BACKGROUND

Burn injuries remain a significant global public health problem, causing substantial morbidity, mortality, and economic burden, particularly in low- and middle-income countries where nearly 90% of cases occur. Despite the importance of timely and appropriate first aid in reducing complications, community responses to burn injuries in many settings continue to rely on traditional or non-evidence-based practices.

OBJECTIVES

This qualitative phenomenological study explored community-based knowledge and responses related to the causes and first aid of burn injuries.

METHODS

The study involved ten (10) purposively selected informants residing in South Mangga Dua Urban Village, Central Jakarta, Indonesia. Data collection methods included semi-structured interviews, direct observations, and focus group discussions (FGDs). Thematic analysis was used.

RESULTS

Three (3) core domains were identified: (1) community perceptions of burn causes and classifications, (2) indigenous first aid practices used in domestic settings, and (3) sources of knowledge and information pathways related to burn first aid. Participants commonly attributed burns to incidents involving fire, hot liquids, and electrical faults. Their understanding of burn severity was limited to superficial assessments, with little awareness of clinical classifications. First aid responses were largely based on traditional practices such as the application of toothpaste, honey, or aloe vera, while evidencebased practices like using running water were rarely mentioned. Notably, most participants relied on familial teachings and informal community experiences as their primary sources of knowledge, with limited exposure to health professionals or verified media content.

CONCLUSION

Community knowledge is culturally rooted but misaligned with medical standards, potentially leading to unsafe practices. Culturally sensitive health education integrating traditional beliefs and accurate information is essential to improve outcomes in burn injury management.

Background: The Community Health and Development Program (CHDP) is a longstanding program of the University of the Philippines (UP) Manila, School of Health Sciences, Extension Campus in South Cotabato. While its service goes beyond teaching and extends to community development projects benefiting indigenous sectors, yet there has been no comprehensive and systematic evaluation on the program's health communication materials which promote culturally safe and acceptable health care practices that lead to normative changes. Objective: Using descriptive-interpretive qualitative approach, the researcher identified the communication approaches and documented their health knowledge, attitude, and practices (KAP) to identify any normative changes. Methodology: A total of 24 respondents gave their views on the program's dengue, malnutrition, teenage pregnancy, and family planning campaigns using a closed-ended survey, qualitative KAP analysis, and ethnovideography. Results: Results showed that information dissemination is limited to printed media and interpersonal communication via small groups; contents of health communication campaigns are less understandable; and social mobilization efforts demonstrated low levels of community engagement. Also, communication between health workers and Blaan indigenous peoples is not transactional due to the lack of feedback and involvement in the decision-making process. The respondents' knowledge on health demonstrated low levels of end-user engagement reflective of unsustainable knowledge on modern medicine. Normative changes are only evident on family planning campaigns. Conclusion The long-term sustainability of health promotion among indigenous peoples may be inhibited due to non-evident normative changes that trigger societal transformation. The paper suggests that a specialized communication strategy intended for Blaan indigenous peoples must be considered and applied in order to react collectively and effectively to address health issues.

Background: Health information systems (HIS) such as Electronic Medical Record (EMR) systems are essential in the integration of fragmented local health systems. Investing in HIS is crosscutting; it can address multiple interrelated health system gaps. However, public health authorities, especially those in resource-constrained communities, are often faced with the dual challenge of upgrading and digitalizing local HIS and addressing other more apparent health system gaps. Objectives: The study aimed to identify and document strategies that not only motivate policy change towards adoption of electronic HIS but also address other health system gaps. Methodology: The author, in his capacity as a local health official in a resource-constrained community, developed, implemented, and documented a social marketing strategy wherein community stakeholders were influenced to invest in an electronic medical record (EMR) system because it was shown to also have the capacity to address other priority health system gaps identified. Results: The strategy, based on situational, stakeholder, and risk analyses, prompted local governance to first invest in improving the delivery of services accredited by the national health insurance program (PhilHealth), for which reimbursements would require electronically submitted claim forms. Community stakeholders then supported the proposal to invest in an EMR system because they were persuaded that it can facilitate increased financing from PhilHealth claims reimbursements, which could be used to enable not only improvement in existing health services but to also initiate other health programs. Conclusion Social marketing using the perspective of health as an investment influenced stakeholders to invest in an EMR system.

INTRODUCTION: Amid the COVID-19 pandemic, many rumours have emerged. Given prior research linking rumour exposure to mental well-being, we conducted a nationwide survey to document the base rate of rumour exposure and factors associated with rumour vulnerability. METHODS: Between March and July 2020, 1,237 participants were surveyed on 5 widely disseminated COVID-19 rumours (drinking water frequently could be preventive, eating garlic could be preventive, the outbreak arose because of bat soup consumption, the virus was created in an American lab, and the virus was created in a Chinese lab). For each rumour, participants reported whether they had heard, shared or believed each rumour. RESULTS: Although most participants had been exposed to COVID-19 rumours, few shared or believed these. Sharing behaviours sometimes occurred in the absence of belief; however, education emerged as a protective factor for both sharing and belief. CONCLUSION Our results suggest that campaigns targeting skills associated with higher education (e.g. epistemology) may prove more effective than counter-rumour messages.

PURPOSE: To estimate the impact of a health information exchange (HIE) pilot project on ophthalmology department care. METHODS: Study sites included 4 ophthalmic clinics in three regions participating in the HIE pilot project (group A), and 12 clinics with similar distances and numbers of patient referrals as group A but who were not participating in the HIE pilot project (group B). The mean wait time, total medical costs, and ophthalmic examinations of referral patients were analyzed. RESULTS: The mean wait times were 8.4 ± 8.0 days in group A, which included 83 patients, and 11.7 ± 15.4 days in group B, which included 417 patients. The wait time was significantly shorter in group A (p = 0.005). Sensitivity analyses also indicated shorter wait times in group A. In 247 patients in group B who were referred to tertiary referral hospitals automatically through the conventional clinical cooperation center with group A, the wait times were 8.4 ± 8.0 and 7.7 ± 8.8 days, respectively, and the total cost of medical care was 260.6 ± 271.4 and 257.0 ± 251.7 thousand Won, respectively. No differences in these factors were found between the groups (p = 0.503, 0.913, respectively). There were no significant differences in participation in the HIE pilot project regarding ophthalmic examinations conducted within 2 weeks since patient referral (p > 0.050 for all). CONCLUSIONS: The HIE is advantageous because it results in shorter wait times to see an ophthalmologist, due to the automatic referral method based on medical records. However, there are no benefits in reducing total costs of medical care or the number of clinical examinations.

Non-technical skill (NTS) in gastrointestinal surgeon is an important part of surgical performance and surgical education. NTS is essential for safe and effective surgery. NTS has four aspects: situation awareness, decision making, communication and teamwork, and leadership. There is evidence that training and assessment of NTS of professional physicians is very important for doctors' career and plays an important role in improving medical quality and ensuring patient safety. The training and assessment of NTS is a great reference to the training of gastrointestinal surgeons in China. In addition, the establishment of a training system for professional gastrointestinal surgeons in line with China's characteristic as soon as possible will improve the overall quality of professional gastrointestinal surgeons in China and better serve the vast number of patients.

BACKGROUND: The persuasive effect of health messages can depend on message features, audience characteristics, and target behaviors. The objective of this study was to compare the responses to persuasive messages encouraging professional help seeking for depression between individuals with and without psychological distress. METHODS: A cross-sectional web-based survey was conducted on Japanese adults aged 35-45 years, who randomly received one of three persuasive messages that aimed to promote help-seeking intentions for depression. The primary message statements were as follows: (1) depression can happen to anyone, (2) depression needs treatment, and (3) depression improves with treatment. Participants rated the messages in terms of comprehensibility, persuasiveness, emotional response, and intended future use. Help-seeking intention for depression was measured using vignette methodology before and after exposure to the messages. Eligible participants who had not received medical treatment for their mental disorders were classified as either distressed (K6 score ≥ 5, N = 824) or non-distressed (K6 score < 5, N = 1133) and analyzed. RESULTS: No significant differences in comprehensibility or persuasiveness scores were observed between the messages, but the distressed group had significantly lower scores than the non-distressed group. Negative emotional responses such as surprise, anger, fear, sadness, guilt, and anxiety were significantly stronger when reading message 2, while a positive emotional response such as happiness was significantly stronger when reading message 3. These emotional responses were more prominent in the distressed than in the non-distressed group. After reading messages 1, 2, and 3, the proportions of participants in the distressed group who reported having a positive help-seeking intention increased by 35.1%, 32.1%, and 27.7%, respectively, and by 6.4%, 17.3%, and 15.2%, respectively in the non-distressed group. Multiple logistic regression analysis among participants having no help-seeking intention before exposure to the messages showed that message 2 had a significantly greater effect of increasing help-seeking intentions in the non-distressed group. CONCLUSION: The exposure to persuasive messages may promote help-seeking intentions for depression. It seems likely that loss framing will work better than neutral and gain framing. Meanwhile, the responses to persuasive messages may differ to some extent between distressed and non-distressed individuals, as individuals with psychological distress are likely to be more susceptible to persuasive messages than those without. BACKGROUND: As Koreans adopt more Westernized diets, consumer demands for processed food products are growing. The Korean government implemented a food labeling system to help people reasonably choose processed foods. This study investigated the utilization of these food labels among Koreans, as well as demographic differences between users and non-users of food labels, ultimately presenting foundational data for strategies to enhance dietary lifestyles by facilitating the use of food labeling. METHODS: Data from the 2014–2017 Community Health Survey were used. The Cochran-Armitage trend test was performed to investigate whether food labeling awareness and utilization increase over time; a data mining technique called decision tree analysis was then used to examine the effects of sociodemographic factors on the use of food labeling for each of the 4 years. RESULTS: Food labeling awareness increased by 4.5% over 4 years; the Cochran–Armitage trend test showed that this increase was statistically significant (χ2 = 1,068.21; P < 0.001). Food labeling utilization increased significantly but marginally, by 0.3%, over 4 years (χ2 = 20.154; P < 0.001). Decision tree analysis showed that food labeling users comprised mainly women in their 30s–50s currently in graduate school or with a graduate degree; the non-user group comprised mainly divorced/widowed/separated people in their 70s who were elementary school grads or below. CONCLUSION: Education level, which could be related to health literacy, substantially affected the use of food labeling. Education level is a typical socioeconomic index, and if people who are less educated tend not to use food labeling, it can lead to health inequity. Thus, food labeling should be reformed with consideration of health literacy among the less-educated social classes. From a health communication perspective, images and pictures can boost consumers' understanding, which should be considered to improve comprehensibility of food labeling.
