Academic publishing is at a critical juncture. The challenges faced by the academics are mired in controversy. Among theseare three hotly debated concerns. First is the issue of whether technological innovations such as artificial intelligence (AI)improves research efficiency or if its use sacrifices research integrity.Another is the controversy between paywall publishingand open access. Lastly, adapting an appropriate business model for sustainability is a contentious issue and the choice betweena commercial or a university-based publishing platform is a difficult one.
Traditional models of scientific investigation relied on tedious intellectual calisthenics in all aspects of research —identifying research gaps, reviewing of published literature, devising valid methodology, collecting data, analysing results, and,finally, drawing conclusions. With the advent of powerful tools employing artificial intelligence, these heavy tasks are efficientlycarried out. The dilemma lies in determining which parts of the work can be attributed to the authors and which are ascribedto the output of large language models (LLMs) and other automated assistance employed.Despite requiring adequate vettingby experts of these AI-aided output, many in the scientific community still question these methods. Can research employingAI be considered honest work? Will full disclosure answer doubts as to the integrity of the scientific work?
Indeed, LLMs just gather information that is already out there, albeit more efficiently. After all, science progresses bystanding on the shoulder of giants. AI makes such work comprehensive and efficient. Standing on those proverbial shoulders,however, require access to prior work, hence our next challenge in academic publishing--open access versus paid access.Paywalls limit the benefits of valuable research to institutions and universities with the capacity to pay. Excluded from these arethose from low resourced countries, with nations from the global south being affected disproportionately. Additionally, whilenumerous authors appreciate the features of open access as it improves their impact and visibility, many feel unduly burdenedsince the cost of publishing in this format is passed on to them.
This brings us to our third issue: who bears the cost of academic publishing? Indeed, it is a lucrative industry, generatingan annual revenue of US$19 billion and an estimated 40 percent profit margin. Many, however, find fault in this businessmodel as concerns about the profit motives of the commercial publishers far overshadow their sustainability goals.
How do we navigate this landscape of controversies? We, at the ACTA, as part of the community of scholars, would needto clarify our mission. Our goals for this publication should be consistent with our values. These values, such as scientific rigor,integrity, and accountability, should be reflected in our policies. We should be cognizant of the role we play in national scientificdiscourse while we endeavor to make an impact in the global scene. We are accountable to our stakeholders — nurturingearly career scholars, supplying evidence to health policymakers, and being accountable to those who provide resources tosustain us. This stewardship is essential so that ACTA will stand shoulder to shoulder with the giants on which science buildsupon to benefit future generations.

3.Relationship of work stress and dysglycemia among healthcare workers doing shift work in a level 1 government hospital in Antipolo City

INTRODUCTION
This cross-sectional study investigated the relationship between work stress and dysglycemia among healthcare workers engaged in shift work at a level 1 government hospital in Antipolo City, Philippines.
METHODSWork stress was assessed using the Effort-Reward Imbalance (ERI) Questionnaire, and dysglycemia was measured through a 75g oral glucose tolerance test (OGTT).
RESULTSA total of 126 healthcare workers aged 20–55 years participated Results showed that 65.1% of participants experienced work stress, with nurses reporting the highest prevalence (43.9%). Dysglycemia was present in 33.3% of participants, predominantly in the form of impaired glucose tolerance. Despite the high prevalence of both work stress and obesity (61.9%), no statistically significant associations were found between work stress and dysglycemia (p = .51), gender and work stress (p = .59), occupation and work stress (p = .059), or obesity and dysglycemia (p = .70).
CONCLUSIONThe findings suggest that while work stress is common among healthcare workers, especially nurses, it may not directly predict dysglycemia in relatively young and active populations. However, the long-term metabolic risks associated with chronic occupational stress should not be overlooked. Future longitudinal studies with larger samples are recommended to better assess causality and guide workplace wellness programs.

4.Assessing the impact of the COVID-19 pandemic on social service accessibility and experiences for persons with disabilities in the Philippines during pandemic peak

OBJECTIVES
This study investigates the impact of the COVID-19 pandemic on social service accessibility for persons with disabilities in the Philippines, with a focus on disability related opportunity costs and out-of-pocket expenses.
METHODOLOGYA mixed-methods approach was employed, combining both qualitative and quantitative data collection techniques. Focus group discussions (FGDs) and interviews were conducted to gather qualitative insights, while quantitative analysis, utilizing one-sample t-tests, was used to assess the impact of the pandemic on key areas such as income, access to essential goods, employment, healthcare, mental health, and social support.
RESULTSUsing a mixed-methods approach, data were collected from 141 participants. Qualitative insights highlighted that 89% of participants reported negative effects on daily living conditions due to rising costs and limited access to necessities. Parents of children with disabilities indicated that early childhood care and development services were adversely affected by school closures, and transitioning to online learning posed significant barriers, with 62% lacking access to training and resources. While many participants had access to healthcare, lockdowns exacerbated disability-related opportunity costs, complicating access to routine care. Quantitative data findings were analyzed through one-sample t-tests to assess the impact of COVID-19 on various aspects of daily life, including income, access to essential goods, employment status, healthcare access, mental health, and social support. Findings revealed significant challenges, with an average income decrease rating of 7.39, reflecting substantial financial loss. Ratings for access to essential goods and healthcare services also averaged 7.39, while employment status was rated at 7.82, indicating instability. Mental health impacts averaged 7.13, and social support was rated at 7.42, underscoring inadequacies during this crisis.
CONCLUSIONOverall, the study emphasizes the urgent need for targeted interventions and inclusive responses in emergency planning to mitigate the pandemic's disproportionate impact on this marginalized group in the Philippines.

5.Exploring the lived experiences of working female nursing students in a private university in Ho Chi Minh, Vietnam: A phenomenological study

INTRODUCTION
For female nursing students in Vietnam, juggling work, school, and personal obligations can be especially difficult. Research on how these students develop resilience while juggling their multiple roles is still lacking, despite the fact that their numbers in nursing school are increasing. This study explores how the work-life-study balance (WLSB) of female students pursuing an Associate Degree in Nursing (ADN) to a Bachelor of Science in Nursing (BSN) program is shaped by their real-life experiences and sociocultural influences.
METHODSA qualitative research design informed by interpretative phenomenological analysis (IPA) was used in this study. In September 2024, ten carefully selected female nursing students participated in semi-structured interviews at a private university in Ho Chi Minh City. From October 2024 to February 2025, each 45–60 minute interview was subjected to a thematic analysis using Delve software.
RESULTSThe challenges faced by the participants, along with their support systems, coping strategies, and aspirations, were captured in four key themes, each with its own set of sub-themes. The first theme, Navigating Life's Crossroads: The Struggle for Balance, highlighted the students' struggles to manage competing demands, featuring subthemes, Pulled in All Directions, Time as a Scarce Commodity, and Compromises and Sacrifices. The second theme, Anchors in the Storm: Finding Strength in Support, emphasized the vital role of relational support, showcasing subthemes, Peer Solidarity and Shared Struggles, and Family as a Pillar of Strength. The third theme, Pathways to Resilience: Strategies for Survival, focused on coping strategies and adaptive techniques, incorporating subtheme, Faith and Inner Strength, Embracing the Role of a Working Learner, and Prioritizing and Organizing. Finally, the last theme, Purpose, Aspiration, and Future Orientation, brought attention to the participants' sources of motivation and their optimistic outlook, with subthemes, Motivation Rooted in Family and Self and Hope and Optimism as Sustaining Forces. These findings, grounded in the Transformative Resilience Model, illustrate how students harness their inner drive, familial and social responsibilities, and cultural values to adapt and thrive in the face of challenges. To maintain their dedication to education and uplift their families, participants leaned on hope, spiritual insights, and a sense of agency, viewing their struggles as meaningful experiences.
CONCLUSIONThe experiences of Vietnamese female nursing students reveal a remarkable resilience shaped by both heavy social expectations and personal challenges. Drawing from the Transformative Resilience Model, this study highlights how facing and overcoming adversity can lead to significant identity development and personal growth. Institutional support plays a crucial role in enhancing a student's well-being, which can include flexible academic policies, accessible mental health services, and adaptable work-study options. Financial pressures, job-related stress, and academic demands often contribute to burnout. These findings underscore the urgent need for systemic, collaborative efforts to foster inclusive and sustainable learning environments for nursing students who are balancing work and study.

7.A temporary trauma team established in primary hospital for disaster rescue.

OBJECTIVE:
To explore the feasibility of establishing a temporary trauma team led by trauma experts in primary hospitals for disaster medical rescue.
METHODS:
In the coal mine flooding accident in Xiaoyi City, Shanxi Province on December 15, 2021, according to the local emergency plan and the characteristics of the accident, the trauma experts trained the medical staff from the local primary hospital on advanced trauma life support (ATLS) and damage control surgery (DCS) in the short time interval between the occurrence of the mine disaster and the admission of medical staff to the disaster scene. A temporary trauma team composed of trauma experts, ATLS team, and DCS team was formed to provide early diagnosis and treatment for survivors before and in the hospital.
RESULTS:
The miners were found on the 36th hour of the disaster. All 22 miners were male, and 2 died underground. Another 20 people were rescued 39-43 hours after the disaster, with a median age of 48 years (34-57 years). All the survivors suffered from hypothermia, dehydration, maceration of feet and other injuries. There were 18 cases of acute inhalation tracheobronchitis, 14 cases of electrolyte acid-base disturbance, 6 cases of trunk contusion, 1 case of psoas major hematoma, and 1 case of lower extremity hematoma. Deep vein thrombosis was in 4 cases. The ATLS team focused on injury assessment, rewarming and rehydration within 50-60 minutes before admission, and completed auxiliary examinations within 2 hours after admission to clarify the diagnosis. The DCS team evaluated 6 patients with mechanical blunt trunk injury and excluded the indication of emergency surgery. The trauma experts conducted the whole process of supervision and quality control of disaster rescue. The positive rate of capillary refill test in the all survivors at the third hour of admission was significantly lower than that immediately after being rescued (75.0% vs. 15.0%, P=0.000 3), and they were discharged 4-7 days after admission.
CONCLUSION
Under the leadership of trauma experts and relying on the medical staff of primary hospitals, it is feasible to establish and train a temporary trauma team with ATLS and DCS functions to participate in the medical rescue of disasters, which is in line with the current national conditions of China.

8.Circadian disruption by simulated shift work aggravates periodontitis via orchestrating BMAL1 and GSDMD-mediated pyroptosis.

Approximately 20% to 30% of the global workforce is engaged in shift work. As a significant cause of circadian disruption, shift work is closely associated with an increased risk for periodontitis. Nevertheless, how shift work-related circadian disruption functions in periodontitis remains unknown. Herein, we employed a simulated shift work model constructed by controlling the environmental light-dark cycles and revealed that shift work-related circadian disruption exacerbated the progression of experimental periodontitis. RNA sequencing and in vitro experiments indicated that downregulation of the core circadian protein brain and muscle ARNT-like protein 1 (BMAL1) and activation of the Gasdermin D (GSDMD)-mediated pyroptosis were involved in the pathogenesis of that. Mechanically, BMAL1 regulated GSDMD-mediated pyroptosis by suppressing NOD-like receptor protein 3 (NLRP3) inflammasome signaling through modulating nuclear receptor subfamily 1 group D member 1 (NR1D1), and inhibiting Gsdmd transcription via directly binding to the E-box elements in its promoter. GSDMD-mediated pyroptosis accelerated periodontitis progression, whereas downregulated BMAL1 under circadian disruption further aggravated periodontal destruction by increasing GSDMD activity. And restoring the level of BMAL1 by circadian recovery and SR8278 injection alleviated simulated shift work-exacerbated periodontitis via lessening GSDMD-mediated pyroptosis. These findings provide new evidence and potential interventional targets for circadian disruption-accelerated periodontitis.

9.Teamwork, moral sensitivity, and work engagement among employees in a government higher education institution.

BACKGROUND
Although higher education institutions (HEIs) are expected to contribute much in the larger communities, it cannot be denied that the employees have the qualities and characteristics with genuine interest to help the institution, other individuals, and the communities surrounding them. Studying teamwork, moral sensitivity, and work engagement is essential in determining how they affected with the organizational management, training of students, and achieving the goals of the college.
OBJECTIVESThe objective of this study was to determine the level of teamwork, moral sensitivity, and work engagement and the relationship between them among faculty members, school administrators, and supporting staff in a government HEI in the Philippines.
METHODSA cross-sectional design was employed involving 49 employees in a government HEI. Three self-report scales were used in gathering the data: TeamSTEPPS Teamwork Perceptions Questionnaire, Ethical Sensitivity Scale, and Utrecht Work Engagement Scale. Data were analyzed using Spearman’s rho correlations, point biserial correlations, and eta correlations.
RESULTSParticipants reported high levels of perceived teamwork, moral sensitivity, and work engagement. Marital status was significantly and positively correlated with moral sensitivity and work engagement, whereas employment status was significantly and positively correlated with teamwork and moral sensitivity. Moreover, moral sensitivity was significantly and positively correlated with teamwork and work engagement.
CONCLUSIONSCollege employees who reported high levels of moral sensitivity are associated with high levels of teamwork and work engagement. Strategies to maintain and enhance teamwork, moral sensitivity, and work engagement are recommended through various activities such as team-building workshops, staff development, and/or leadership trainings.

10.Predictors of nurses' organizational commitment in crisis: Evidence from Saudi Arabia.

BACKGROUND
Organizational commitment strengthens care delivery and system responsiveness, yet high turnover and workforce exhaustion threaten healthcare stability. Understanding factors that sustain commitment is especially critical during crises such as the COVID-19 pandemic.
AIMThis study examined how nurses' attitudes toward their practice environment predict organizational commitment at King Abdullah Medical City (KAMC), Makkah, Saudi Arabia.
DESIGNAcross-sectional predictive correlational design was used.
METHODSData was collected from 275 staff nurses between October and November 2020 using convenience sampling. Attitudes toward the practice environment were assessed with the Practice Environment Scale of the Nursing Work Index (PESNWI), while organizational commitment was measured using the Revised Three-Component Model Employee Commitment Survey. Descriptive statistics, Pearson's correlations, and multiple regression were conducted in SPSS version 21.
RESULTSNurses reported positive work attitudes, with the highest ratings for nursing care and interdisciplinary relationships, followed by management and leadership, and the lowest for adequate resources. Organizational commitment was moderate across affective, normative, and continuance domains. Regression analyses identified nursing care and interdisciplinary relationships as the most consistent predictor across all domains of commitment, including affective (p < .001), normative (p = .047), continuance (p = .004), and overall organizational commitment (p < .001). Adequate resources significantly predicted affective (p = .014) and overall commitment (p = .050), while management and leadership predicted normative commitment (p = .016).
CONCLUSIONNurses' perceptions of their practice environment significantly shape organizational commitment during crisis conditions. Interdisciplinary collaboration consistently emerged as the strongest predictor, underscoring the value of teamwork and nurse–physician partnerships. Adequate staffing and resources also reinforced commitment, particularly in sustaining affective bonds. These findings underscored that supportive practice environments, through collaboration, adequate resources, and participatory leadership, enhance nurses' autonomy and sense of meaningful work, which in turn strengthen organizational commitment, retention, and resilience during crises.
