1.The interaction between plasma aluminum concentration and schooling year on cognitive function

Objective:To explore the effect of the interaction between plasma aluminum concentration and schooling year on cognitive function, it provides the basis for early identification of its damaging effect.Methods:From October to December, 2014, 1114 on-the-job aluminum exposed workers in a large aluminum factory in Shanxi Province were investigated. The plasma aluminum concentrations were measured by inductively coupled plasma-mass spectrometry (ICP-MS) as an internal exposure indicator. The subjects were divided into low, medium, and high aluminum exposure group based on the blood lead concentration levels. The general information was collected with a self-designed questionnaire, and their cognitive function was evaluated with the mini-mental state examination and the clock drawing test. The interaction between plasma aluminum concentration level and education level was analyzed by using unconditional logistic regression fitting multiplication model and cross analysis fitting addition model.Results:The average level of plasma aluminum of the subjects was 21.68 (10.51, 45.78) μg/L. The detection rate of cognitive impairment was 19.9%. The influencing factors of cognitive function were plasma aluminum concentration, age, education level and marital status, among which high schooling year, and being married were the protective factors (all P<0.05), plasma aluminum concentration, and age were the risk factors (all P<0.05) ; After adjusting the confounding factors, such as age, marital status, schooling year, income level, smoking, and drinking, logistic regression model and the multiple linear regression model showed that there was a dose response relationship between plasma aluminum concentration and cognitive impairment. With the increase of plasma aluminum concentration, the score gradually decreased, and the risk of cognitive impairment increased ( Ptrend<0.05) ; There were multiplicative interaction ( AOR=2.15, 95% CI: 1.10-4.19) and additive interaction ( RERI=5.38, 95% CI: 2.60-8.16; AP=0.65, 95% CI: 0.51-0.80; S=3.88, 95% CI: 2.00-7.56) between plasma aluminum concentration and low schooling year on cognitive function. Conclusion:There was a dose-response relationship between aluminum exposure and cognitive impairment. There was an interaction between aluminum exposure and low schooling year. When both of them worked together, the risk of cognitive impairment increases.
2.Study on the application of immature platelet and thromboelastography in assessing the risk of hemorrhage after blood-poisonous snake bite

Objective:To explore the value of immature platelets and thromboelastography (TEG) in evaluating bleeding tendency in patients bitten by blood venomous snakes.Methods:In December 2023, a bleeding scoring system was used to grade the bleeding score and degree of bleeding in 147 patients with blood venomous snake bites. Immature platelets and TEG were also tested to analyze the correlation between the bleeding degree of patients and indicators such as TEG, immature platelet ratio (IPF), and absolute value of immature platelets (A-IPF). The measurement data were represented by Mean±SD, and ANOVA was used for comparison among groups. Statistical data were analyzed using χ2 test. Spearman correlation was used to analyze the correlation between bleeding degree and the platelet count (PLT), immature platelet and TEG. Pearson linear correlation was used to analyze the correlation of PLT, immature platelets and TEG. The indexes of immature platelets and TEG were analyzed by receiver operating characteristic curve. Results:The 147 patients were graded for bleeding severity based on their bleeding scores, and divided into mild (22 cases), moderate (119 cases), and severe groups (6 cases), and the clinical data such as age, gender and the time from bite to visit were compared among the three groups. The differences were not statistically significant ( P>0.05). The coagulation reaction time (R), clot formation time (K), and immature platelet parameters (IPF, A-IPF) in TEG parameters increased with the degree of bleeding. PLT decreased with the degree of bleeding, and the difference was statistically significant ( P<0.05). Blood venomous snake bite patients have a negative correlation between PLT and IPF ( r=-0.301, P=0.012), but no correlation with A-IPF ( r=0.072, P=0.461) ; PLT was positively correlated with coagulation Angle (αAngle) and maximum thrombus amplitude (MA) of TEG ( r=0.220、0.243, P=0.028、0.015), but negatively correlated with R value and K value ( r=-0.368、-0.362, P<0.001). IPF is positively correlated with R and K values ( r=303、271, P=0.0020、0.035) ; IPF is not correlated with MA value and Angle angle, while A-IPF is positively correlated with R value ( r=0.162, P<0.001). The degree of bleeding in patients is positively correlated with R value ( r=0.237, P<0.001), but not with K value, MA value, and Angle angle ( P>0.05) ; The degree of bleeding in patients is correlated with PLT ( r=-0.411, P<0.001). The ROC curve analysis results show that the R value, K value, and MA value are effective in evaluating the bleeding risk of blood venomous snake bites in patients, and the PLT combined with A-IPF has the best efficacy in evaluating the bleeding risk of blood venomous snake bites in patients. Conclusion:The combination of PLT and A-IPF, TEG's R value, K value, MA value indicators can accurately evaluate the bleeding risk of blood venomous snake bites, and can be widely used in clinical practice as an important prediction method for bleeding tendency in patients bitten by blood venomous snakes.
3.Investigation and analysis of the etiology of suspected occupational asthma caused by toluene diisocyanate

Objective:To conduct an occupational hygiene investigation and simulated sampling and testing on welding positions in a company in a certain province, and to perform a simulated analysis of the concentration of occupational hazard factors in workers suspected of occupational asthma.Methods:In November 2023, the investigation and analysis of workers' exposure to occupational hazard factors were carried out through occupational hygiene surveys, laboratory simulated sampling and testing, and a combination of qualitative and quantitative detection methods.Results:Toluene diisocyanate (TDI) was present in the workplace air of the employer. TDI was detected in the raw materials provided by the workers. Laboratory simulations of the production process and working conditions at the position were conducted with sampling at different distances, revealing a maximum TDI concentration of 0.49 mg/m 3 and a minimum concentration of 0.01 mg/m 3. The results showed that the closer the sampling distance was to the breathing zone, the more significantly the TDI concentration increased. The laboratory simulated sampling and testing results were highly consistent with the on-site sampling and testing results from a testing agency in Beijing. Conclusion:The occupational hazard factor TDI is present in the workers' workplace, and TDI may be the cause of the suspected occupational asthma in the workers.
4.The effects and mechanisms of PAD2 inhibitor AFM-30a attenuates pulmonary fibrosis in silicotic mice

Objective:To observe the effects of peptidylarginine deiminase 2 (PAD2) inhibitor AFM-30a on silicotic mice and its possible mechanisms.Methods:In May 2022, 40 SPF male C57BL/6J mice were randomly divided into control group, AFM-30a group, silicosis model group and AFM-30a treatment group, with 10 mice in each group. Silicosis model group and AFM-30a treatment group were perfused with silicon dioxide (SiO 2) suspension (10 mg/piece, 50 μl), and the other groups were perfused with an equal amount of sodium chloride solution. After 2 weeks, AFM-30a group and AFM-30a treatment group were intraperitoneally injected AFM-30a (20 mg/kg, 100 μl) daily, and mice of other groups were injected with equal amounts of sodium chloride solution for 4 weeks. Mouse RAW264.7 monocytes/macrophages were cultured in vitro and divided into blank control group, AFM-30a group (5 μmol/L), SiO 2 group (200 μg/ml), and SiO 2+AFM-30a group (200 μg/ml SiO 2 induction for 12 h, followed by 5 μmol/L AFM-30a treatment for 12 h). As well as blank control group, vimentin (Vim) group (2 μg/ml), citrullinated vimentin (Cit-Vim) group (2 μg/ml), and Cit-Vim+TLR4-C34 group (10 μmol/L TLR4-C34 treatment for 1 h, followed by 2 μg/ml Cit-Vim induction for 24 h). Hematoxylin Eosin (HE) and Masson staining were used to observe the pathological morphology of lung. The lung fieldclarity and lung texture of each group was observed by micro-CT. The number of positive cells was detected by tartrate resistant acid phosphatase (TRAP) staining. The localization and expression levels of PAD2, Cit-Vim, toll-like receptor 4 (TLR4) signaling and receptor activator of nuclear factor-κB ligand (RANKL) signaling proteins were measured by Immunofluorescence staining and Western blotting in vitro and in vivo. The experimental data were all presented as Mean±SD. A completely random design of one-way analysis of variance was used among the groups. The pduo comparison was performed using LSD test for homogeneity of variance and Tamhane's test for inconsistency. Results:Compared with the control group, the silicosis model group showed the formation of silicon nodules accompanied by collagen deposition, the silicosis model group showed thickened, and several high-density shadows of varying sizes in the lung field, and the number of TRAP positive cells in silicosis model group were increased significantly, the expression levels of PAD2, Cit-Vim, TLR4 and RANKL signal-related proteins were also significantly increased in silicosis groupmodel ( P<0.05). Compared with the silicosis model group, the AFM-30a treatment group reduced deposition of collagen in lung, and the number of TRAP positive cells was decreased in AFM-30a treatment group. The expression levels of PAD2, Cit-Vim, TLR4 and RANKL signaling related proteins were significantly decreased in AFM-30a treatment group ( P<0.05). In vitro, compared with the blank control group, the number of TRAP positive cells and the expression levels of PAD2, Cit-Vim, TLR4 and RANKL signaling related proteins in the SiO 2 group were significantly increased ( P<0.05). Compared with the SiO 2 group, the number of TRAP positive cells and the expression levels of PAD2, Cit-Vim, TLR4 and RANKL signaling related proteins in the SiO 2+AFM-30a group were significantly decreased ( P<0.05). Compared with the blank control group, the expression levels of TLR4 and RANKL signaling related proteins in the Cit-Vim group were significantly increased ( P<0.05). Compared with the Cit-Vim group, the expression levels of TLR4 and RANKL signaling related proteins in the Cit-Vim+TLR4-C34 group were significantly decreased ( P<0.05) . Conclusion:PAD2 inhibitor AFM-30a may play an antagonisticrole in silicotic fibrosis in mice by potentialregulating TLR4 and RANKL signaling pathways.
5.The role of ferroptosis in renal injury induced by diquat

Objective:To investigate the role of ferroptosis in renal cell injury induced by diquat (DQ) .Methods:From January to October 2022, human renal tubular epithelial (HK-2) cells were treated with DQ for 48 h, and different doses of ferroptosis inhibitors [deferoxamine (DFO), Fer-1] were added, and cells were harvested 24 h later. The experiment was divided into 6 groups ( n=6) : control group, DQ group (60 μmol/L), 20 μmol/L DFO (DFO-H) group, 10 μmol/L DFO (DFO-L) group, 5 μmol/L Fer-1 (Fer-1-H) group, 0.5 μmol/L Fer-1 (Fer-1-L) group. From December 2022 to June 2023, male C57bl/6 mice were selected to establish the animal model, and the experimental group was divided into 4 groups ( n=6) : control group, DQ group (25 mg/kg), DFO group (100 mg/kg) and Fer-1 group (2.5 μmol/kg). The changes of renal tissue were detected by HE staining. The fluorescence probe of ferrous ions was used to detect the change of iron ions in cells, and the colorimetric determination of total iron and ferrous ions in mouse kidney tissues was performed. Reverse transcription real-time quantitative polymerase chain reaction (qRT-PCR) and Western blotting were used to detect mRNA and protein expression changes related to ferroptosis signaling pathway. TUNEL staining was used to detect apoptosis. Enzyme-linked immunosorbent assay (ELISA) was used to detect the changes of antioxidant-related proteins and oxidative stress-related products. Differences among groups were analyzed by one-way analysis of variance. Results:In vitro test, compared with the control group, the iron ion level of HK-2 cells in DQ group was increased, the mRNA and protein expression levels of glutathione peroxidase 4 (GPX4), solute carrier family 7 member 11 (SLC7A11) and ferritin heavy chain (FTH) were decreased, the mRNA and protein expression levels of transferrin receptor 1 (TFR1) and divalent metal transporter 1 (DMT1) were increased, and the apoptosis level was significantly increased ( P<0.05). The expression levels of glutathione (GSH) and super oxide dismutase (SOD) in HK-2 cells in DQ group were significantly lower than those in control group ( P<0.05), and the expression levels of malondialdehyde (MDA) and reactive oxygen species (ROS) were significantly higher than those in control group ( P<0.05). Compared with DQ group, iron ion levels in HK-2 cells in the intervention groups of DFO and Fer-1 at different doses were decreased ( P<0.001), and GPX4, SLC7A11 and FTH mRNA and protein expression levels were increased, and the mRNA and protein expression levels of TFR1 and DMT1 were decreased in DFO-H and DFO-L groups ( P<0.05). The apoptosis levels in the intervention groups of DFO and Fer-1 at different doses were decreased compared with DQ group ( P<0.001), the expression levels of GSH and SOD were higher than those in DQ group ( P<0.05), and the expression levels of ROS were lower than those in DQ group ( P<0.05). In vivo, HE staining showed that the renal tissue of DQ group mice had obvious renal tubular epithelial cell injury with inflammatory cell infiltration. Compared with DQ group, DFO group and Fer-1 group had less damage of renal tubular epithelial cells and less inflammatory cell infiltration. Compared with the control group, the total iron content and ferrous iron content in kidney tissue of mice in DQ group were increased, the mRNA and protein expression levels of GPX4, SLC7A11 and FTH were decreased, the mRNA and protein expression levels of TFR1 and DMT1 were increased, and the apoptosis level was increased in DQ group ( P<0.05). The levels of GSH and SOD in DQ group were lower than those in control group, while the levels of MDA and ROS in DQ group were higher than those in control group ( P<0.05). Compared with DQ group, the total iron content and ferrous iron content in DFO group, and ferrous iron content in Fer-1 group were decreased ( P<0.001), the mRNA and protein expression levels of GPX4, SLC7A11 and FTH in kidney tissues of mice in DFO group and Fer-1 group were increased ( P<0.05), and the mRNA and protein expression levels of TFR1 and DMT1 were decreased ( P<0.05). The level of apoptosis in DFO group and Fer-1 group was lower than that in DQ group ( P<0.001). Compared with DQ group, the expression levels of GSH in kidney tissues, and the expression levels of SOD in serum and kidney tissues in DFO group were increased ( P<0.05), and the expression levels of GSH and SOD in serum and kidney tissues in Fer-1 group were increased ( P<0.05). The expression levels of MDA and ROS in serum and kidney tissues of DFO group and Fer-1 group were lower than those of DQ group ( P<0.05) . Conclusion:Ferroptosis may be involved in renal cell injury induced by DQ poisoning, and ferroptosis inhibitor may alleviate DQ-induced renal injury by inhibiting ferroptosis.
6.Influencing factors and risk analysis of anxiety and depression in occupational population

Objective:To identify and evaluate the important risk factor set of anxiety and depression in occupational population, establish a risk prediction model, and provide scientific basis for making targeted mental health protection plan and promoting the mental health of workers.Methods:In August 2016, a cluster random sampling method was used to investigate 807 employees who underwent physical examination in a hospital as research objects. The simplified Chinese version of the core job content questionnaire, Athens Insomnia Scale, AIS-5 and Symptom Check List-90 (SCL-90) were used for the Occupational stress, insomnai and negative emotional symptom investigation. Chi-square and Fisher exact probability method were used for data analysis, and Bayesian network was used for model construcion and analysis.Results:The score of occupational stress was 0.88±0.15, and the incidence of occupational stress was 18.09% (146/807). AIS-5 scores were (3.03±2.82), and the incidence of insomnia was 15.99% (129/807). Depression (16.89±5.73) scores, anxiety (12.36±4.11) scores. Depression (16.89±5.73) score, anxiety (12.36±4.11) score, the detection rate was 8.55% (69/755), 7.31% (59/762). Gender, illness, education, insomnia and occupational stress were correlated with depression ( P<0.01), while education, illness, insomnia and anxiety were correlated ( P<0.05). When both occupational stress and insomnia existed, the detection rate of depression was the highest (0.4006) . Conclusion:Insomnia was a valid predictor of anxiety and depression, suggesting that occupational groups should pay attention to sleep quality and managers should rationalize work tasks in order to reduce the risk of anxiety and depression.
7.Analysis of serum tumor marker levels in 629 patients with pneumoconiosis

Objective:To investigate the relationship between serum tumor markers and the severity of silicosis.Methods:In April 2024, A cluster sampling was conducted on the pulmonary rehabilitation centers in Jiangsu Province. A retrospective analysis was carried out on 629 patients who received follow-up physical examinations for pneumoconiosis from April to November 2023 at the selected rehabilitation centers. 281 healthy volunteers who underwent occupational health examinations at our institution were selected as the blank control group, and 384 dust exposed workers from a coal mining enterprise were selected as the dust exposed control group. Detect the levels of peripheral bloodtumor markers carcinoembryonic antigen CEA, carbohydrate antigen CA199, and neuron-specific enolase NSE in specimens. Measure the patient's forced vital capacity (FVC), forced expiratory volume in one second (FEV 1), and lactate acid dehydrogenase (LDH) levels in the blood. The Jonckheere-Terpstra test, a non-parametric test for ordered independent samples, was used to compare the expression levels of tumor markers among multiple groups. Spearman's rank correlation analysis was adopted to explore the correlations between serum tumor markers and the staging of pneumoconiosis as well as lung function parameters. Results:Among the 629 patients with pneumoconiosis, 454 were in stage Ⅰ, 150 were in stage Ⅱ, 25 were in stage Ⅲ. There were significant differences in serum CEA, CA199 and NSE levels among patients with different periods of pneumoconiosis, blank control group and dust-exposed control group ( P<0.001). Serum CEA, CA199 and NSE levels were positively correlated with pneumoconiosis stage ( P<0.05). There is a negative correlation between serum CA199 levels and FEV 1% in patients with pneumoconiosis ( r=-0.082, P<0.05). The serum CA199 level was positively correlated with LDH in patients with pneumoconiosis ( r=0.108, P<0.05) .The serum NSE level of pneumoconiosis patients was negatively correlated with FVC% ( r=-0.079, P<0.05) .There is a positive correlation between serum NSE levels and LDH in patients with pneumoconiosis ( r=0.435, P<0.05) . Conclusion:The serum CA199 and NSE of pneumoconiosis patients are directly related to the severity and stage of the disease, which have practical significance in judging the treatment effect of pneumoconiosis patients and monitoring the development of the disease.
8.Ultrasonic-assisted alkaline dilution-inductively coupled plasma mass spectrometry for the determination of total bromine in whole blood

Objective:To establish an inductively coupled plasma mass spectrometry (ICP-MS) method for the rapid determination of total bromine in whole blood, and to provide technical support for the monitoring of bromine exposure in occupational populations.Methods:In September 2023, 0.25 ml of whole blood sample was added with 0.25% tetramethylammonium hydroxide solution to a volume of 5.0 ml, ultrasonicated and homogenized, and then quantified by ICP-MS with rhodium solution as the online internal standard solution.Results:The linear relationship of bromine in blood was good within the range of 0~10.00 mg/L, with a correlation coefficient ( r) >0.999. The limit of detection of the method was 0.07 mg/L, and the quantification limit was 0.22 mg/L. The recoveries of the total bromine in whole blood were in the range of 95.5%-102.9%, and the relative standard deviations (RSDs) were in the range of 3.1%-4.7% ( n=7) . Conclusion:An ICP-MS method was established for the rapid determination of total bromine in whole blood, which is accurate, simple, rapid, highly automated, and can be used for the determination of total bromine in whole blood of occupationally exposed people.
9.Study on the current situation and influencing factors of uncivilized behavior in workplace encountered by nurses in Xianyang City

Objective:To investigate the current situation of uncivilized behavior in workplace encountered by nurses in Xianyang City, and analyze its influencing factors, so as to provide a basis for formulating corresponding intervention measures.Methods:In May 2023, convenience sampling was used to investigate the general information, current situation of uncivilized behavior in workplace and coping style of 1310 nurses in 8 secondary and tertiary general hospitals in Xianyang City by using the general information questionnaire, the Chinese version of the Nurse Incivility Scale and the Simplified Coping Style Questionnaire. Multiple linear regression analysis was used to analyze the influencing factors of uncivilized behavior encountered by nurses.Results:A total of 1285 valid questionnaires were collected, with an effective recovery rate of 98.09%. The total score of uncivilized behavior encountered by nurses was (86.41±22.49) points. Among them, the scores of five dimensions were nurses' uncivilized behavior (19.95±4.70) points, patients' uncivilized behavior (23.66±8.96) points, supervisors' uncivilized behavior (13.25±4.87) points, doctors' uncivilized behavior (14.32±5.63) points and colleagues' uncivilized behavior (15.23±4.93) points. The multiple linear regression analysis results showed that the hospital level and coping style were the significant influencing factors of uncivilized behavior in workplace encountered by nurses ( β=0.104, 0.056; P<0.05) . Conclusion:The current situation of uncivilized behavior in workplace encountered by nurses needs to be improved. Hospital managers should focus on coping styles, such as setting up simulation scene exercises, team cooperation training, training and counseling, so as to reduce the uncivilized behavior in workplace encountered by nurses.
10.Study on the pollution and influencing factors of legionella pneumophila in industrial circulating cooling tower

Objective:To investigate the pollution status of legionella pneumophila of industrial circulating cooling towers, and explore the factors affecting the positive rate and quantity of legionella pneumophila, in order to provide data support for the risk assessment and hierarchical management of legionella pneumophila in workplaces.Methods:In February, May, August and October 2023, the cooling water samples were collected from a total of 20 industrial circulating cooling towers of 9 enterprises in an industrial park in Shanghai. The water temperature, free residual chlorine, total residual chlorine, turbidity, chroma, pH, conductivity, chemical oxygen consumption, iron content and total plate count in the water samples were detected. Meanwhile, the legionella pneumophila in the water samples were qualitatively and quantitatively analyzed, and the influence of water quality indexes on the positive rate and number of legionella pneumophila detected was analyzed by logistic regression and linear regression respectively.Results:The positive rates of legionella pneumophila in the first to fourth quarters of 20 industrial circulating cooling towers of 9 chemical enterprises were 35% (7/20), 45% (9/20), 45% (9/20), and 85% (17/20). The serotypes of legionella pneumophila detected in the cooling tower were mainly legionella pneumophila type 1 (95.2%, 40/42), and the number of legionella pneumophila ranged from 0.2 to 316.8 cfu/ml. Circulating cooling water temperature ( OR=1.36, 95% CI: 1.12-1.64, P=0.001) and conductivity ( OR=1.02, 95% CI: 1.00-1.03, P=0.026) were independent risk factors affecting the detection of legionella pneumophila. There was a positive correlation between chemical oxygen consumption and the detected amount of legionella pneumophila ( β=8.08, P=0.002) . Conclusion:The positive rate of legionella pneumophila in industrial circulating cooling water system is high, and the detected quantity fluctuates greatly, which may have health risks, suggesting that it is necessary to pay attention to the water quality management such as water temperature, conductivity and chemical oxygen consumption of industrial circulating cooling water and the monitoring and control of legionella pneumophila.
