeal carcinoma Ⅲ-Ⅳ staging who received synchronous radiotherapy and chemotherapy at our Hospital from October 2022 to October 2024 were regarded as the study subjects. Complying with the evaluation results after radiotherapy and chemotherapy, they were divided into a complete remission(CR) group of 53 cases and a non CR group of 57 cases. All patients underwent DCE-MRI and energy dispersive CT scans to obtain parameters, such as iodine concentration(IC), volume transfer constant(Ktrans), slope of spectral HU curve(λHU), rate constant(Kep), and normalized iodine concentration(NIC). Logistic regression analysis was used to screen for influencing factors. ROC curve was used to analyze the evaluation value of various parameters. In addition, Z-test was used to compare area under the curve(AUC). Results:The proportion of retropharyngeal lymph node metastasis and λHUvalue in the non CR group were higher than those in the CR group, while Ktrans, Kep, IC value, and NIC value were lower than those in the CR group(P<0.05). Retropharyngeal lymph node metastasis, Ktrans, Kep, IC value, λHUvalue, and NIC value were all influencing factors(P<0.05). The AUC of individual prediction of Ktrans, Kep, IC value, λHUvalue, and NIC value was 0.817, 0.800, 0.785, 0.783, and 0.835, respectively. The AUC of the combination of DCE-MRI parameters, the combination of spectral CT parameters, and the combination of the five parameters were 0.874, 0.900, and 0.980, respectively, the AUC of the combination of the five parameters was significantly higher than the AUC of each indicator alone, the AUC of the combination of DCE-MRI parameters, and the AUC of the combination of spectral CT parameters(P<0.05). Conclusion:The DCE-MRI, and spectral CT parameters (Ktrans, Kep, IC value, λHUvalue, and NIC value)can be used to evaluate concurrent radiotherapy and chemotherapy short-term efficacy for nasopharyngeal carcinoma. And the combination of various parameters can greatly improve the predictive value of efficacy, which has important clinical application value.

9.Application Value of Gastric Contrast-Enhanced Ultrasound in the Diagnosis of Extraluminal Duodenal Diverticulum.
Acta Academiae Medicinae Sinicae 2025;47(5):733-737
Objective To evaluate the value of gastric contrast-enhanced ultrasound in diagnosing extraluminal duodenal diverticulum. Methods A retrospective analysis was conducted on 40 cases of extraluminal duodenal diverticula confirmed by upper gastrointestinal barium meal and gastroduodenoscopy, all of whom underwent gastric contrast-enhanced ultrasound examination. The detection rate and imaging features of duodenal diverticula were retrospectively analyzed. Results Gastric contrast-enhanced ultrasound detected 34 out of 40 cases of extraluminal duodenal diverticula, with the detection rate (85.0%) higher than that (10.0%,4/40) of conventional fasting ultrasound (P<0.001).Gastric contrast-enhanced ultrasound demonstrated higher detection rate for single diverticula than for multiple diverticula (97.1% vs.44.4%,P<0.001) and was particularly effective for duodenal diverticula in the descending segment (91.7%).According to imaging features, duodenal diverticula were classified into sac-like,gas-containing mass, and air-line types. Conclusion Gastric contrast-enhanced ultrasound facilitates the detection of extraluminal duodenal diverticula in the descending segment, allows concurrent visualization of biliary-pancreatic system pathologies, and enables comprehensive assessment of the association between duodenal diverticula and biliary-pancreatic diseases, demonstrating significant clinical value.

Acta Academiae Medicinae Sinicae 2025;47(5):738-743
Objective To evaluate the diagnostic value of transrectal contrast-enhanced ultrasound (CEUS) for rectal cancer with intestinal stenosis caused by tumors. Methods Forty-nine patients with rectal cancer underwent transrectal CEUS and magnetic resonance imaging (MRI) before surgery.Intraoperative tumor localization and postoperative pathological results were taken as the gold standard for diagnosis. The differences in T stage, localization, and tumor length of rectal cancer were compared between the two methods. Results The total accuracy rates of transrectal CEUS and MRI in diagnosing T stage were 75.5% (36/49) and 67.3% (33/49),which had no significant difference (χ2=0.8,P=0.371).The total accuracy rates of transrectal CEUS and MRI in judging tumor localization were 79.5% (39/49) and 77.5% (38/49),which had no significant difference (χ2=0.061,P=0.806).The measurement results of tumor length in pathological examination had no significant difference from the transrectal CEUS results (t=1.42,P=0.162) but a significant difference from the MRI results (t=3.38,P=0.001).Furthermore,transrectal CEUS detected 8 (16.3%) cases of colonic polyps among the 49 patients, while MRI did not detect colon lesions. Conclusions Transrectal CEUS has good consistency with MRI in T staging and localization judgement of rectal cancer with intestinal stenosis, and this method can more accurately evaluate the tumor length and simultaneously evaluate whether there is a lesion in the entire colon at the proximal end of stenosis. It can be used as a supplementary examination before rectal cancer treatment in clinical practice.
