1.Correlation between frequency of follow up and death under SDTM mode in advanced age patients with type 2 diabetes mellitus

Objective To conduct the standardized management on the advanced age(≥75 years old)outpatients with type 2 diabetes mellitus(T2DM)by adopting the staged diabetes targeted management(SDTM)mode,and to observe the correlation between the follow up frequency and death.Methods The ad-vanced age patients with T2DM participating in the SDTM mode in the diabetes clinic of a tertiary hospital in Jiangsu Province from September 2009 to September 2022 were retrospectively selected and divided into the survival and death groups according to whether or not the patient died.The correlation between the follow up frequency and death was analyzed.Results A total of 947 advanced age patients with T2DM participated in the follow up,the mean age was(82.1±4.7)years old,and the mean follow-up time was(7.5±3.2)years.A total of 169 cases died with a death rate of 17.8%.The Cox regression analysis showed that the annual average follow up frequency had the correlation with death;after correcting the factors such as age and gender,the an-nual average follow-up frequency of the patients was correlated with death(HR=0.880,95%CI:0.854-0.907,P<0.001).Conclusion Using the SDTM mode to conduct the standardized management on the advanced age patients with T2DM and increasing the follow-up frequency could reduce the death rate of the patients and im-prove their survival rate.
2.Analysis on mtDNA variation rate of elderly people at different age groups in a"regional longevity"gathering area in Chongqing

Objective To study the differences in living habit and blood biochemical indicators of male elderly people at different age groups in a"regional longevity"gathering area in Chongqing and the high fre-quency variation of mitochondrial DNA(mtDNA)in the elderly people(≥90 years old).Methods The male elderly people aged ≥60 years old in a"regional longevity"gathering area in Chongqing were randomly select-ed as the study subjects and divided into the group A(60-<70 years old),group B(70-<80 years old),group C(80-<90 years old)and group D(≥90 years old).The differences of vegetable and fruit consump-tion,dairy bean consumption,outdoor activities,sleep time and blood biochemical indexes were compared a-mong different age groups.Then peripheral blood in the above subjects was collected for extracting mtDNA and sequencing.The meaningful mtDNA mutations between the various groups and group D were found out by the bioinformatics analysis.Results There were no statistically significant differences in consumption of vegetables and fruits,consumption of dairy beans,outdoor activities and sleep duration among different age groups(P>0.05).There were statistically significant differences in erythrocyte and creatinine of blood bio-chemical indexes among the partial groups(P<0.05).The mitochondria mutation analysis found 4 105 mtD-NA mutation sites,mainly single base mutation,the most common mutation was T>C(22.57%),followed by C>T(19.82%)and A>G(19.36%).The average mutation rate of mtDNA in the group D was the high-est(8.392%),in which 3 mtDNA variants were found to have significant differences between different age groups and group D(P<0.1),including mtDNA5396-5411CAACGTAAA>C,mtDNA5414A>T and mtD-NA5417G>T.Conclusion There was no significant difference in the living habits of male elderly people of different ages.The mutation rate of mtDNA in the advanced age group was the highest.mtDNA5396-5411CAACGTAAA>C,mtDNA5414A>T and mtDNA5417G>T variations may be related to"regional lon-gevity".
3.Analysis of MMPI results in patients with anxious depression or non-anxious depression

Objective To investigate the differences in personality traits between the patients with anx-ious depression(AND)and non-anxious depression(NAD)in order to provide the possible basis for early find of the patients with AND.Methods A total of 572 adult patients with depression visiting in the psychiatric outpatient department of the First Affiliated Hospital of Chongqing Medical University from January 2022 to December 2022 were selected to conduct the questionnaire survey.General demographic questionnaire,Self-rating Depression Scale(SDS),Generalized Anxiety Disorder Scale(GAD-7)and Minnesota Multiphasic Per-sonality Inventory(MMPI)were collected and analyzed.The patients with GAD-7 total score ≥5 points served as the AND group(n=499)and those with GAD total score<5 points served as the NAD group(n=73).The correlation between the general demographic questionnaire,SDS and GAD-7 with MMPI was ana-lyzed.Results There were statistically significant differences in the place of residence,number of children in a family,education years,MMPI total score and high score proportions of psychopathy,athopia,hysteria,depres-sion,hypochondriasis,paranoea,schizophrenia,social introversion and hypomania dimensions between the two groups(P<0.05).The SDS and GAD-7 scores in the AND group were higher than those in the NAD group(P<0.05).The MMPI total score,athopia,hysteria,depression,hypochondriasis,paranoea,schizophrenia,so-cial introversion and hypomania were positively correlated with SDS and GAD-7(P<0.05).Conclusion The patients with depression accompanied by anxiety symptom could be early identified by the MMPI testing results.
4.Construction of hypertension structured database based on Yi-9B big language model

Objective To construct a hypertension structured database based on Yi-9B large language model by aiming at the large amount of unstructured data generated in the process of hypertension diagnosis and treatment in order to elevate the efficiency of data management and provide the support for clinical deci-sion-making.Methods The key clinical informations of 114 369 patients with hypertension visiting in the Sec-ond Affiliated Hospital of Army Medical University during 2014-2023 were extracted.The Yi-9B large lan-guage model was used for conducting the entity identification and data structuring,and the database architec-ture was designed for statistical analysis and clinical application.Results After the database structuring process,the mean values of systolic and diastolic blood pressure were(149.98±20.55)mmHg and(86.90±13.75)mmHg,respectively.According to the classification of blood pressure level,the proportions of the nor-mal high value for high risk,very high risk of hypertension grade 1,and very high risk of hypertension grade 2 were the highest,which accounted for 20.73%,27.80%and 19.59%respectively.52.64%of the patients were complicated with heart disease,10.18%with complicating diabetes and 12.71%with complicating hy-perlipidemia.Logistic regression analysis showed that>50-60 and>60-70 years old was the high incidence age segment,moreover the systolic blood pressure showed an increasing trend with the age increase,reflecting the universality of hypertension in aging.This database significantly improved the efficiency of diagnosis and treatment in clinical application and realized the efficient analysis and management of data.Conclusion The hyper-tension structured database based on Yi-9B large language model effectively processes the unstructured data,significantly improves the efficiency of data extraction and management,helps to optimize the diagnosis and treatment decision-making,improves the management efficiency and provides the support for intelligent man-agement and personalized diagnosis and treatment.
5.Study on causal relationship of type 2 diabetes mellitus and lung function by two-sample bidirectional Mendelian randomization

Objective To study the causal relationship between type 2 diabetes mellitus(T2DM)and lung function by using Mendelian randomization(MR).Methods Single nucleotide polymorphism sites(SNP)correlated with T2DM,FVC and FEV1 were extracted from the published genome-wide association study(GWAS)database as the instrumental variables.The random effect inverse-variance weighting method(IVW)was used as the primary method for analyzing the bidirectionary causal relationship between T2DM with FVC and FEV1.Meanwhile,the MR-Egger,weighted median method(WM),weighted mode,simple mode and MR-PRESSO method were used to verify and correct the statistical results.Results The MR analy-sis showed that T2DM had the causal relationship with FVC(OR=0.97,95%CI:0.94-0.99,P=0.01)and FEV1(OR=0.95,95%CI:0.91-0.98,P=0.01).There was a causal relationship between FVC and T2DM(OR=0.69,95%CI:0.52-0.92,P=0.01),but no causal relationship existed between FEV1 and T2DM(OR=0.92,95%CI:0.70-1.19,P=0.52).Conclusion T2DM has a certain causal relationship with the lung function.
6.Efficacy of PD-1 antibody combined with chemoradiotherapy in early high-risk extranodal nasal natural killer/T-cell lymphoma

Objective To explore the short-term efficacy and safety of programmed death receptor-1(PD-1)antibody combined with chemoradiotherapy in early high-risk nasal extranodal natural killer/T cell lymphoma(ENKTCL,Nomogram modified risk index ≥1).Methods This study was a prospective phase Ⅱclinical study.A total of 20 patients with stage Ⅰ/Ⅱ nasal ENKTCL initially treated in the Affiliated Hospi-tal of Guizhou Medical University from June 2021 to December 2023 were selected as the study subjects.They were given 3 cycles of PD-1 antibody+P-Gemox(pegaspargase+gemcitabine+oxaliplatin)regimen induced immunochemotherapy and 3 cycles of PD-1 antibody concurrent immunoradiotherapy,14 d/cycle.The com-plete response(CR)rate after 3 cycles of induced immunochemotherapy,CR rate after treatment,objective re-sponse rate(ORR)after 3 cycles of induced immunochemotherapy and ORR after comprehensive treatment,occurrence of adverse reactions and plasma Epstein-Barr virus DNA copy number were observed.Results Af-ter 3 cycles of induction immunochemotherapy,the CR rate was 35.0%(7/20)and ORR was 90.0%(18/20).After the concurrent immunoradiotherapy,the CR rate was 95.0%(19/20)and ORR was 100.0%(20/20).The adverse reactions were mainly the grade 1/2.the immune-related adverse reactions included 9 ca-ses of grade 1/2 hypothyroidism(45.0%),4 cases of grade 1 capillary vessel hyperplasia(20.0%),1 case(5.0%)of grade 2 infusion reaction,and no immune myocarditis occurred.All 7 cases of positive EBV DNA turned to negative after 3 cycles of induced immunochemotherapy.Conclusion PD-1 antibody combined with P-Gemox in the radiotherapy of early high-risk ENKTCL has a high short-term remission rate with good tol-erance for treatment.
7.Efficacy and safety of standard pulsed radiofrequency for treating rare-site glomus tumors

Objective To explore the efficacy and safety of standard pulsed radiofrequency for treating rare-site glomus tumors.Methods A prospective self before-and-after control study was conducted and 6 pa-tients diagnosed as glomus tumors in the pain department of Yongkang Municipal First People's Hospital from June 2022 to January 2023 served as the study subjects.Among them,1 case was located in the outer beam of deltoid muscle in the upper arm,and 5 cases were located under the nail beds of the hands/feet.All subjects underwent the standard pulsed radiofrequency treatment under ultrasound guidance and were fol-lowed up for at least 12 months.The changes in the preoperative and postoperative Numerical Rating Scale(NRS)scores were observed,and the changes in the"typical triad",healing degree of the nail growth matrix and recurrence of pain were recorded.Results The postoperative follow-up results showed that all patients had significant decreases in NRS scores,indicating that the pain degree obtained the effective control.Among them,4 patients did not experience the pain recurrence after adopting the standard pulsed radiofrequency;1 pa-tient had pain recurrence in postoperative 6 months and the tumor body was ultimately removed by surgery.Apart from the patient 2 developing transient subungual congestion,no significant adverse events were observed.Conclusion The standard pulsed radiofrequency treatment is a safe and effective therapeutic method for pain caused by rare-site glomus tumors.
8.Analysis on related factors of Helicobacter pylori infection and upper gastrointestinal bleeding in patients with acute myocardial infarction

Objective To explore the related factors between Helicobacter pylori(Hp)infection and upper gastrointestinal bleeding in the patients with acute myocardial infarction.Methods A total of 180 pa-tients with acute myocardial infarction hospitalized in the department of cardiovascular medicine of Shaanxi Provincial People's Hospital from May 2021 to May 2023 were selected as the study subjects.The general condition of the patients was recorded,the relevant blood biochemical indexes of the patient were recorded,the 3C urea breath test was used to detect Hp,and the expression levels of VEGF,VCAM-1 and CTRP9 were de-termined by ELISA.All study subjects were followed up in 3 months after discharge,and the major adverse cardiac events(MACE)and upper gastrointestinal bleeding events during the follow-up period were recorded.Results Among the 180 study subjects,78 cases(43.33%)were Hp-negative and 102 cases(56.67%)were Hp-positive.The Hb level in Hp-positive patients was lower than that in Hp-negative patients,the levels of IL-6,IL-8,TNF-α,and C-reactive protein were higher than those in Hp-negative patients,and the differences were statistically significant(P<0.05).The levels of VEGF and VCAM-1 in Hp-positive patients were high-er than those in Hp-negative patients,the level of CTRP9 was lower than that in Hp-negative patients,and the differences were statistically significant(P<0.05).The results of logistic regression analysis showed that Hb,IL-6,IL-8,TNF-α and C-reactive proteins were the independent influencing factors in predicting Hp infec-tion(P<0.05).The incidence rate of upper gastrointestinal bleeding events in Hp-positive patients was high-er than that in Hp-negative patients,and the difference was statistically significant(P<0.05).Hp positivity,Hb and C-reactive protein were the independent influencing factors in predicting the occurrence of upper gas-trointestinal bleeding(P<0.05).Conclusion The incidence rate of Hp infection in the patients with acute myocardial infarction is high,and the Hp positivity could promote the changes in the levels of various cyto-kines and further aggravate coronary artery injury.Hp infection may increase the incidence rate of upper gas-trointestinal bleeding in the patients with acute myocardial infarction.
9.Predictive value of PCT,NLR and CAR in short-term prognosis in patients with severe community acquired pneumonia

Objective To investigate the application value of procalcitonin(PCT),neutrophil-to-lym-phocyte ratio(NLR)and C-reactive protein to albumin ratio(CAR)in the short-term prognosis of the pa-tients with severe community acquired pneumonia(SCAP).Methods A total of 225 patients with communi-ty-acquired pneumonia(CAP)treated in this hospital from July 2021 to December 2022 were selected as the study subjects and divided into the SCAP group and non-SCAP group.Then the patients with SCAP were di-vided into the survival group and death group according to their survival state in 28 d.The general data and re-lated laboratory indexes were collected.NLR and CAR were calculated.The binary logistic regression was a-dopted to analyze the risk factors of death on 28 d for the SCAP patients.The receiver operating characteristic(ROC)curve was drawn and the area under the curve(AUC)was calculated to evaluate the predictive value of relevant indicators.Results The age,levels of PCT,NLR,CAR,WBC and NEU,and the proportion of cor-onary heart disease in the SCAP group were higher than those in the non-SCAP group,while the levels of LYM and ALB were lower than those in the non-SCAP group,and the differences were statistically significant(P<0.05).The age,levels of PCT,NLR,CAR,WBC and NEU,and the proportion of COPD in the death group were higher than those in the survival group,while the levels of LYM and ALB were lower than those in the survival group,and the differences were statistically significant(P<0.05).The binary logistic regres-sion analysis showed that the age(OR=1.069,95%CI:1.020-1.120),chronic obstructive pulmonary dis-ease(OR=5.633,95%CI:2.019-15.712),PCT(OR=1.045,95%CI:1.002-1.090)and CAR(OR=2.170,95%CI:1.616-2.915)were the independent influencing factor for the death on 28 d in the patients with SCAP.The ROC curve analysis indicated that AUC,sensitivity and specificity of PCT,NLR and CAR combined detection were superior to those of detection alone,having good predictive value.Conclusion PCT,NLR and CAR have a certain predictive value for death within 28 d in the patients with SCAP.The combined detection has better predictive efficiency.
10.Study on correlation between chest CT quantitative parameters and TCM constitution classification in adult persons with different BMI

Objective T o explore the correlation between adult chest CT quantitative parameters and traditional Chinese medicine(TCM)constitution classification in adult persons with different body mass index(BMI).Methods The basic information of 205 adult subjects in the hospital was collected,including gender,age,height,weight,BMI,etc.They were divided into the low body mass group(BMI<18.5 kg/mi2),normal body mass group(BMI 18.5-<24.0 kg/m2)and overweight/obese group(BMI≥24.0 kg/m2)according to BMI.All subjects underwent the TCM constitution classification.The distribution of TCM constitutions a-mong the three groups with different BMI was analyzed.The quantitative chest CT parameters were collected,including fat content,muscle content,area of the middle slice of the transverse section image,transverse diam-eter,anteroposterior diameter,scan length,etc.The differences were compared among different TCM constitu-tions,individual basic information and chest CT quantitative parameters.Results In the low BMI group,the Qi-stagnation constitution was the highest;in the BMI normal group,the gentler constitution was the highest,while in the overweight/obese group,the phlegm-damp constitution was the highest.The gender,age,height,weight and BMI had significant correlation with the TCM constitution(P<0.05).The fat content,fat vol-ume,muscle volume,area of transverse section ROI(AROI),transverse diameter,anteroposterior diameter and scan length/transverse diameter had significant correlation with the TCM constitution(P<0.05).The differ-ences in BMI and fat content had statistical difference among the phlegm-damp constitution,Qi-stagnation constitution and the other Chinese medicine constitutions(P<0.01).The fat content,fat volume and muscle volume had significant correlation with BMI(P<0.01).The muscle content had no significant correlation with BMI(P>0.05).The muscle content,scan length and transverse diameter/anteroposterior diameter had no significant correlation with the TCM constitution(P>0.05).Conclusion The distribution difference of TCM constitution in adult persons with different BMI has the statistical significance.The chest CT quantita-tive parameters are closely correlated with TCM constitution classification.
