1.Expert recommendations on vision friendly built environments for myopia prevention and control in children and adolescents

Abstract
The prevention and control of myopia in Chinese children and adolescents has become a major public health issue. While maintaining increased outdoor activity as a cornerstone intervention, there is an urgent need to explore new complementary approaches that can be effectively implemented in both indoor and outdoor settings. In recent years, environmental spatial frequency has gained increasing attention as one of the key environmental factors influencing the development and progression of myopia. Both animal studies and human research have confirmed that indoor environments lacking mid to high spatial frequency components, often characterized as "visually impoverished", can promote axial elongation and myopia through mechanisms such as disruption of retinal neural signaling, impaired accommodative function, and altered expression of related molecules. Based on the scientific consensus, it is recommended that "enriching of environmental spatial frequency" should be integrated into the myopia prevention and control framework. Following the principles of schoolled organization, family cooperation, community involvement, and student participation, specific measures are put forward in three areas:optimizing school visual settings, improving home spatial environments, and promoting healthy visual behavior. The aim is to create "visually friendly" indoor environments as an important supplement to outdoor activity, thereby providing a novel perspective and strategy for comprehensively advancing myopia prevention and control among children and adolescents.
2.The Impact of Emotional Release Technique Combined with the PERMA Model Intervention on the Fear of Disease Progression in Patients after Liver Cancer Surgery

Objective To explore the effects of Emotional Freedom Techniques combined with PERMA model intervention on fear of disease progression and quality of life in postoperative liver cancer patients.Methods A total of 58 liver cancer patients admitted from September 2023 to February 2024 were randomly divided into the experimental group(n=29)and the control group(n=29)using a random number table method.The experimental group received Emotional Freedom Techniques combined with PERMA-based positive psychological intervention,while the control group received the holistic nursing and routine psychological guidance.Both groups underwent the continuous intervention for 8 weeks.Outcomes were assessed using the Fear of Progression Questionnaire-Short Form(FoP-Q-SF),Distress Thermometer(DT),Self-Rating Anxiety Scale(SAS),Self-Rating Depression Scale(SDS),Cancer Fatigue Scale(CFS),Athens Insomnia Scale(AIS)and Index of Well-Being(IWB).Results After the intervention,the experimental group showed the significantly lower scores than those in the control group in FoP-Q-SF(24.97±2.34/34.10±8.46),DT(2.24±0.95/2.93±1.33),SAS(49.55±1.35/58.55±4.60),SDS(51.86±1.58/61.79±4.00),CFS(4.03±0.94/5.10±1.18),and AIS(2.45±1.12/4.07±0.96)(all P<0.05),while the IWB score(10.56±1.74/7.39±2.05)was significantly higher than that in the control group(P<0.05).Conclusion The combination of Emotional Freedom Techniques and PERMA model intervention can effectively alleviate the disease-related fears,improve the negative emotions,reduce the physical symptoms and enhance the subjective well-being in postoperative liver cancer patients,and it has a significant clinical application value.

Objective To investigate the factors that influence ocular surface biology and visual acuity in individuals with diabetic dry eye(DDE)and analyze how these factors contribute to changes in visual quality.Methods Based on the disease duration,fasting blood glucose(FBG),and glycated hemoglobin(HbA1c)levels of patients with type 2 diabe-tes mellitus(T2DM),the DDE patients were divided into different groups.Logistic regression analysis was used to identify influencing factors related to ocular surface biology and visual quality in each group of DDE patients.Tear film stability was evaluated based on the tear film rupture time(BUT),Schirmer I test(SIt),and ocular surface disease index(OSDI).Lip-iview? Surface interferometers were used to measure tear film lipid layer thickness(LLT),meibomian gland loss rate(MGP),meibomian gland opening number(MGYLS),and meibomian gland secretion score(MGYSS).Wavefront aber-rometry was used to measure corneal wavefront aberration values at 4 mm and 6 mm pupil diameters.Ocular response ana-lyzer(ORA)was adopted to analyze corneal hysteresis(CH)and corneal resistance factor(CRF).Moreover,ELISA ex-periment to evaluate the trend of changes in inflammatory factors in tears.Results Logistic regression analysis revealed that T2DM duration,smoking history,FBG,HbA1c,total cholesterol(TC),triglycerides(TG),OSDI score,LLT,BUT,SIt,MGP,MGYLS,MGYSS,total higher-order aberrations,spherical aberration,coma aberration,trefoil aberration,tumor necrosis factor-α,interleukin-6,matrix metalloproteinase-9,receptor for advanced glycation end products,and insu-lin were all influencing factors for the risk of DDE(all P<0.05).As the T2DM course prolonged and FBG or HbA1 c levels rose,tear film-related indicators(LLT,BUT,and SIt)and meibomian gland-related indicators(MGYLS and MGYSS)inpa-tients gradually decreased,while OSDI scores and MGP gradually increased(all P<0.05).As the T2DM course prolonged and FBG or HbA1c levels rose,the total higher-order aberrations,spherical aberration,coma aberration,and trefoil aber-ration in DDE patients under 4 mm and 6 mm pupil diameters gradually increased;Meanwhile,best corrected visual acuity,corneal hysteresis,and corneal resistance factor gradually decreased;The contents of tumor necrosis factor-α,interleukin-6,matrix metalloproteinase-9,receptor for advanced glycation end products,and insulin in tears all gradually increased,while mucin-5AC gradually decreased(all P<0.05).Conclusion With the prolongation of T2DM duration and the in-crease of FBG or HbA1c,the ocular surface inflammatory response in DDE patients gradually worsens,corneal biological function decreases,and visual quality deteriorates.Timely systemic and local interventions are of great significance for im-proving dry eye symptoms and visual quality in DDE patients.
5.Application value of synthetic magnetic resonance imaging in predicting cervical lymph node metastasis of oral cancer

Objective·To explore the value of synthetic magnetic resonance imaging(SyMRI)technology in the diagnosis of cervical lymph node metastasis in patients with oral cancer.Methods·A retrospective analysis was conducted on patients admitted to Shanghai Ninth People's Hospital,Shanghai Jiao Tong University School of Medicine,from November 2023 to April 2024,who were pathologically diagnosed with oral cancer and had a clear lymph node metastasis status.Pre-operative maxillofacial magnetic resonance images of these patients were collected.Histogram features were extracted and screened from the quantitative maps generated by SyMRI,including synthetic T1,T2,and proton density(PD)maps,apparent diffusion coefficient(ADC)maps,and volumes of interest(VOIs)from contrast-enhanced images.The optimal range was determined by comparing the histogram parameters of different peritumoral regions.On this basis,habitat analysis was further carried out by combining the quantitative maps and ADC maps,and the habitat features of the tumor invasive sub-regions were extracted to construct a prediction model.The performance of the model was comprehensively evaluated using receiver operator characteristic curves(ROC curves),net reclassification improvement(NRI),integrated discrimination improvement(IDI),and decision curve analysis(DCA).Results·A total of 61 patients with oral cancer were included in the study.The area under the curve(AUC)value for predicting cervical lymph node metastasis based on the intratumoral histogram features extracted from SyMRI was 0.798(95%CI 0.673?0.924).Adding the ADC map could increase the AUC value to 0.818(95%CI 0.635?0.861).By combining the histogram features from the 12-mm peritumoral region with habitat features,the AUC value for predicting lymph node metastasis could be further increased to 0.907(95%CI 0.812?0.993).The analysis results of NRI,IDI,and DCA all showed that the predictive performance of the model was better than that of clinical diagnosis.Conclusion·Based on SyMRI,combining the histogram features from the intratumoral and peritumoral regions with habitat features shows high performance in the prediction of lymph node metastasis in oral cancer,providing a feasible,contrast agent-free approach for rapid imaging and prediction of metastatic lymph nodes.
6.The value of synthetic MRI combined with reduced field of view intravoxel incoherent motion diffusion weighted imaging in preoperative predicting TN stage of rectal cancer

Objective To explore the feasibility and value of synthetic MRI combined with reduced field of view(r FOV)intravoxel incoherent motion diffusion weighted imaging(IVIM-DWI)in preoperative predicting TN stage of rectal cancer.Methods The ima-ging and clinical data of 40 patients with rectal cancer confirmed by operation and pathology were collected and divided into low T stage group(T1-T2 stage)and high T stage group(T3-T4 stage),N0 stage group and N1-N2 stage group according to postoperative pathological staging as the golden standard.Independent sample t-tests were conducted to compare the parameter differences of synthetic MRI[T1,T2,and proton density(PD)values]and IVIM-DWI(D,D*,and f values)between the two groups.Receiver operating charac-teristic(ROC)curves were used to evaluate the efficacy of each parameter with statistically significant differences.Results Signifi-cant differences were observed in the T2 values of synthetic MRI and the D and f values of IVIM-DWI between high and low T stage groups,as well as between N0 and N1-N2 stage groups(P<0.05).The D value showed the highest area under the curve(AUC)(AUC=0.888)in predicting T stage group,and the T2 value was predominant(AUC=0.790)for N stage group prediction.The combination models of T2,D,and f values yielded superior predictive capability for TN stage in preoperative predicting rectal cancer,with AUC of 0.890 and 0.807,respectively.Conclusion Synthetic MRI combined with r FOV IVIM-DWI is feasible in preoperative prediction of TN stage of rectal cancer,which shows a higher efficacy,and is a useful supplement to conventional MRI technology.
7.Mechanism of airway remolding in a mouse model of chronic obstructive pulmonary disease induced by cigarette smoke combined with Klebsiella pneumoniae

Objective Cigarette smoke(CS)exposure combined with Klebsiella pneumoniae(KP)infection in mice was used to establish a model of chronic obstructive pulmonary disease(COPD)to investigate the mechanism of airway remodeling.Methods Male BALB/c mice were randomly divided into a Control group,CS group,KP group,and CS+KP group.The mice were exposed to CS,KP,and CS+KP from weeks 1 to 8,and were sacrificed in weeks 4,8,16,and 24.MV,Penh,MLI,MAN,and changes in lung pathological structure were detected.The expression levels of IL-1β and TNF-α in lung tissue were detected by ELISA.Collagen deposition was observed by Masson staining and immunohistochemistry.α-SMA and TGF-β1 expression in lung tissue was detected by immunofluorescence.Human bronchial epithelioid cells(16HBE)were also stimulated by CS and lipopolysaccharide(LPS)in vitro,and the expression levels of airway epithelial junction proteins,autophagy-related protein,and mTOR signaling proteins were detected.Results Compared with the Control group,the CS+KP group mice had significantly decreased MV from weeks 4 to 24(P<0.05 or P<0.01)and significantly increased Penh from weeks 8 to 24(P<0.05 or P<0.01);while the CS group had markedly decreased MV and markedly increased Penh from weeks 8 to 16(P<0.05 or P<0.01).Compared with the Control group,massive inflammatory cell infiltration,alveolar wall thickening,alveolar rupture and fusion,and airway wall thickening were observed by HE staining in CS+KP group from weeks 4 to 24.The CS+KP group mice had significantly decreased MAN and significantly increased MLI,IL-1β and TNF-α in their lung tissue from weeks 4 to 24(P<0.05 or P<0.01).The aforementioned inflammation and tissue damage were observed in the CS group and the KP group from week 8 to 16.Compared with the Control group,COL Ⅰ,COL Ⅲ,α-SMA,and TGF-β1 were significantly increased in lung tissue of mice in the CS+KP group from weeks 8 to 16(P<0.01);COL Ⅰ was significantly increased in the CS group and KP group from weeks 8 to 16(P<0.01).In addition,increased E-cad and decreased N-cad(P<0.05);significantly decreased LC3B and Beclin-1(P<0.05);and significantly increased p-mTORC1,p-P70-S6K,and p-4E-BP1 expression were observed in 16HBE cells exposed to CS and LPS(P<0.05 or P<0.01).Conclusion Pulmonary functional decline,pathological changes in lung tissue,and airway remodeling appeared to occur early and persist in COPD mice induced by CS and KP.The mechanisms may be related to the activation of mTORC1 signaling pathway and subsequent inhibition of autophagy.
8.CT and MRI findings of mucosa-associated lymphoid tissue lymphoma of salivary gland

Objective To explore the CT and MRI manifestation of salivary gland mucosa-associated lymphoid tissue(MALT)lymphoma.Methods The preoperative CT and MRI images of 89 patients with salivary gland MALT lymphoma who were confirmed by surgical pathology were analyzed retrospectively.Results Salivary gland MALT lymphoma were mainly located in the parotid gland(64 cases,71.9%).The ratio of male to female was 1 ≤ 5.Sixty-nine cases(77.5%)showed concomitant Sjogren's syndrome(SS).Typical imaging findings were round or lobulated well-marginated solid masses with moderate to severe enhancement.Atypical imaging findings were all from parotid gland MALT lymphoma,including cyst-solid mixed masses(34 cases,38.2%),poorly marginated boundaries(9 cases,10.1%),and punctuate calcification(5 cases,5.6%).The average apparent diffusion coefficient(ADC)value of lesions was(0.59±0.09)× 10-3 mm2/s and the time-signal intensity curve(TIC)type was mainly type Ⅱ(66.7%).Conclusion Salivary gland MALT lymphoma exhibits certain clinical and imaging characteristics.But when occur in parotid gland,some lesions may present atypically,which require diagnosis with the aid of functional MRI.
9.Application potential of adaptive window width and level algorithm in 3D vascular reconstruction from head and neck CT angiography

Objective To explore the application value of adaptive window width and level algorithm in 3D vascular reconstruction from head and neck CT angiography for achieving consistent reconstruction results at different contrast agent dosages.Methods Both direct vascular reconstruction and reconstruction after applying adaptive window width and level algorithm were conducted in 100 cases undergoing head and neck CT angiography;and the reconstruction results were compared.Results In the cases with excessive or insufficient contrast agent,the overall effect of reconstructions using adaptive window width and level algorithm was superior to that of direct reconstruction,particularly in displaying intracranial small vessels and plaques.Conclusion The adaptive window width and level algorithm has high application potential in vascular reconstruction from head and neck CT angiography,and demonstrates superior generalization in highlighting plaques,adapting to different contrast agent dosages,and displaying intracranial small vessels.
